2.The Protective Effects of Various Stress Modalities on Ischemic / Reperfused Hearts of Rats.

BACKGROUND: It has been found that sterss challenge with heat shock produces the acquisition of cellular resistance to ischemin injury in the hearts, which is associated with stress protein induction. The conventional heat shock(42degrees C of rectal temperature for 15min, anesthetized animal), however, is strong enough to endanger the animal life and then not suitable for practiocal application in human. The present study was performedd in an attempt to search the safely applicabel stress modalities to acquire the myocardial tolerance to ischemia-reperfusion in jury. METHODS: Male, Sprague-Ddawley rats(200-250g) were exposed to various stressful conditions, such as heat stimulation(environmental temperature of 42degrees C for 30min, live animal), swimming(20min), immobilization(60min), treadmill exercise(20M/min, 30min) and hyperbaric oxygenation(3atm, 60min) given once a day for 5 days. Twenty-four hours after the last application the hearts were isolated and perfused with oxygenated Krebs-Henseleit buffer solution by Langendorff method. Ischemia-reperfusion injury was produced by 20 min-global ischemia followed by 30 min-reperfusion. Cardiac mechanical function, lactate dehydrogenase release, the induction of stree proteins were assayed and compared dbetween the stressed dand the control animals. RESULTS: Upon reperfusion after ischemia the recovery of cardiac function was significantly improved in the stressed animals. The percentile recovery at 30min of reperfusion was in a range from 55.3%(swimming) to 89.3%(treadmill exercise), which was significantly higher than that of the control hearts(38%). The functional recovery of the conventional heat shocked heart was 57.7%. In stressed animals, lactate dehydrogenase release, which indicates myocardial cell injury, was significantly reduced by 20 to 30% compared to that for the control. The expression of an inducible form of 70 series stress protein, SP72, which was assayed by immunoblotting method, was markedly increased by heat stimulation while the other stress modalities failed to increase, it. There were no appreciable inductions of SP73(constitutive form) and GRR78 in the stressed animals. CONCLUSION: These results suggest that the cardiac protection from the ischemia-reperfusion injury could be induced by the repetitive non-fatal stress stimulations and that SP70 family proteins may be partly involved in the cardioprotective effect produced by heat stimulation, but not play the essential roles in anti-ischemic effects produced by other stress modalities.

3.Metabolic Activities of Benign Musculoskeletal Tumors Using 2- F - 18 - Fluoro -2 - deoxy - D - glucose ( FDG ) Positron Emission Tomogrphy ( PET ) ( preliminary report ).

Positron emission tomography (PET) with 2-[F-18]-Fluoro-2-deoxy-D-glucose (FDG) was performed in ninteen patients who had benign musculoskeletal tumors in order to determine if there was a relationship between histologic grade of tumor and FDG uptake of tumor. These patients had been evaluated previously with computed tomography (CT) or magnetic resonance (MR) imaging or both. The diagnoses were confirmed with incisional or excisional biopsy or by radiographic follow-up. Generally high-grade tumors had significantly greater uptake of FDG than low-grade lesions . Benign lesions such as giant cell tumor, fibrous dysplasia, and osteofibrous dysplasia showed significant elevation of SUV (Standardized Uptake Value) above 4.0. On the contrary. The current studies suggest the utility of FDG-PET imaging as an adjunct to CT or MR imaging in the evaluation of benign tumors. And results of PET suggest benign tumors with high SUV are histologically active lesion and tend to be locally aggressive.

6.Navigation-Assisted Knee Arthroplasty in Case of Extra-Articular Deformity or Retained Hardware

Total knee arthroplasty is a surgical treatment for end-stage osteoarthritis of the knee. Postoperative lower limb alignment is one of the factors determining the long-term prognosis after total knee arthroplasty. Navigation-assisted surgery can be used to achieve an accurate lower limb alignment. Particular situations, such as severe extra-articular deformity in the femur or tibia and retained hardware, are a good indication for navigation-assisted surgery. On the other hand, thorough preoperative planning and an understanding of each steps of surgery are necessary to perform total knee arthroplasty successfully in these special cases. In this review article, preoperative radiological measurements and surgical planning for extra-articular deformity correction will be elucidated. The surgical steps and necessary instruments for navigation-assisted total knee arthroplasty will be described in cases of extra-articular deformity or retained hardware. A literature review showed that the radiological and clinical results after total knee arthroplasty using navigation in osteoarthritis with extraarticular deformity were good. Therefore, it is essential to use navigation when performing total knee arthroplasty in patients with extraarticular deformity or retained hardware.

9.Relationship of Mothers' Recognition of Attention Deficit Hyperactivity Disorder (ADHD), Parenting Stress and Family Support in Children Diagnosed with ADHD.

PURPOSE: The purpose of this study was to examine the relationship of mothers' recognition of Attention deficit hyperactivity disorder (ADHD), parenting stress and family support in mothers of children diagnosed with ADHD. METHODS: The data were collected from December 24, 2009 to July 23, 2010. The participants were 141 mothers of children diagnosed with ADHD and who lived in D city. Descriptive statistics, t-test, ANOVA, Pearson correlation coefficients with SPSS 17.0 were used to analyze the data. To determine the most effective variable (mothers' recognition, parenting stress and family support) to predict parental stress, data were analyzed using canonical correlation with SAS 9.1 TS. RESULTS: Canonical correlation analysis revealed that professional and teachers' roles of the 3 sub-domains of mothers' recognition were the most outstanding variables in predicting parenting stress. Parental distress/parent-child dysfunctional interaction of parenting stress was the most outstanding variable in predicting both mothers' recognition and family support. Spouse cooperation in family support was the most outstanding variable in predicting parenting stress. CONCLUSION: The results suggest that strengthening parents' recognition of ADHD and supporting interventions are important for reducing parenting stress. Professional and teachers' roles, parental distress/parent-child dysfunctional interaction and spouse cooperation should be included in the contents of structured programs.

10.Meta Analysis of Variables Related to Attention Deficit Hyperactivity Disorder in School-Age Children.

PURPOSE: The purpose of this study was to use meta-analysis to examine recent domestic articles related to attention deficit hyperactivity disorder (ADHD) in school-age children. METHODS: After reviewing 213 articles published between 1990 and 2009 from and cited in RISS, KISS, and DBpia, the researchers identified 24 studies with 440 research variables that had appropriate data for methodological study. SPSS 17.0 program was used. The outcome variables were divided into five types: Inattention, hyperactive impulsive, intrinsic, extrinsic, and academic ability variables. RESULTS: Effects size of overall core symptoms was 0.47 which is moderate level in terms of Cohen criteria and effects size of overall negative variables related ADHD was 0.27 which is small level. The most dominant variable related to ADHD was obtained from hyperactive-impulsive (0.70). Also academic ability (0.45), inattention (0.37), and intrinsic variables (0.29) had a small effect whereas extrinsic variables (0.13) had little effect on descriptive ADHD study. CONCLUSION: The results reveal that ADHD core symptoms have moderate effect size and peripheral negative variables related ADHD have small effect size. To improve the reliability of the meta-analysis results by minimizing publication bias, more intervention studies using appropriate study designs should be done.
