The cause of primary spontaneous pneumothorax (PSP) is obvious. Recently, the FLCN mutation was suggested to be a causal factor in PSP. A 47-year-old Korean male patient with chief complaint of repetitive PSP had numerous emphysematous bullae and multiple large cysts based upon high resolution computer tomography. Here we report a case of PSP with an FLCN c.468_470delTTC mutation.

5.Influence of the Size of the Spoon on the Eating rate, Energy Intake and the Satiety Levels of Female College Students.

OBJECTIVES: This study examined the influence of different sizes of spoons (normal spoon, 8.3 cc vs small spoon, 4 cc) on eating rate, energy intake and the satiety levels of female college students. METHODS: Twenty four healthy female college students participated in this study once a week for 2 weeks. Two hundred ten grams of cooked rice and 250 g of beef shank soup with a normal spoon and same amount of rice and soup with a small size spoon were served to the same participants over two consecutive weeks. After each lunch, the eating rate, energy intake, and the satiety levels were measured. RESULTS: Results showed that the subjects who were using a small spoon ate less beef shank soup (149.0 kcal) (p < 0.01) and had lower total energy intake (423.3 kcal) (p < 0.05) than using a normal spoon (178.7 and 461.1 kcal, respectively). Also, the meal time (15.7 min) (p < 0.01), a serving per one spoon (8.6 g) (p < 0.001), and eating rate (27.9 g/min) (p < 0.001) of those who used a small spoon were significantly different than that of those who used a normal spoon (13.6 min, 12.5 g and 35.7 g/min, respectively). However, despite consuming less energy at lunch, the level of satiety after eating from the small spoon was not significantly different from the normal spoon immediately after, 1 hour after and 2 hour after lunch. CONCLUSIONS: Our results revealed that students were able to control their eating rate by using a small spoon and they could feel full enough even though they eat less. In conclusion, eating rate decrease by using a small spoon may play an important role in food intake.

6.Tuberculous Epididymo-orchitis in Acquired Immune Deficiency Syndrome Patients.

The urogenital tract manifestations of acquired immune deficiency syndrome (AIDS) include the entire array of voiding symptoms; infections, neurogenic bladder problems, impotence, infertility, neoplasma and AIDS associated renal syndromes. Tuberculosis occurs in approximately 4% of patients with AIDS, and involves at least one extrapulmonary site in nearly 50% of cases. However, tuberculous infection of the testicle in AIDS is rarely seen. Herein, we report a case of tuberculosis epididymoorchitis, which developed as a result of reactivation in an AIDS patient. To the best of our knowledge, this is the first reported case of AIDS presenting as tuberculous epididymoorchitis in Korea.

7.A Case Study Related to Identifying the Driver of Motorcycle.

It is a frequently asked question who was the driver of a vehicle when a traffic accident occur. To give an expert opinion who was the driver of vehicle, expert witness must inspect not only trace evidences remained in vehicle and occupant's clothing but also occupant's injuries. Thus, the collaboration of forensic engineering and forensic medicine is very fruitful in this kind of case. Due to open sitting position of occupants, deciding the driver of motorcycle is a very difficult and case dependent problem and it is very rare that the driver of motorcycle can be decided only by the estimation of occupant's injuries. In this report, we introduce the case that the driver of motorcycle can be decided by the collaboration of forensic engineering and forensic medicine.

8.Therapeutic Efficacy of Extracorporeal Magnetic Therapy in Chronic Pelvic Pain Syndrome.

PURPOSE: A physical therapy, with extracorporeal magnetic innervation (ExMi) treatment, has recently substituted direct electrical stimulation for the treatment of many diseases. The efficacy of ExMi treatment was evaluated as a new therapeutic modality in chronic pelvic pain syndrome. MATERIALS AND METHODS: In 30 patients, diagnosed as NIH type III prostatitis, the efficacy of an ExMi was evaluated. Each treatment session was composed of 10 min at 40Hz, 2 min rest and 10 min at 50Hz, with an on-off ratio of 5sec:5sec. Both before and after the completion of 8 sessions of ExMi treatment, self-administered (NIH-CPSI) questionnaire were recorded to assess the changes of patient's symptoms. Three months after treatment, any recurrence was evaluated in patients with symptom improvements of 30% or more by telephone interviews. RESULTS: The mean age of the patients and number of treatment sessions were 39.3 years and 10.5, respectively. Following the ExMi treatments, 20 of the 30 patients (66.7%) experienced a 30% or more improvement of their symptoms. The total NIH-CPSI score was significantly improved, from 23.7 to 15.2 (p<0.01). Each domain of the NIH-CPSI was improved as follows: pain, from 11.37 to 6.77; urination, from 5.03 to 3.27 and the effect on the QOL (quality of life), from 7.27 to 5.17. In responding patients, the improvement remained in 72% of the patients for 3 months after completion of the treatment. CONCLUSIONS: These results suggest that ExMi is an effective and useful tool in the management of chronic pelvic pain syndrome. In chronic pelvic pain syndrome patients, ExMi treatment has advantages over other treatment options due to its easy, comfortable, non-invasive nature.

9.Acute Urinary Retention by Leiomyoma of the Bladder Neck.

Bladder leiomyoma is a very rare tumor, and only 0.43% of bladder tumors are of this type. Bladder leiomyoma may cause various symptoms depending on its anatomical location and size. The most common voiding symptoms manifested by leiomyoma are the obstructive symptoms. We report a case of a 42 year old woman who had voiding symptoms for 3 years while visiting the outpatient department of genito-urology due to acute urinary retention. We performed ultrasonography, computed tomography, MRI and transurethral resection, and from these test, the pathologic diagnosis revealed a leiomyoma of bladder.

10.A Case of Cesarean Scar Endometriosis.

Extrapelvic endometriosis is a fairly rare phenomenon. The majority of extrapelvic endometriosis involves scar tissue following obstetric or gynecologic procedures. Abdominal wall endometriosis secondary to cesarean section is a very rare condition, being reported in less than 0.5% of patients undergoing cesarean section. It has a distinct presentation and treatment. An abdominal mass with noncyclical symptoms is a common presentation. Imaging techniques are nonspecific and needle biopsy may confirm the diagnosis. Wide excision is the treatment of choice for abdominal wall endometriosis as well as for recurrent lesions. A patient with a history of cesarean section presented with a painful, enlarging mass. The pain was cyclic and aggravated just prior to menstruation. The patient was treated with surgical scar excision pathologically confirmed as endometriosis. We present this case with a brief review of literature.
