3.Effects of Job Stress, Physical Activity, Subjective Happiness on Wellness among Bus Drivers

Purpose:
This study aimed to identify the factors that influence the wellness of bus drivers by examining the relationships between job stress, physical activity, subjective happiness, and wellness.
Methods:
Self-report questionnaires were distributed during October 2017.
Results:
Based on a multiple regression analysis, the significant factors found to affect the wellness of bus drivers were physical activity (β=.48,p<.001), subjective happiness (β=.25, p=.001), and job stress (β=-.17, p=.025). The total explanatory power of the study variables for wellness was 45.7% (F=33.04, p<.001).
Conclusion
These findings highlight a need to develop workplace wellness programs to reduce job stress and improve physical activity and subjective happiness among bus drivers.
4.Influence of the Awareness of Healthcare Accreditation on Job Stress and Turnover Intention in Tertiary Hospital Nurses

PURPOSE: The aim of this study was to investigate the influence of nurses' awareness of healthcare accreditation on their job stress and turnover intention. METHODS: Across-sectional correlation study design was used. Participants consisted of 143 nurses from two tertiary hospitals in Seoul and Gyunggido, South Korea. Data were collected in October 2016 using self-report questionnaires and analyzed using descriptive statistics, t-test, an ANOVA, Pearson's correlation coefficient, and multiple regression in SPSS/WIN 21.0. RESULTS: Awareness of health care accreditation correlated negatively with job stress (r=−63, p < .001) and turn over intention (r=−.50, p < .001). Awareness of health care accreditation was the factor that most influenced job stress, explaining 38% of the variance (F=23.10, p < .001). Awareness of health care accreditation and duty pattern were the factors that most influenced intention to leave, explaining 32% of the variance (F=10.35, p < .001). CONCLUSION: These findings suggest raising nurses' awareness of certification systems to reduce their job stress and turnover intentions. It will be necessary to provide support for the aggressive work nurses do and improve their work structure, highlighting the need for both manpower and institutional support. Accordingly, providing regular education programs and appropriate compensation schemes, by raising nurses' awareness of medical institutions' certification systems, is necessary.

6.The Effects of Repeated Information using Visual and Video-Audio Media on Uncertainty and Anxiety in Patients undergoing Thyroidectomy

Purpose:
The purpose of this study was to identify the effects of repeated audiovisual information on the reduction of uncertainty and anxiety in patients undergoing thyroidectomy.
Methods:
A non-equivalent control group pretest-posttest design was used. The study included 32 patients in the experimental group and 32 in the control group. Preoperative information using video-audio media was provided two times for 5 minutes and 45 seconds each in the experimental group, while the control group only received routine care. The data collection period was from November 1, 2017 to February 23, 2018. The data were analyzed using χ² test, t test, and independent t-test with SPSS/WIN 24.0.
Results:
Significant differences were observed between the two groups in pre- and post-uncertainty scores (t = -2.06, p= .044) and anxiety scores (t = -2.06, p= .044).
Conclusion
Repeated information provided by using videoaudio media is effective as a nursing intervention to reduce uncertainty and anxiety in patients undergoing thyroidectomy.
7.Prediction of Clinical Outcomes in Patients with Acute Ischemic Stroke by Measurements of Motor Evoked Potentials.

OBJECTIVE: The integrity of the motor pathways can be assessed objectively and quantitatively by measuring the motor evoked potentials (MEPs). However, the early prognostic application of MEPs for assessing the motor and functional recovery of patients with acute ischemic stoke has yielded contradictory results. Therefore, we assessed the value of MEPs for predicting the clinical outcomes of acute ischemic stroke patients. METHODS: Thirty three stroke patients with different degrees of hemiparesis were enrolled in this study. The stroke severity and outcome were assessed using the National Institute of Health Stroke Scale (NIHSS) and the modified Rankin Scale (mRS) at admission and at 3 months. The MEPs were measured at the adductor pollicis brevis and adductor hallucis muscles and the relationships between the NIHSS, the mRS and the MEP findings were analysed. RESULTS: In the acute phase of stroke, the presence of MEPs in the upper or lower extremities was correlated with a better clinical outcome (NIHSS: 9.12+/-5.86->2.81+/-1.47, mRS: 2.81+/-1.47->1.62+/-1.31) than the absence of an MEP in at least one extremity (NIHSS: 11.47+/-5.53->8.88+/-6.02, mRS: 3.70+/-1.31->2.94+/-1.67). Especially, there was significant clinical improvement after 3 months in the large territorial infarction patients according to the presence of MEPs. 8.Resilience and Characteristics of Sleep and Defense among Shift Work Nurses.

OBJECTIVES: Shift work is a stressful situation. It is important to know the factors associated with the ability to adapt to a shift work schedule. The aim of the present study was to investigate the association between sleep, as well as personality variables, and the resilience of shift work nurses. METHOD: Self-report questionnaires were administered to 95 nurses who worked in one national university hospital. Connor-Davidson resilience scale, hospital anxiety and depression scale, morningness-eveningness scale, Pittsburgh sleep quality index, other sleep-related questionnaires, and Korean defense style questionnaires were used. RESULTS: Age, shift work duration, off-day oversleep, depression, anxiety, adaptive defense style, and self-suppressive defense style were significantly associated with resilience (p < 0.05). Multiple regression analysis showed that age (beta = 0.34, p < 0.05), depression (beta = -0.25, p < 0.05), adaptive defense style (beta = 0.45, p < 0.001), and self-suppressive defense style (beta = -0.19, p < 0.05) significantly predicted the resilience of shift work nurses. Concerning individual defense mechanisms, resignation (beta = -0.20, p < 0.05), sublimation (beta = 0.19, p < 0.05), omnipotence (beta = 0.19, p < 0.05), and humor (beta = 0.20, p < 0.05) significantly predicted the resiliency. CONCLUSION: The findings indicate that a specific defense style and other mechanisms were associated with the resilience of shift work nurses. A future prospective study with more participants could further clarify the relationship between sleep-related variables, as well as personality factors, and resilience of shift work nurses.

9.Risk factors for severe postpartum hemorrhage requiring blood transfusion after cesarean delivery for twin pregnancy: a nationwide cohort study

Postpartum hemorrhage (PPH) is a leading cause of maternal morbidity and mortality. Twin pregnancy and cesarean delivery are well-known risk factors for PPH. However, few studies have investigated PPH risk factors in mothers who have undergone cesarean delivery for twin pregnancies. Therefore, this study investigated the risk factors associated with severe PPH after cesarean delivery for twin pregnancies. Methods: We searched and reviewed the Korean Health Insurance Review and Assessment Service’s claims data from July 2008 to June 2021 using the code corresponding to cesarean delivery for twin pregnancy. Severe PPH was defined as hemorrhage requiring red blood cell (RBC) transfusion during the peripartum period. The risk factors associated with severe PPH were identified among the procedure and diagnosis code variables and analyzed using univariate and multivariate logistic regressions. Results: We analyzed 31,074 cesarean deliveries for twin pregnancies, and 4,892 patients who underwent cesarean deliveries for twin pregnancies and received RBC transfusions for severe PPH were included. According to the multivariate analysis, placental disorders (odds ratio, 4.50; 95% confidence interval, 4.09– 4.95; P < 0.001), general anesthesia (2.33, 2.18–2.49; P < 0.001), preeclampsia (2.20, 1.99–2.43; P < 0.001), hemolysis, elevated liver enzymes, low platelets (HELLP) syndrome (2.12, 1.22–3.68; P = 0.008), induction failure (1.37, 1.07–1.76; P = 0.014), and hypertension (1.31, 1.18–1.44; P < 0.001) predicted severe PPH. Conclusions: Placental disorders, hypertensive disorders such as preeclampsia and HELLP syndrome, and induction failure increased the risk of severe PPH after cesarean delivery for twin pregnancy
10.Characteristics of Dizziness in Supratentorial Infarctions.

BACKGROUND: Dizziness due to brain lesions manifests mainly in infratentorial lesions, with few cases related to supratentorial lesions having been reported. This study aimed to elucidate the clinical characteristics and demographic factors of patients with dizziness caused by cerebral infarction and to determine the site of the brain where supratentorial lesions are most prevalent. METHODS: Patients with prominent dizziness who visited the emergency room of Gachon University Gil Hospital between July 2006 and July 2007 were included. Among them, 101 patients with acute cerebral infarction were categorized into supratentorial (n=51) and infratentorial (n=50) groups based on brain MRI. Demographics and clinical characteristics of dizziness in each group were compared, and common brain sites of the supratentorial group were assessed. RESULTS: The nature of the dizziness differed between the supratentorial group (vertigo, 27.4%; presyncope, 5.9%; disequilibrium, 29.4%; ocular, 11.8%; and nonspecific, 25.5%) and the infratentorial group (vertigo, 50.0%; presyncope, 6.0%; disequilibrium, 32.0%; ocular, 0%; and nonspecific, 12.0%; p=0.02). The duration of dizziness was shorter in the supratentorial than the infratentorial group (p<0.01). In the supratentorial group, common sites of the lesion were the thalamus (19.6%) and frontoparietal lobe (15.7%). The lesion usually appeared in the left hemisphere (60.8%). CONCLUSIONS: Dizziness from supratentorial lesions manifests in different ways, and its duration is shorter than that from infratentorial lesions. The central vestibular pathway may be located in the thalamus and frontoparietal lobe.
