1.Dysphagia after Cervical Intradiscal Electrothermal Therapy: A Case Report

ntradiscal electrothermal therapy (IDET), which is a minimally invasive treatment, was performed to treat cervical HNP. Although IDET has a relatively short recovery time and lower cost compared to surgical treatment, it can still cause complications, such as nerve injury due to improper insertion of the catheter and temperature of the heating coil. This paper reports a rare case of dysphagia following IDET in Korea. A 71-year-old male complained of hoarseness and dysphagia after IDET on the C6-7 level. A Video Fluoroscopic Swallowing Study revealed vallecula and a pyriform sinus remnant, as well as incomplete vocal cord adduction. Subglottic aspiration was observed in the entire examination. The main cause of the dysphagia was attributed to recurrent laryngeal nerve injury during IDET through the right anterior approach at the C6-7 level. In conclusion, even in the case of minimally invasive procedures, the anterior approach may cause neurological damage, such as the recurrent laryngeal nerve. A fter the procedure, it is necessary to observe the patient’s symptoms, consider further evaluations, and perform rehabilitation treatment.
2.Insufficiency Fracture of Proximal Tibia in a Young Male Patient with Osteoporosis

We report a case of a healthy 38-year-old man presenting with insufficiency fracture of proximal tibia associated with unrecognized osteoporosis. Radiologic bone DEXA (Dual Energy X-ray Absorptiometry) assessment revealed osteoporosis, but the patient displayed no apparent risk factors for insufficiency fractures and osteoporosis except for a history of smoking. Following right proximal tibial open reduction and internal fixation, the patient commenced smoking cessation and began taking alendronate.The patient was treated for gait rehabilitation, which included quadriceps strengthening exercises and his condition improved with independent walking. This report highlights an unusual site of proximal tibial insufficiency fractures and the potential effect of smoking on low bone mineral density in a young male patient.

4.A Study on the Type of Role Awareness for Medical Institutions Home Health Care Specialists: A Q-Methodological Approach.

PURPOSE: This study is to identify the types of role awareness of home health care specialists working at medical institutions, to understand their role awareness correctly by grasping the quality of each type, and to provide useful help in the education of home health care specialists. METHOD: Q-methodology is used to objectify role awareness of medical institution specialists who may recognize situations differently according to their individual experience and comprehension based on the view of behavior. Q-classification was carried out on 30 home health care specialists working at medical institutions using 30 selected questions. Collected data were examined through factor analysis using QUANL PC program. RESULTS: Three different types of role awareness of home health care specialists working at medical institutions were identified. Type I is "educational-function-centered", Type II "patient-centered" and Type III "professional-service-centered". CONCLUSION: Regardless of these types, home health care specialists commonly had a high pride as a specialist and a sense of mission regarding themselves as important persons responsible for patients' health.

5.Changes in Blood Glucose Level After Steroid Injection for Musculoskeletal Pain in Patients With Diabetes

Objective:
To investigate changes in blood glucose level after steroid injection in patients with type 2 diabetes mellitus (DM) and factors affecting those changes.
Methods:
We retrospectively studied 51 patients with type 2 DM who underwent steroid injection for shoulder and back pain. Mean fasting blood sugar (FBS) levels for 7 days before steroid injection was used as the baseline blood glucose level, which was compared with FBS levels for 14 days after steroid injection. We compared the differences in blood glucose changes between HbA1c >7% and HbA1c ≤7% groups and those between insulin and non-insulin treated groups. Demographic data, injection site, and steroid dose were analyzed.
Results:
Compared to baseline, blood glucose significantly (p=0.012) elevated 1 day after steroid injection but not 2 days after injection. In the HbA1c >7% and insulin groups, blood glucose was significantly increased 1 day after injection compared to that in the HbA1c ≤7% (p=0.011) and non-insulin (p=0.024) groups, respectively. Higher HbA1c level before injection was significantly (p=0.003) associated with the degree of blood glucose increase 1 day after injection. No significant differences were noted in the degree of blood glucose increase according to injection site or steroid dose.
Conclusion
Higher HbA1c level was associated with greater elevation in blood glucose 1 day after steroid injection. Careful monitoring of blood glucose is required on the first day after steroid injection in patients with poorly controlled DM.
6.Diagnostic Value of Tendon Reflex in Diabetic Peripheral Neuropathy.

OBJECTIVE: To investigate the role of tendon reflex test in the diagnosis of diabetic peripheral neuropathy. METHOD: Patellar tendon reflex (PTR) and achilles tendon reflex (ATR) were recorded in forty six diabetic patients and thirty seven normal adults by delivering tendon taps with an electric reflex hammer. Forty six diabetic patients were divided into two groups based on nerve conduction study and diabetic neuropathy score: group 1 consisted of nineteen patients with peripheral neuropathy, group 2 consisted of twenty seven patients without peripheral neuropathy. Multiple regression equations using latency as a variable dependent on age and height were used and upper crossing of the 3 standard deviation level with regression on height and age was considered abnormal. RESULTS: Mean latencies of PTR and ATR were prolonged in the diabetic patients in comparison with the controls (p<0.01) and were prolonged in group 1 compared to group 2. In group 1, PTR was abnormal in 14 cases (sensitivity: 73.6%, specifity: 88%) and ATR was abnormal in 13 cases (sensitivity: 68.4%, specifity: 85.1%). In group 2, PTR was abnormal in 3 cases and ATR was abnormal in 4 cases. CONCLUSION: Tendon reflex test would be a valuable supplement to conventional nerve conduction studies for detection of diabetic peripheral neuropathy, especially in the proximal segment.

7.Evaluation of titanium surface properties by Nd: YVO4 laser irradiation: pilot study.

PURPOSE: This study was conducted to evaluate the roughness and surface alternations of three differently blasted titanium discs treated by Nd: YVO4 Laser irradiation in different conditions. MATERIALS AND METHODS: Thirty commercially pure titanium discs were prepared and divided into three groups. Each group was consisted of 10 samples and blasted by ZrO2 (zirconium dioxide), Al2O3 (aluminum oxide), and RBM (resorbable blasted media). All the samples were degreased by ultrasonic cleaner afterward. Nine different conditions were established by changing scanning speed (100, 300, 500 mm/s) and repetition rate (5, 15, 35 kHz) of Nd: YVO4 Laser (Laser Pro D-20, Laserval Korea(R) Seoul, South Korea). After laser irradiation, a scanning electron microscope, X-ray diffraction analysis, energy dispersive X-ray spectroscopic analysis, and surface roughness analysis were used to assess the roughness and surface alternations of the samples. RESULTS: According to a scanning electron microscope (SEM), titanium discs treated with laser irradiation showed characteristic patterns in contrast to the control which showed irregular patterns. According to the X-ray diffraction analysis, only Al2O3 group showed its own peak. The oxidation tendency and surface roughness of titanium were similar to the control in the energy dispersive X-ray spectroscopic analysis. The surface roughness was inversely proportional to the scanning speed, whereas proportional to the repetition rate of Nd: YVO4. CONCLUSION: The surface microstructures and roughness of the test discs were modified by the radiation of Nd: YVO4 laser. Therefore, laser irradiation could be considered one of the methods to modify implant surfaces for the enhancement of osseointegration.

8.Spontaneous Thoracolumbar Spinal Cord Infarction: A case report.

Vascular disease of the spinal cord occurs less frequently than of the brain, and its incidence is not known. Case reports of spinal cord infarction are uncommon, especially ones with spontaneous causes. We experienced one case with a spontaneous spinal cord infarction in the territory of the Adamkiewicz artery. In this case, the clinical pictures were characterized by sudden onset of paraplegia, bilateral radicular pain, dissociated sensory loss below the level of infarction and sphincter dysfunction. Neuroradiological investigation and CSF analysis ruled out compressive or infectious lesions. Selective spinal angiography revealed an occlusion of the Adamkiewicz artery. The patient had a substantial recovery over a period of weeks with intensive rehabilitation treatments.

9.The Effect of Warm Scarf on Postoperative Xerostomia and Sore Throat in Colon Cancer Patients

Purpose:
This study aimed to identify the effect of warm scarf on xerostomia and sore throat in postoperative colon cancer patients.
Methods:
A total of 40 participants with colon cancer who underwent colon cancer operation over 2hours were included from C University hospital in Seoul. The number of experimental group and control group is each 20 calculated by G*Power, and they were assigned by using nonequivalent control group no-synchronized design. In the experimental group, a warm scarf was applied to the neck for 120 minutes from entering the recovery room after the colorectal cancer surgery was completed. In the experimental group and the control group, xerostomia and sore throat were measured twice at 60-minute intervals. The degree of xerostomia was measured through the degree of wetness of the absorbent paper in mm, and the degree of sore throat was measured through the NRS (Numeral Rating Scale). Data were collected using self-administered questionnaires from August 2018 to September 2020 and were analyzed using IBM SPSS/WIN 21.0 Descriptive statistics, x2 test, Fisher's exact test, t-test were used to determine the participant's characteristics. The effect of warm scarf on xerostomia and sore throat were separately estimated by Repeated Measures ANOVA.
Results:
The experimental group showed significant decrease of xerostomia and sore throat as time goes (p<.001).
Conclusion
Results indicate that warm scarf on xerostomia and sore throat in postoperative colon cancer patients is helpful method for relieving side effect of tracheal intubation.
10.Effect of Low Frequency Cerebellar Repetitive Transcranial Magnetic Stimulation on Balance Impairment in Patients With Cerebral Infarction

Objective:
To investigate the effect of low frequency cerebellar repetitive transcranial magnetic stimulation (rTMS) on balance impairment in patients with cerebral infarction.
Methods:
Thirty-two patients were randomly divided into two groups: rTMS group (n=16) and control (n=16). In the rTMS group, treatment was performed five times per week for 2 weeks (10 sessions), and in the control group, a sham coil was used with the sound and sensation of scalp similar to the rTMS coil. Patients in both groups underwent a conventional rehabilitation program. Berg Balance Scale (BBS) was used as the primary outcome measurement. Timed Up and Go test (TUG), 10-m walk test (10mWT), and Activity-specific Balance Confidence scale (ABC) were used as the secondary outcome measurement. All scales were measured at baseline (T0), after 10 sessions of rTMS (T1), and at 4 weeks after treatment completion (T2) by therapists with over 5 years of clinical experience.
Results:
There were significant improvements between T0 and T1, and between T0 and T2, for all assessed items in the rTMS group. Whereas there were significant improvements between T0 and T1, and between T0 and T2, for the BBS and 10mWT in the control group. TUG (-4.87±5.05 vs. -0.50±2.97 seconds) and ABC score (8.10±8.33 vs. 0.16±0.97) were observed significant differences in comparison of the changes from T0 to T1 between the two group. BBS score (4.40±3.66 vs. 1.88±3.14), TUG (-4.87±4.56 vs. -0.62±2.96 seconds) and ABC score (8.22±7.70 vs. -0.09±0.86) differed significantly from T0 to T2 between the two groups.
Conclusion
Our findings suggest that low-frequency cerebellar rTMS is helpful for improving balance in patients with cerebral infarction, and maybe a beneficial treatment for these patients.
