3.A Clinical study of the High Tibial Osteotomy in Osteoarthritic Knees with Varus Deformity

During the period of 5 years and 5 months, from January 1979 to May 1984, we treated 27 cases(17 patients) of osteoarthritis with genu varum deformity by high tibial osteotomy. The results were as follows: l. In the 27 cases of the high tibial osteotomy, 24 cases were female and 3 cases were male. 2. Of the etiology of the osteoarthritis with genu varum, 14 female patients had primary one and 3 male patient knees were related with infection(2 patients) and trauma (1 patient). 3. Preoperative varus angles, ranging from 0° to 20°, 6.4° in an average and postoperative valgus angles, ranging from 5° to 15°, 8° in an averge. 4. In a review of the high tibial osteotomy, good and fair results were noted in 26 cases (96.3%) of the knees after average 2 years of follow-up. 5. The high tibial osteotomy is found to be useful method in the treatrnent of the osteoarthritic knee with varus deformity.

7.Surgical Management of Ureteropelvic Junction Obstruction in Children.

From January 1988 to January 1996, 42 infants and children (44 renal units) had undergone surgical management to correct ureteropelvic junction obstruction. Median patient age was 8.5 years (range from 2 months to 17 years) and 11 patients were less than 1 year old at operation. Of 44 renal units surgically managed, 30 were on the left side and 10 were right side. 2 patients had undergone bilateral surgical management. Presenting symptoms were febrile urinary tract infection in 14 cases, abdominal pain in 14 cases, abdominal mass in 5 cases, gross hematuria in 3 cases and 3 cases were detected prenatally. We used imaging antegrade pyelography (AGP) in 15 cases, additional retrograde pyelography (RGP) in 12 cases and both AGP and RGP were performed in 1 case. To correct ureteropelvic junction obstruction, we performed dismembered pyeloplasty in 33 renal units, ureterolysis in 2 renal units, ureterocalycostomy in 1 renal unit, endopyelotomy in 1 renal unit and nephrectomy in 7 renal units. To diverge the urinary flow, we used nephrostomy in 19 renal units, ureteral stenting in 6 renal units and both nephrostomy and ureteral stenting were used in 10 renal units. As postoperative complications, restenosis was developed in 5 renal units, delayed open in 5 renal units, urinary tract infection in 2 renal units and wound infection, prolonged urine leakage, ureteral stone in each 1 renal unit. Postoperative success rate in followed-up patients, who had undergone pyeloplasty, was 91.6%. The success rate in children, who was less than 1 year old, was 100% and in children, who was more than 1 year old, was 88.4%. Finally we suggest that the surgical correction is safe and proper method for ureteropelvic junction obstruction in children. Additionally early operation of ureteropelvic junction obstruction is recommendable.

10.Early exploration of acute scrotum in children.

A retrospective analysis was made to evaluate the value of early surgical exploration of acute scrotum in 19 children. Testicular torsion (9 cases ) was the most frequent cause followed by epididymitis(4), hematocele(2). appendix testis torsion(2), infected hydrocele(1) and benign cystic teratoma(1). The age distribution of testicular torsion showed a biphasic pattern, neonatal and pubertal and 3 out of 9 torsed testes showed transverse lie in standing position on physical examination. The pyuria was present only in l out of 4 cases with epididymitis and leukocytosis was found in 7 cases, 6 with testicular torsion and 1 with epididymitis. Preoperative ultrasonographic diagnoses were correct in 2 out of 4 cases, 3 with testiculer torsion and 1 with epididymitis. Testicular salvage was possible in 3 torsed testes if surgery was performed within 14 hours from the onset of the symptom. In summary. testicular torsion was the most frequent cause of acute scrotum in children and early surgical exploration was necessary to improve the salvage of torsed testes.
