1.A Clinical Observation on Benign Prostatic Hyperplasia.

A clinical study was made on 117 cases of benign prostatic hyperplasia admitted to the Department of Urology, Chungnam University Hospital during the period from January 1974 to August 1982. The results were summarized as follows: 1. Common symptoms were dysuria (82.1%) and urinary retention (41.0%). 2. Gross or microscopic hematuria was noticed in 48 cases (41.0%). 3. The degree of prostatic enlargement by rectal palpation was not related to the severity of symptoms. 4. Pulmonary disease (19.5%) and cardiac disease (12.8%) were frequently associated with B.P.H. 5. Treatment consisted of suprapubic of retropubic prostatectomy in 94 cases, TUR in 7 cases and cystostomy in 10 cases. 6. Postoperative complications were urinary incontinence (34%), wound problem (9.7%), delayed bleeding (8.7%), and urinary tract infection (4.8%). 7. Mean weight of removed prostatic tissue was 31.3gm.

2.Clinical usefulness of morphine skin prick test in diagnosis of allergic diseases.

BACKGROUND: Few studies have demonstrated an enhanced skin responsiveness to opiates in atopic subjects. OBJECTIVE: To determine whether the skin response to morphine is increased in atopics and to assess the clinical usefulness of morphine skin prick test in diagnosis of allergic diseases. METHOD: Allergy skin prick tests were performed using 55 common allergens, histamine, and morphine in 158 patients with allergic diseases. RESULTS: Wheal and flare sizes for morphine (1mg/mL) were significantly related to and smaller than those for histamine (1mg/mL). Although the proportion of subjects with allergic rhinitis and the level of serum total IgE were not different between responders (wheal >- 2mm) and nonresponders to morphine, the positive response rate to allergens was significantly lower in nonresponders. The flare sizes for morphine were significantly higher in positive allergen test group (A/H ratio >- 0.5). Among positive allergen test group, the subjects with atopy score >- 5 showed a larger flare size for morphine than those with atopy score < 5 while the sizes for histamine were not different. CONCLUSION: Morphine skin prick test is helpful for detecting false negative responses to allergens, and morphine skin test responses are increased in highly atopic patients probably due to enhanced mast cell releasability.

3.The Effectiveness and Safety of DA-3030 ( rhG-CSF ) for Chemotherapy - induced Neutropenia: A Randomized Controlled Trial.

PURPOSE: We investigated the effectiveness and safety of DA-3030 for prophylatic use in patients receiving chemotherapy for malignant disease. MATERIALS AND METHODS: Seventy cancer patients were randomized to receive chemotherapy alone (36 patients) or with DA-3030 administered (34 patients) after stratified block randomization according to chemotherapeutic regimen. DA-3030 was subcutaneously administered at the dose of 100 pg/m/day for 10 days from 24 hours after the completion of chemotherapy. RESULTS: Of the 70 enrolled patients, 62 patients were evaluable. The neutropenia (absolute neutrophil count [ANC] <1,000/mm) occurred in 9 of 32 (28.1%) of the DA-3030 group and 21 of 30 (90.0%) of the control group, giving relative risk for control group of 0.154 (95% confidence interval [CI], 0.05 to 0.45; p-0.0001). Severe neutropenia (ANC 500/mm') occurred in 4 of 32 (12.5%) of the DA-3030 group and in 20 of 30 (66.7%) of the control group (relative risk for control group of 0.316 [95% CI, 0,18 to 0.55]; p=0.0001). The mean duration of neutropenic period (+/-standard error) was 1.13+/-0.34 days in the DA-3030 group and 6.73+/-0.69 days in the control group respectively, and was significantly shorter in the DA-3030 group (p<0.0001). And, there was higher nadir ANC in the OA-3030 group than that in the control group (p=0.0001); the mean nadir ANC was 2,547+/- 343/mm and 442+/-120/mm, respectively. The DA-3030 group had significantly higher incidence of myalgia in comparison to the control group (43.8% compared with 3.3%; p=0.001). However, it was tolerable and was easily managed by conservative therapy CONCLUSION: The use of DA-3030 was effective in preventing chemotherapy-induced neutropenia.

4.Recent updates on extranodal NK/T-cell lymphoma.

Extranodal NK/T-cell lymphoma (ENKL) is a rare subtype of NHL (Non-Hodgkin lymphoma]. It occurs mostly in the nasal and paranal areas. Most of the cases are presented stage I/II. International prognostic index (IPI) can predict the outcome. However, better prognostic model is available such as NKIPI. Because of high expression of p-glycoprotein, ENKL is refractory to chemotherapy. Early stage disease can be bestly treated with concurrent chemoradiation. For advanced stage disease, new clinical trials are now being conducted.

5.The Effect of Additional Factor XIII on Cross-linking in Fibrin Glue.

To investigate the effect of additional factor XIII on the rate of cross-linked fibrin formation in fibrin glue, we prepared concentrated fibrinogen as a source of fibrin glue using thaw -syphon method from fresh frozen plasma, purified human factor XIII from plasma, and performed biochemical analysis of the fibrin glue formed with or without additional factor XIII. It was concluded that additional factor XIII promoted rapid formation of cross-linked fibrin (gamma-gamma dimer and a-polymer) which would be essential to clinically effective fibrin glue.

6.Evaluation of the Third-generation Enzyme Immunoassay and Confirmatory Test for Anti-HCV.

The ultimate goal of blood donor screening for anti-hepatitis C virus(HCV) antibodies is the specific exclusion of vital carriers from the blood donor population. Recently, a third-generation anti-HCV screening (Lucky HCD 3.0) and immunoblot assay (Lucky Confirm) using antigens derived from the core and different nonstructural regioris (NS3, NS4 and NS5) of the HCV vital genome were developed. To evaluate the usefulness of these assays, anti-HCV reaction patterns of the RIBA-2 and the presence of HCV-RNA detected by reverse transcriptase-polymerase chain reaction(RT-PCR) were examined in 180 sera, which were repeatedly positive in Abbott EIA-2, and HCV seroconversion panel sera. The reaction intensity of HCD 3.0 was higher than that of HCD 2.0. The sensitivity and positive predictive value for vital carrier state of HCD 3.0 were 98.4% and 85.4%, respectively. HCD 3.0 assay enabled the detection of the antibody response 2 weeks earlier than did other second-generation EIAs. RT-PCR testing of sera with RIBA-2-indetermihate results showed that 33.3%(10/30) had evidence of HCV-RNA. However, all of nine Lucky Confirm-indeterminate cases were negative for HCV-RNA. The sensitivity and specificity of Lucky Confirm test were 99.2% and 76.4%, respectively, and the positive and negative predictive values were 90.5% and 97.7%, respectively.

7.B lunted dyspnea perception in severe asthmatics.

BACKGROUND: Blunted perception of dyspnea, which may be related to the increased mortality, has been demonstrated in patients with a history of near-fatal asthma and in the elderly. OBJECTIVE: The purpose of this study was to evaluate the relationship between the perception of dyspnea and the severity of asthma. METHOD: Baseline spirometry and Borg score change(A Borg score) during breathing through an inspiratory muscle trainer were measured in 27 consecutive asthma patients and 11 normal subjects. RESULTS: The baseline Borg score was negatively related to FEV1 A Borg score was significantly lower in asthmatics than in controls at high level of loads. A Borg score was lower in severe asthma than in mild asthma. A Borg score was positively related to the baseline FEV1. Seventy-five percent of severe asthma, 62.5% of moderate one, and 9.1% of mild one showed impaired dyspnea perception. Dyspnea perception was related to age(r, = -0.49, p<0.001). CONCLUSION: Three quarters of patients with severe asthma showed impaired dyspnea perception to inspiratory resistive load. Dyspnea perception was related to asthma severity and age.

8.EFFECTS OF SQUARE-PULSE STIMULATION ON FUNCTIONAL RECOVERY FOLLOWING A CRUSH INJURY TO THE SCIATIC NERVE IN RATS.

Effects of square-pulse stimulation on the functional recovery following a nerve injury were examined in rats. a crush injury was made on the sciatic nerve under ketamine anesthesia and square-pulse stimulation with 5 - 7 V, 0.2 ms, 20 Hz, and 5 sec ON-OFF type was applied to the lesion site through teflon coated stainless steel wires(0.45mm in diameter) for 4 hours per day up to 2 days. Motor evoked potential(MEP) of the tibial nerve and somatosensory evoked potential (SSEP) of the T12 spinal cord were recorded. And their threshold, amplitude and latency were then analyzed. 1. Threshold of MEP was 41+/-5 uA, and that of SSEP was 39+/-13 uA. 2. MEP was composed of 3 waves, i.e., N1, N2, N3, in which Nl was conducted by A alpha fibers and the others by the synaptic fibers from the spinal cord. 3. SSEP was also composed of 3 waves, in which N1 was conducted by IA, N2 to II, and to III fibers. 4. MEP and SSEP were abolished immediately after crush injury and SEsP was more sensitive, N1 disappeared faster than N2 and N3. 5. Evoked potentials were abolished by application of local anesthetics, in which N2 and N3 were more sensitive than Nl. 6. Electrical stimulation decreased thresholds evoked potentials down to 50% in MEP, and 30% in SSEP. 7. Electrical stimulation increased amplitudes of evoked potentials and decreased latencies following the injury. These results suggest that square-pulse stimulation is effective in promoting the functional recovery following a nerve injury, Study of MEP and SSEP may be useful parameters to evaluate the nerve function following an injury.

9.A Study of Effect on Sperm Motility by Some Bacteria.

This study was carried out to investigate the effect of sperm motility to some bacteria which were possible cause of infertility by genitourinary tract infection. Human spermatozoa, from 42 cases of healthy medical student, were exposed to E. coli (30 cases) and Staphylococcus aureus (12 cases) varying dilutions from 10(4), 10(5), 10(6) colonies/ml of normal saline and broth solution. Spermatozoal numerical motility scale devised by Emmens was checked at the serial time of hours in Vitro. The following results were obtained: 1. Sperm motilities of control groups among pure semen, semen with normal saline, semen with broth solution and semen with broth extract contained endotoxin were no significant differences. 2. A significantly decreased sperm motility ensued in E. coli experimental groups than control group. And the more concentrated bacteria the more significant decrease of sperm motility was observed. 3. There were no significant differences between control group and experimental groups in Staphylococcus aureus.
