A retained surgical drain in the operation site is a rare complication that may cause clinical problems and result in medical disputes.The discovery of a retained drain is most often reported after abdominal and pelvis surgery but drain migration in the prevertebral space after anterior cervical spine surgery has not been reported. This paper reports a case of drain retained and migrated into the thorax level following anterior cervical spine surgery with a review of the relevant literature.
5.Mandibular implant overdenture using milled bar attachment on implants with unfavorable fixture angulation: A case report.

Implant overdenture exhibits higher retention, masticatory function, and patient's satisfaction compared to conventional dentures. Particularly, in treatment of severe alveolar bone resorption, implant overdenture can be considered as the first treatment option. The types of attachments used for implant overdenture can be classified into solitary type, which implants are not connected to each other, and bar type in which implants are connected. In the case of solitary type commonly used in clinical practice, parallel relationship is important. When it is not established, there is a higher risk of attachment wear, retention loss, and frequent visits for maintenance. In this case, satisfactory results have been obtained with implant overdentures using milled bar on two unparallel implants placed on the mandible.

6.Botulinum Toxin Treatment in a Patient with Frey Syndrome After Upper Thoracic Sympathicotomy.

Frey syndrome is characterized by profuse facial sweating which is provoked by the taste of food. It has been described as a consequence of upper thoracic sympathicotomy. Botulinum toxin injection is a relatively new treatment modality for Frey syndrome. Botulinum toxin type A (Botox , Allergan, USA) had been successfully used to treat Frey syndrome occurring in a 38-year-old patient following thoracic sympathicotomy for palmar hyperhidrosis. A total dose of 100U of botulinum toxin (1.0U/0.1ml/cm2) injection resulted in a 6 months' resolution of sweating, burning, and flushing sensation on eating. During the follow-up period, no systemic side effects were experienced.

7.Clinical Experiences of Lower Limb Amputation in Ischemic Arterial Disease.

PURPOSE: Despite the decrease in the number of lower limb amputation in ischemic limb patients as a result of advances in vascular reconstruction surgery, amputation still plays an important role in the management of end-stage peripheral vascular disease. Owing to the importance of the amputation level in postoperative rehabilitation and prevention of reamputation, there have been many reports defining theses level. Clinical characteristics were determined by retrospectively reviewing medical records of patients who underwent lower limb amputation for peripheral vascular disease. METHOD: Between June 1997 and September 2002, lower limb amputation was performed in 73 patients with peripheral vascular disease. RESULT: Mean follow-up period was 15 months; male to female ratio was 7.1 to 1; and mean age was 62.6 years. Associated diseases included DM (39 patients), hypertension (15 patients), ischemic heart disease (13 patients), and cerebrovascular disease (6 patients). Etiologies were atherosclerosis in 47.9%, Buerger's disease in 15.1%, DM foot in 13.7%, acute arterial embolization in 2.7%, ruptured abdominal aortic aneurysm in 1.4%, and combined atherosclerosis with DM foot in 19.2%. Bypass surgery was performed in 24 cases and only 4 cases underwent radiologic vascular intervention. The frequency of amputation was one surgery in 50 cases and more than two surgeries in 23 cases. Amputation level was digit amputation in 49 cases (67.1%), transmetatarsal in 10 cases (13.7%), below-knee in 11 cases (15.1%) and above-knee in 3 cases (4.1%). In 73 cases, reamputation was performed in 19 cases (26%) for poor stump wound healing. The cause of reamputation was atherosclerosis in 17.1%, Buerger's disease in 27.3%, DM foot in 60.0% and combined atherosclerosis with DM foot in 28.6%; the reamputation rate was the highest in DM foot patients. Bypass surgery for improved blood flow in the stump was performed in 24 cases, among these cases, reamputation was performed in 2 cases (8.3%). CONCLUSION: Amputation in ischemic limb patients was most commonly performed in artherosclerosis patients and the most common amputation level was digit. The reamputation rate was the highest in DM foot patients; a more careful selection of the amputation level in DM foot patients may be needed.

9.The effect of temperature and salinity on maturation and hatching of Fibricola seoulensis eggs.

This study was carried out to reveal the effect of temperature, salinity and aeration on maturation and hatching of Fibricola seoulensis eggs. The eggs were incubated and were observed daily for the appearance of eyespots and hatching. The results were summerized as follows: From 4-5 days after incubation in distilled water at 28C or at 11-26C, the eyespots began to appear and the rate of eggs with eyespots were over 90% in 28C on the 7th or 8th day. However, eyespots did not appear in 5-15C or 4C by the 18th day. The mature eggs began to hatch at the 8th day, and hatching rate 2 weeks after incubation was over 90% at 28C, but it was below 5% at 11-26C, and 0% at 5-15C and 4C. Aeration did not influence the appearance of eyespots nor hatching. In saline under 0.6%, the rates of eyespots were over 90% on the 7th day. The rate was 55.0% in 0.9% at 20 days, and 0% in 1.2%. The hatching rates in salines below 0.3% concentration were over 90% by 14 days of incubation. However, the rate decreased to 44% in 0.6% saline and to 0% over 0.9% salinity. The eggs incubated in the dark hatched in 12.5% on the 10th day, but hatching rate of maturation eggs increased to 85.7% within 2 hours after exposure to light. Above results demonstrated that the best temperature for maturation and hatching of F. seoulensis eggs was 28C, and the miracidia began to hatch at 8-9 days after incubation. In the field hatching and invasion into snails of the miracidia may occur from May to September in Korea. In salines under 0.3 percent concentration maturation and hatching were not influenced, but as salinity increased hatching was inhibited more than maturation was.

10.A Case of Synchronous Mucinous Tumor of the Ovary and Appendix.

The coexistence of mucinous ovarian and appendiceal tumors in associated with pseudomyxoma peritonei is well established. But, ovarian and appendiceal mucinous tumors occur without involvement of other organs is unusual. When the two lesions are morphologically similar, it is difficult to determine whether one reflects a metastasis or whether the tumors are independent. The typical synchronous presentation of the ovarian and appendiceal tumors, their histologic similarity, the frequency of bilaterality of the ovarian tumors, the predominance of right-sided ovarian involvement. Adenocarcinoma of the appendix constitutes 0.2 to 0.5 percent of all gastrointestinal tumors and accounts for 6 percent of primary malignant lesion of the appendix. The authors experienced a rare case of synchronous tumor of ovary and appendix in 59 years old female patient who has suffered for 3-4 months from abdominal pain. Pathologic examination of the resected specimen revealed a synchronous mucinous type tumor of vermiform appendix and ovary.
