1.DNA analysis of squamous cell carcinoma and basal cell carcinoma of the skin using flow cytometry.

DNA flow cytometric analysis was performed on paraffin-embedded tissue from 10 cases of squamous cell eareinoma (SCC) and 10 cases of basal cell carcinoma(BCC). These results were applicable to do a better prognosis in BCC than SCC. In 10 cases of SCC, the DNA index was 1.34 and aneuploidy was identified in 9. In 10 cases of BCC, the DNA index was 1.30 and aneuploidy was identified in 6.

2.A Case of Generalized Normolipemic Plane Xanthoma.

We report a case of generalized plane xanthoma which showed no-rmolipoproteinemia and was not associated with unerlying disorders or other type of xanthomas. A 48-year-old female has noticed well defined, slightly elevated, yellowish plaques which occurred on the periorbital areas, sides of the neck, scapular areas, anterior chest, buttocks and lower extremities for 3 years. Biopsy specimen taken from the neck showed perivascular accumulations of foam cells in the upper and mid dermis. Paper electrophoresis didn't, show elevation of any lipoprotein.

3.The Risk Factors of Carditis in Children with Kawasaki Disease.

PURPOSE: The most common complication of Kawasaki disease(KD) is the development of coronary artery aneurysm. Other cardiac complication include myocarditis, pericarditis, mitral or aortic insufficiency, congestive failure, and arrhythmia. These non- coronary cardiac problems have important role on clinical course in acute phase of KD. Extensive researches have been performed about the pathogenesis, natural course and treatment of coronary artery lesion. But the studies about non-coronary cardiac complication have been limited, so we aimed to study the manifestation, incidence, and clinical predictor of carditis. METHODS: The retrospective studies was performed to the 48 KD patients diagnosed in Chungnam National University Hospital from May 1998 to September 2004. The patients were divided into two groups. Patients with carditis(11 patients) and without carditis(37 patients) before receiving the IVGG infusion were analyzed. RESULTS: There were no differences in age and sex between two groups. Patients with long duration of fever and high Harada score had the significant increase in the incidence of carditis before the initial IVGG infusion. No significant differences were not shown between the two groups in terms of WBC count, hemoglobin, hematocrit, platelet count, total protein, AST/ALT, LDH, CPK, troponin I, total cholesterol, and triglyceride before the initiL IVGG infusion. The incidence of carditis was high in patient with high CRP, low albumin, and high cholesterol and triglyceride. CONCLUSION: The possibility of significant carditis should be considered before the infusion of high dose IVIG, which may worse the caridac dysfunction. Some predictors of significant carditis such as long duration of fever, high Harada score, high levels of CRP, and low levels of albumin can be suggested.

4.The relationship of common reasons for encounter and major diagnoses of patients admitted to a hospital.

BACKGROUNDS: It is important to know the contents of health problems in patients in a primary care setting. The aim of this study was to explore the main chief complaints and major diagnoses of patients, who were admitted to a hospital using the ICD-10 and to observe difference according to each department and admission route and how diagnoses were made. METHODS: A total of 18,560 patients who were admitted to a hospital located in Chungnam Province from 1 Jan 1998 to 31 Dec 1998. Main chief complaints and major diagnoses made through the admissions departments were chosen as subjects (medical vs. surgical) and by admission route (emergency vs. OPD)were analyzed. How the diagnoses were derived from the most common chief complaints among medical and surgical departments were analyzed by admissions departments and by admission route. RESULTS: The most common 10 chief complaints revealed no significant difference by admission route in medical departments, but there was some difference in surgical departments. There was some difference in the most common 10 major diagnoses by admission routes in both medical and surgical departments. Abdominal and pelvic pain, which was the most common chief complaints, became a significantly different diagnosis by admission route in both departments. CONCLUSION: Main chief complaints were similar regardless of admission routes, but the diagnoses were different. ICD-10 classification may be useful to classify the diagnoses, but have limitations to classify chief complaints or reasons for encounter. It is necessary to introduce a new classification such as ICPC-2 for reasons for encounter in order to explore the dimension of health problems.

5.A Case of Turner Syndrome with Coarctation and Subclavian Steal Syndrome by the Left Subclavian Artery from the Descending Aorta Below the Coarcted Segment.

Juxtaductal coarctation is usually distal to the origin of the left subclavian artery, occasionally the orifice of the subclavian artery is involved in the coarctation and partially obstructed. An anomalous origin of the right subclavian artery from the descending aorta below the coarcted segment is also occasionally encountered. Reversed vertebral artery flow to a subclavian artery arising at or below a coarctation may produce the subclavian steal syndrome. Rarely both subclavian arteries arise beyond the coarctation. These abnormal subclavian arteries are important in clinical diagnosis and treatment. We report a case of Turner syndrome with coarctation, which present as juxtaductal type and the left subclavian artery from the descending aorta below the coarcted segment with reversed vertebral artery flow to a subclavian artery producing the subclavian steal syndrome. Resecton of coarctation segment and end-to-end anastomosis was successfully performed after transfer of left subclavian artery to distal segment of descending aorta.

6.Trapped Stent in the Left Coronary Sinus in a Myocardial Infarction Patient.

7.Risk Factors of Primary Nocturnal Enuresis.

Urinary cytology has become an essential element in the diagnosis and management of transitional cell carcinoma(TCC) of the urinary tract. It has the advantage of being noninvasive, inexpensive, and easily accessible. Besides that it can even detect malignancy when unsuspected at cystoscopy. We report a retrospective review of urine cytology in the diagnosis of 83 TCC cases that underwent 295 cytologic evaluation. All patients had biopsy-proven TCC of the bladder, ureter and renal pelvis. The overall inci- dence of the positive cytology cases was 66.2%. To define the cytologic features of tumor cells, we tried to use three cytologic gradings such as "grade 1", "grade 2", and "grade 3" according to the cytologic degree of anaplastic neoplastic cells. These cytologic gades of TCC were relatively well correlated with the histologic grade and tumor invasiveness. This result suggests that the recognition of characteristic cellular features of TCC can suspect the histologic grade and tumor stage. The false negative TCC cases were 78.9%. They showed severe inflammatory or bloody background and a few neoplastic cells. Therefore, a cautious approach for accurate interpretation, personal experience, and proper fixation and processing could expand the role of urinary cytology.

Pleural effusion due to hepatic cirrhosis with ascites is well known, although hepatic hydrothorax in the absence of ascites is a rare condition, the pathogenesis of which is still unknown. We report a case of hepatic hydrothorax without ascites confirmed by the intraperitoneal injection of Tc-99m macroaggregated serum albumin (Tc-99m MAA) that demonstrated the passage of Tc-99m MAA into the right pleural cavity.

10.Statistical Analysis of Diseases with Discharge Data Set.

This study was performed to investigate the factors concerning the structures of diseases in Korea. The data were collected from the 11 tertiary referral hospitals and the number of investigated patients were 242.038 and the diagnoses were based on 21 chapters of Korean Standard Classification of Diseases(KCD-3). The results were as fellows: 1. The number of investigated patients were 242,038 Among them male(51.1%) was a little more than female 48.9%). In age distribution, the group of 45-64 was the highest(25.5%) and 30-49year age group was 20.9%. under l4year age group was 20.3%. and above 65years age group was the lowest( 15.3%). 2. In single diagnosis group division by 21 chapters of KCD-3. diseases of the respiratory system were the highest(11.3%) , injury. poisoning and the next were certain other consequences of external causes( l0.39% ) and neoplasms(10.0%) . The low est were diseases of the blood and blood-forming organs and certain disorders involving the immune mechanism(0.7%). in single diagnosis group, sex, age. season, region and operation and procedures were statistically significant in the x2-test analysis. 3. In multiple diagnoses group. neoplasms were the highest (17.0%). and the next were pregnancy, chirdbirth and the puerperium(l0.6%) and the diseases of the circulatory systems(9.2%). The lowest was diseases of the blood and blood forming organs and certain disorders involving the immune mechanism(0.6%). In multiple diagnoses group, sex. ace. season, region and operation and procedures were statistically significant iii the x2-test analysis.
