1.Genetic Polymorphism in E7 Gene of Human Papillomavirus Type 16 Isolated from Uterine Cervical Cancer in Korean Women.

Human papillomavirus (HPV)infection are now generally accepted as the most important factor for development of uterine cervical cancer and its precursor lesions. With increasing evidences that the HPV E7 encodes for oncoproteins critical for viral replication, host cell immortalization and transformation. Based on the previous reports that the high risk HPV type 16 DNA is frequently detected in specimens from Korean women with cervical cancer and that there is the sequence variation and geographical dependence of HPV 16 E7 gene in preinvasive and invasive cervical lesions, it is crucial to determine the prevalence of HPV 16 variants in uterine cervical lesions of Korean women. This study was performed to identify sequence variations of HPV 16 E7 gene and an association between HPV 16 E7 variants and uterine cervical cancer. The author has determined nucleotide sequences of the E7 gene of HPV 16 isolated from uterine cervical tissues in Korean women. HPV 16 DNAs were detected by the nested PCR in 112 (24.5%) of a total of 457 samples. By direct sequencing of PCR-HPV 16 E7 positive cases, 79 samples (70.5%) showed variant sequences, while the prototype sequence was found in only 33 samples (29.5%). Twenty-three cases (57.5%) of 40 normal cervical samples showed sequence variation. Forty-eight (77.4%) of 62 cervical cancer cases showed sequence diversity from prototype HPV 16 E7 gene. There were four types of sequence variations. A single nucleotide change at position 647 (A-->G) was found in 52 cases (65.8%) of 79 HPV 16 E7 variants. Predicted amino acid change (Asn -->Ser) was found in the HPV 16 E7 oncoproteins at amino acid position at 29. And this KE7-1 variant was commonly detected in the uterine cervical cancer compared to the normal cervix. The second most common variant, detected in 16 cases (20.3%), had three silent mutations at nucleotide positions 732 (T-->C), 789 (T-->C) and 795 (T-->G). The third variant had a single nucleotide change at position 666 (G-->A), and the fourth had a change at position 796 (T-->C). Furthermore, PCR-SSCP clearly showed distinct bands compatible with HPV 16 E7 variants as with the direct-sequencing method. PCR-SSCP was also an effective and reliable tool in detecting HPV 16 E7 variants. This study showed that there were four variant types of HPV 16 E7 in uterine cervical tissues and KE7-1 with corresponding amino acid change was the most commonly detected type in E7 variants of HPV 16 isolated from uterine cervical cancer in Korean women.

4.A Case of Intraorbital Ectopic Lacrimal Gland with Lacrimal Duct.

Ectopic lacrimal gland denotes a lacrimal gland which is located anywhere other than its normal site, lacrimal fossa, and it can occur in places like conjunctiva, cornea, lid, nasal mucosa, and orbit. It is a rare congenital anomaly and needs to be differentiated form other tumors. The literature contains few reports of ectopic lacrimal gland. We experienced a case of a 28-year-old Korean man who presented with an ectopic lacrimal gland with duct at temporal aspect of bulbar conjunctiva. The glandular tissue was excised and was histopathologically confirmed as such. We report it with reviews of the literature concerned.

5.Treatment of Spinal Tuberculosis Associated with Neurological Symptom

The result of interference with the conductibility of the spinal cord is one of the most disabling and distressing complications of splnal tuberculosis. This fact was recognized as early as in 1779, when Pott published. The antituberculous drugs and improved surgical treatment have made possible a more effective treatment than before and improved the prognosis but not solved all problems completely. The following results were shown by analysis of 63 cases of spinal tuberculosls with neurological symptom experienced in the Department of Orthopedic Surgery, Seoul National University in the past 9.5 years from Jan. 1970 to Jun. 1979. 1. Number of cases In child was 15. (29%). 2. Pulmonary and other tuberculous diseases were manifested in 29 cases (46%). 3. Regions of the spine inovolved were most common In thoracic spine (43 cases), especially lower thoracic spine. 4. There were 49 early and 14 late paraplegia 5. There were 24 complete and 39 incomplete paraplegia. 6. Spastic type was more common than flaccid type. 7. Anterior fusion was done in 55 cases and costotransversectomy in 8. 8. There were 36 cases (57%) of complete recovery and 8 of fallure. 9. The prognosis for recovery from paraplegia appeared to depend on age, duration, type and onset of paraplegia.

Pure, nongestational ovarian choriocarcinomas is extremely rare. Most ovarian choriocarcinoma are combined with other malignant germ cell tumors or can arise as a metastaais from a primnry gestational choriocarcinoma. We experienced a case of primary ovarian choriocarcinoma that probably was associated with a past history of the mixture of germ cell tumor and present it with a review of literature.

8.Are the elderly patient’s changes in the health‑related quality of life one year after gastrectomy for stomach cancer different from those in young patients?

Purpose:
Gastrectomy for elderly patients can significantly deteriorate the health-related quality of life (HRQoL). There was no report comparing HRQoL of elderly patients with young patients after gastrectomy for gastric cancer. This study assessed the differences in the changes of HRQoL at one year after gastrectomy according to age.
Methods:
From May 2014 to Feb 2016, we prospectively enrolled patients undergoing gastrectomy for gastric cancer. They completed the European Organization for Research and Treatment of Cancer and gastric questionnaires preoperatively and at postoperative 1, 3, 6, 9, and 12 months.
Results:
We included 57 elderly patients (≥70 years old) and 74 younger patients. The elderly had similar demographic, surgical, and pathological characteristics with young patients except that elderly had more comorbidity, laparoscopic gastrectomies, and lesser postoperative chemotherapy. One month after gastrectomy, the score of global health status/ quality of life, physical, role, and social functioning were significantly impaired in elderly patients. Among them, physical and role functioning were more impaired than those of young patients. The scores of physical functioning, role functioning, cognitive functioning, and social functioning were not fully recovered till 1 year after surgery. There was a significant age group difference in the changes in physical function over the 1-year follow-up.
Conclusion
Elderly patients’ global health status/quality of life and social functioning significantly decreased at postoperative 1 month and recovered by 6 months after gastrectomy. There was a significant age-specific difference in physical functioning throughout the 1-year follow-up. Surgeons need to pay more attention to recovery of the elderly patients’ HRQoL after gastrectomy.
9.Angiographic Embokization in the Control of Bleeding Related to Gynecologic Malignancy.

The use of angiographic embolization gas been well described for the control of gynecologic bleeding. Recently, we have experienced the successful embolization in two case of serons bleeding related to giynecologic malignancy. Clinical evaluation revealed a gynecologic cancer with active bleeding. Becaause we judged that conservative management was unable to cuntrol the bleeding, we underwent an angiographic embolization. The result presented here indicates that angiographic embolization is a relatively safe, effective and rapid procedure that should be considered early in management of gynecohogic cancer bleeding that is unresponsive to conservative management.

Tuberculous spondylitis is not rare disease. Today the posterior instrumentation and posterior or posterolateral fusion concomitant with the anterior decompression and anterior interbody fusion have been used for the treatment of spinal tuberculosis. The authors experienced a case of tuberculous spondylitis complicated with descending aortic pseudoaneurysm. An aneurysm is defined as a localized dilatation of an artery that is at least one-half the size greater than is expected for that artery. Pseudoaneurysm occurring after previous operation, trauma, and infection. Erosion of the thoracic aorta with the development of an fistula in the presence of infection is an unusual and difficult problem to manage. We report a case and review related articles briefly.
