1.FAG, ICG and OCT Findings in Multiple Evanescent White Dot Syndrome.

PURPOSE: We evaluated one patient with multiple evanescent white dot syndrome (MEWDS), using fundus fluorescein angiography (FAG), indocyanine green angiography (ICGA), optical coherence tomography (OCT) on acute phase of the disease, at 2 weeks after onset and at 10weeks. METHODS: Each diagnostic images were compared with patient's clinical course .OCT measurements of serial retinal thickness were done to assess the change in retinal thickness over the disease process. RESULTS: White dots on fundoscopic examination showed hypofluorescent on ICGA and imaged hyperreflective on OCT without presence of neither subretinal fluid nor neuroretinal increase in thickness. These ICGA and OCT findings suggest that lesions of perfusion defect on ICGA may be caused by interstitial precipitations of inflammatory cells in choriocapillaries and the underlying mechanism is not due to vasculitis with edema or serous exudation. CONCLUSIONS: ICGA and OCT allowed more precise anatomic evaluation than FAG in diagnosis and correlated well with clinical symptoms.

2.Toxoplasma antibodies by indirect latex agglutination tests in National Seoul Mental Hospital patients.

A total of 573 patients hospitalized in National Seoul Mental Hospital and 76 of healthy persons as control were examined by indirect latex agglutination test in order to evaluate Toxoplasma antibody titers in mental patients. Throughout this survey, 1:32 or more titers of diluted sera were regarded as positive. The 573 samples of test sera showed negative in 386 cases (67.4 percent), 1:2 in 93 cases (16.2 percent), 1:4 in 57 cases (9.9 percent), 1:8 in 14 cases (2.4 percent), 1:16 in 12 cases (2.1 percent), 1:32 in 5 cases (0.9 percent), 1:64 in 1 case (0.2 percent), 1:128 in 3 cases (0.5 percent) and 1:256 in 2 cases (0.3 percent) respectively. Among total 573 mental patients, 11 cases (19 percent) showed positive, and they were 9 cases (1.8 percent) of schizophrenia and 2 cases (7.4 percent) of manic depression. One case (1.3 percent) out of 76 control sera showed positive result.

3.A Gastric Composite Tumor with an Adenocarcinoma and a Neuroendocrine Carcinoma: A Case Report.

A 70-year-old woman was admitted to our department with epigastric discomfort and nausea over the duration of 1 month. An esophagogastroduodenoscopy showed the presence of a 1.0x1.0 cm-sized flat lesion with central ulceration at the greater curvature side of the antrum. A biopsy demonstrated the presence of an adenocarcinoma of well differentiated, intestinal type in the stomach. Endoscopic submucosal dissection was done and the diagnosis of a composite neuroendocrine carcinoma with an adenocarcinoma of the stomach was confirmed. We report a case of a gastric composite tumor with an adenocarcinoma and neuroendocrine carcinoma confirmed by endoscopic submucosal dissection with a review of the literature.

4.A Case of Leiomyosarcoma of the Spermatic Cord.

The leiomyosarcoma of the spermatic cord is a rare malignant tumor which constitutes approximately about 10% of sarcomas of the spermatic cord and tunics. In Korea, only one case has been reported. Because the cases presented are too small to be mentioned, the efficacy of retroperitoneal lymph node dissection, radiotherapy and chemotherapy are not settled. The standard therapy for leiomyosarcoma has been radical orchiectomy with high ligation of the spermatic cord. Here, we report a case of the leiomyosarcoma of the spermatic cord and review the role of retroperitoneal lymph node dissection.

5.The Efficacy of Aspirin and Acetaminophen in the Management of Delayed Onset Muscle Soreness.

OBJECTIVE: To investigate the efficacy of commonly available analgesics in the management of delayed-onset muscle soreness (DOMS) over an 8-day period, and to compare the efficacy between aspirin and acetaminophen. METHOD: Forty-two subjects were recruited. DOMS was induced by using the isokinetic dynamometer (KinCom(R)) in standardized fashion in the nondominant knee extensor with subjects seated at 30 degree-angle velocity. Subjects were asked to extend their non-dominant knee with concentric method and to hold the knee with eccentric flexion force at 30 degree-angle velocity, with maximal efforts. On this way, they did 10 repetitions, and then 3 cycles. We categorized four groups (n=10, for each group), that were control group with no medication, placebo group with placebo medication (antacid tablets), aspirin group with medication of 900 mg of aspirin, and acetaminophen group with medication with 3,900 mg of acetaminophen. Visual Analogue Scale (VAS: twice a day, until on day 8). and McGill Pain Questionnaire (MPQ: on day 1 and 3) were measured. RESULTS: We didn't find any significant difference of peak VAS score and relief time between four groups (P>0.05), The score of MPQ was not different between four groups (P>0.05). CONCLUSION: We concluded that the medication may not be beneficial, at least at the doses stated, in the management of DOMS.
