3.Percutaneous Microwave Coagulation Therapy combined with Transcatheter Arterial Chemoembolization for Hepatocellular Carcinoma: Preliminary Results.

PURPOSE: To evaluate the efficacy of percutaneous microwave coagulation therapy (PMCT) combined with transcatheter arterial chemoembolization (TACE) for the treatment of hepatocellular carcinoma (HCC). MATERIALS AND METHODS: Twenty-five nodular HCCs [long diameter of 1.2 -10 (mean, 3.4) cm] in 19 patients (15males and 4 females) were treated by PMCT 4-138 days after TACE. Under ultrasound guidance, the carcinomas were punctured with a 14-G guideneedle through which a microwave electrode(25.0 cm in length, 1.6mm in diameter) was inserted. To coagulate the HCCs and surrounding hepatic parenchyma, microwave irradiation at 60W for 45 -60 seconds was then applied. One to three sessions of PMCT were performed at intervals of 2-6 days, and one week to 29 months later, the therapeutic effect was evaluated by spiral CT, angiography, and serum alpha-fetoprotein. RESULTS: Eighteen of 25 HCCs (72.0%) were necrotized completely, but seven (28.0%) recurred. Ninety percent of HCCs smaller than 4 cm in long diameter showed complete remission, but all those larger than 4 cm recurred. Alpha-fetoprotein levels decreased markedly in five patients (26.3%), while in 12 (63.2%), asparate aminotransferase (AST)/alanine aminotransferase (ALT) showed transient elevation. Minor complications occurred after PMCT (mild abdominal pain in 8 patients, fever in 7, pleural effusion in 3, portal vein thrombosis in 1, and hepatic abscess in 1), but in no case was this serious. CONCLUSION: PMCT combined with TACE provides effective and safe treatement for nodular HCCs with a long diameter of less than 4 cm.

4.Assessment of Temperature and Humidity in Operating Room, Recovery Room and Intensive Care Unit.

It is recognized that there may be numerous medical hazards associated with the maintenance of relative humidity below 50 percent. These include the danger of producing hypothermia in patients, especially during long operative procedures; the fact that floating particulate matter increased in conditions of low relative humidity, and the fact that the incidence of wound infections is minimized following procedures performed in those operating rooms in which the relative humidity is maintained at the level of 50 to 55 percent. So, we have checked room temperature and relative humidity of operating theater in Hanyang University Hospital during 1 year from January, 1991. The results were as following; 1) Room temperature wae maintained at the level of 23.6+/-0.8 to 27.7+/-1.3 degrees C, and it was somewhat lower in shadowy rooms than in sunny ones. 2) Relative humidity was highest level in July, and it was maintaiaed at the level of 41.7+/-4.9 to 47.0+/-7.7%. Generally, it was decreased unsignificantly in order of ICU, recovery room, newbuilt shadowy rooms, old shadowy rooms, and sunny rooms. 3) Relative humidity was significantly decreased to 2.4+/-3.8-28.9+/-5.0% in spring, autumn, and winter. 4) Absolute humidity was maintained at the level of 11.6+/-1.7 mgH2O/L in summer, and l.l+/-1.4 mgH2O/L in winter. With above results, we recognized that room temperature of operating theater was maintained at the level of recommending degree, but relative humidity(percent) was too low to be suitable for operation and/or special patient care. This means that ventilation of operating theatre should be installed with proper temperature and relative humidity controlled units.

5.A Case of Fibrolipoma on the Palm.

Fibrolipoma is a rare histological variant of lipoma characterized by proliferation of mature fat and fibrous tissue. Most of the benign lipomatous tumors in the palm reported in the literature developed in the nervous tissue, especially the median nerve sheath. We report an unusual form of fibrolipoma on the palm which arose from the subcutaneous tissue or superficial palmar fascia in the palm, not the nerve or tendon sheath. There was no recurrence 3 years after surgical removal was performed.

6.Interaction of Magnesium Sulfate with Mivacurium-induced Neuromuscular Block in Rabbits.

BACKGROUND: Magnesium sulfate(MgSO4) has long been used in the treatment of pre-eclampsia and hypertension. Mivacurium was found to undergo hydrolysis by plasma cholinesterase at 70-88% of the rate of succinylcholine. This study has been conducted to evaluate the time course of mivacurium-induced neuromuscular block according to the pretreatment doses of MgSO4 in rabbits. METHODS: This study was done using a common peroneal nerve-anterior tibialis muscle preparation in sixty adult rabbits. The onset and recovery times of mivacurium(0.16 mg/kg) were evaluated after the infusion of a placebo in group 1, MgSO4 30 mg/kg in group 2, MgSO4 50 mg/kg in group 3 and MgSO4 70 mg/kg in group 4, respectively(n=15 each). RESULTS: The mean serum concentration of magnesium was 2.4 mEq/L in group 1, 3.3 mEq/L in group 2, 4.3 mEq/L in group 3 and 5.5 mEq/L in group 4, respectively. The mean time from injection of mivacurium to 95% recovery of twitch amplitude was 32.7 min. in group 1, 32.9 min. in group 2, 30.4 min. in group 3 and 33.0 min. in group 4, respectively. CONCLUSIONS: The results of the present study suggested that the neuromuscular blocking actions of mivacurium are not affected according to the pretreatment doses of MgSO4.

7.A Trial of Hyperfractionated Radiotherapy in Supratentorial Gliomas.

Fractionation dose and number have been known as radiation factor affecting the radiation complication and the effectiveness in radiotherapy for brain tumors. In this study hyperfractionation technique with 115cGy/fractioin 2 fractions daily 5days/wk, upto 5750-6900cGy to partial brain volume was compared with conventional fractionation technique with daily 200cGy/fraction 5 fraction/wk, upto 5400-6000cGy, in regarding to the effectiveness of hyperfractionated radiotherapy and eraly and later radiation reavtion. The survival period was longer in hyperfractionated irradiated group particularly if the tumors were located in the posterior portion of brain, however there was no singificant statistics due to small number of patients. Mean survival period for glioblastoma multiforme was 11.8 months in hyperfractionated group vs 8.7 months in conventional fractionated group and for high grade astrocytoma 36month in hyperfractionated group, but in conventional fractionated group all was died in 18 months. Acute radiation reaction occurred less frequently in hyperfractionated group, 15.8% vs 47.8% in conventional fractionated group(p<0.024). Alopeci was developed in 31.6% of the hyperfractionated group vs 82.6% of the conventional fractionated group(p<0.0031). One case of later radiation necrosis in cancer region was suspected in the hyperfractionated group but we has been in a dilemma for confirmatory diagnosis in present available diagnostic technique. The hyperfractionated irradiation technique was proven to be superior to conventional fractionated technique regarding the radiation reaction and the effectiveness of the treatment.

9.Cytoprotective effects of Morinda officinalis against hydrogen peroxide-induced oxidative stress in Leydig TM3 cells.

AIMTo investigate the antioxidant effects of Morinda officinalis (Morindae radix, MR) on H(2)O(2)-induced oxidative stress in cultured mouse TM3 Leydig cells.
METHODSWe carried out 2,2-diphenyl-1-picrylhydrazyl free radical scavenging, 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide, lipid peroxidation, testosterone enzyme immunoassay, superoxide dismutase (SOD), and catalase (CAT) assays in Leydig TM3 cells.
RESULTSMR showed a 47.8% 2,2-diphenyl-1-picrylhydrazyl radical scavenging effect in TM3 cells with no significant cytotoxicity. Oxidative stress was induced in TM3 cells with 100 micromol H(2)O(2), and treatment of the cells with 250 microg/mL MR showed the most significant protective effect (64%, P < 0.001) in the cell viability assay with a decreased lipid peroxidation level (1.75 nmol/mg protein, P < 0.05), increased testosterone production (43.5 pg/mL), and improvements in SOD activity (7.49 units of SOD/mg protein, P < 0.001) and CAT activity (74.6 units of CAT/mg protein, P < 0.001).
CONCLUSIONThese findings indicate that MR, as an antioxidant, protects functions of cultured mouse TM3 Leydig cells from H(2)O(2)-induced oxidative stress.

10.Long-term Survivorship and Non-cancer Competing Mortality in Head and Neck Cancer: A Nationwide Population-Based Study in South Korea

Purpose:
As the survival of head and neck cancer (HNC) improves, survivors increasingly confront non-cancer–related deaths. This nationwide population-based study aimed to investigate non-cancer–related deaths in HNC survivors.
Materials and Methods:
Data from the Korean Central Cancer Registry were obtained to characterize causes of death, mortality patterns, and survival in patients with HNC between 2006 and 2016 (n=40,890). Non-cancer-related mortality relative to the general population was evaluated using standardized mortality ratios (SMRs). The 5- and 10-year cause-specific competing risks probabilities of death (cumulative incidence function, CIF) and subdistribution hazards ratios (sHR) from the Fine-Gray models were estimated.
Results:
Comorbidity-related mortality was frequent in older patients, whereas suicide was predominant in younger patients. The risk of suicide was greater in patients with HNC than in the general population (SMR, 3.1; 95% confidence interval [CI], 2.7 to 3.5). The probability of HNC deaths reached a plateau at 5 years (5-year CIF, 33.9%; 10-year CIF, 39.5%), whereas the probability of non-HNC deaths showed a long-term linear increase (5-year, CIF 5.6%; 10-year CIF, 11.9%). Patients who were male (sHR, 1.56; 95% CI, 1.41 to 1.72), diagnosed with early-stage HNC (localized vs. distant: sHR, 1.86; 95% CI, 1.58 to 2.21) and older age (65-74 vs. 0-44: sHR, 6.20; 95% CI, 4.92 to 7.82; ≥ 75 vs. 0-44: sHR, 9.81; 95% CI, 7.76 to 12.39) had an increased risk of non-cancer mortality.
Conclusion
Non-HNC–related deaths continue increasing. HNC survivors are at increased risk of suicide in the younger and comorbidity-related death in the older. Better population-specific surveillance awareness and survivorship plans for HNC survivors are warranted.
