1.Clinical effect of laparoscopic ovarian endometriosis cyst resection and the influence of different operative methods on follicle loss

Objective To analyze the clinical effect of laparoscopic ovarian endometriosis (EMs) cyst resection and the influence of different operative methods on follicle loss.Methods Selected 86 EMs patients with laparoscopic ovarian cyst resection in our hospital from January 2013 to January 2016 and randomly divided them into two groups.The control group adopted traditional blunt tearing method,while the observation group adopted sharp isolating method.The specimen thickness along with the number,thickness and grading of specimen with ovarian cortex at hilus orarii,middle part of ovarian fossa and ovarian fossa were observed.Meanwhile,the influence on ovarian function and follicle loss of the two operation methods were compared.Results The specimen thickness close to the hilus orarii was more than that of the ovarian fossa and the middle part of ovarian fossa (t =4.474,t =3.520,P < 0.001).The thickness of specimen with ovarian cortex at ovarian fossa was significantly less than that of the middle part of ovarian fossa and the hilus orarii (t =1.705,t =4.428,P =0.046,P =0.000).The number of specimen ovarian cortex grading of 3 to 4 at hilus orarii was 20,which was significantly more than 2 at the ovarian fossa and 2 at the middle part(x2 =19.790,x2 =19.790,P < 0.001).The specimen thickness and ovarian cortical thickness of the middle part of the observation group were less than those of the control group (t =3.897,t =2.166,P =0.000,P =0.017).The specimen number of ovarian cortex at the hilus orarii was significantly less than the control group(x2 =5.512,P =0.019),and the specimen thickness and the ovarian cortex thickness were obviously less than those of the control group (t =4.992,t =2.349,P =0.000,P =0.011).After operation,the differences between FSH,LH,and E2 levels in the two groups were not statistically significant(P > 0.05),but the ovarian sinus follicle number and ovarian volume of the observation group were significantly more than those of the control group (t =7.181,t =10.921,P < 0.00 001).The pregnancy rate of the observation group was 60.0%,which was significantly higher than 43.75 % of the control group (x2 =5.289,P =0.021).Conclusion The laparoscopic ovarian EMs cyst resection can cause the great loss of follicle in the progress of resecting cyst at hilus orarii,and advanced sharp separation can obviously reduce the loss of normal ovarian follicles and cause smaller influence on ovarian function.
2.Clinical significance of serum fibrosis markers in diagnosis of infantile hepatitis syndrome.

OBJECTIVETo investigate the correlation between four serum fibrosis markers and liver function in patients with infantile hepatitis syndrome (IHS), and to explore the clinical significance of these markers in the diagnosis of IHS and the assessment of disease severity.
METHODSA retrospective study was performed on 60 patients with IHS who were divided into hepatic fibrosis and normal groups based on ultrasound diagnosis. Levels of four liver fibrosis markers, i.e., hyaluronic acid (HA), type III procollagen (PC-III), type IV collagen (IV.C), and laminin (LN), were compared between the two groups, and the correlation between these markers and liver function was analyzed.
RESULTSLevels of liver function markers (alanine aminotransferase (ALT), glutamyl transpeptidase (GGT), total bilirubin (TBil), direct bilirubin (DBil), indirect bilirubin (IBil), and total bile acid (TBA)) in the hepatic fibrosis group were significantly higher than those in the normal group (P<0.05). Levels of HA and IV.C in the hepatic fibrosis group were significantly higher compared with those in the normal group (P<0.05). Furthermore, HA, IV.C, and PC-III levels were positively correlated with those of ALT, TBil, GGT, DBil, IBil, and TBA (r=0.25-0.49), and the strongest correlation existed between HA/IV.C and ALT/jaundice markers.
CONCLUSIONSAssay measuring serum fibrosis markers (HA, IV.C, and PC-III) in combination with liver function tests and ultrasound examination has an important clinical value in the early diagnosis of IHS and evaluation of disease severity.

3.Comparative study on imaging and pathological features of elastofibroma dorsi.

BACKGROUND AND OBJECTIVEElastofibroma dorsi has an extremely low incidence. At present, comparative study on imaging manifestations and pathologic findings of elastofibroma dorsi has not been reported in China. This study was to investigate clinical manifestations, computed tomography (CT) and magnetic resonance imaging (MRI) appearances, and pathologic features of elastofibroma dorsi and to improve preoperative imaging diagnosis of this disease.
METHODSThe clinical manifestations, imaging findings, and pathologic appearances of 6 cases of elastofibroma dorsi were retrospectively analyzed and related literatures were reviewed. All patients were examined with MRI, and 4 of them were examined with CT scan.
RESULTSAll patients were above 30 years old without obvious symptoms. The tumors presented as a lenticular soft-tissue mass in the deep subscapular region. The tumor's density on plain CT scan or signal intensity on MR T1-weighted image was approximately equal to that of muscle with some interlaced fat-like areas within mass suppressed by fat-suppression MR sequences, which corresponded to dense collagen tissue and interspersed mature adipose tissue observed microscopically.
CONCLUSIONSCT and MRI can reflect the histological features of elastofibroma dorsi. On the basis of their imaging characteristics, a correct preoperative diagnosis of elastofibroma dorsi can easily be made.

4.Comparison of two nucleic acid extraction methods for norovirus in oysters.

OBJECTIVETo explore a convenient and effective method for norovirus nucleic acid extraction from oysters suitable for long-term viral surveillance.
METHODSTwo methods, namely method A (glycine washing and polyethylene glycol precipitation of the virus followed by silica gel centrifugal column) and method B (protease K digestion followed by application of paramagnetic silicon) were compared for their performance in norovirus nucleic acid extraction from oysters. Real-time RT-PCR was used to detect norovirus in naturally infected oysters and in oysters with induced infection.
RESULTSThe two methods yielded comparable positive detection rates for the samples, but the recovery rate of the virus was higher with method B than with method A.
CONCLUSIONMethod B is a more convenient and rapid method for norovirus nucleic acid extraction from oysters and suitable for long-term surveillance of norovirus.

5.Research progresses of the completed pediatrics projects funded by National Natural Science Foundation of China from 2002 to 2006.

OBJECTIVETo understand the projects completion and research progresses in pediatrics which were funded by the National Natural Science Foundation of China (NSFC), and evaluate the accomplishment objectively and justly.
METHODSThe completion status of projects in pediatrics funded by department of clinical medicine II from 2002 to 2006 was analysed retrospectively, and important research achievement and outstanding development in some projects were reported.
RESULTSDuring the period between 2002 and 2006, 420 articles were published, and the average was 8.1 papers per project, which included 56 papers that were published in journals indexed by SCI (the average was 1.1 papers per project). The completion of general project was better than that of "the Young Researchers Fund" and small grant project. Ten post-doctors, 102 doctors and 109 masters were trained. Two projects were awarded with the first grade prize and another 2 with the second grade prize at the provincial and ministerial level, 4 items applied for patent and 1 was granted. These completed projects, which were mainly related to 7 of 12 subspecialties in the field of pediatrics, such as the respiratory disease, nephrology, neurology, cardiology, endocrinology, hematology, neonatology, are the major portion of the application projects and subsidized projects funded by NSFC, and achieved great research progresses.
CONCLUSIONDuring the period between 2002 and 2006, the 52 completed projects in pediatrics showed difference in the distribution and quality of accomplishment among subspecialties and among types of supported projects; there are some gaps between pediatrics and some other clinical basic subspecialties II, this situation released the research status and problems in development of pediatrics in China. The general projects completion was good, and many projects obtained research achievements, which reflect the leading function of NSFC in pediatric research.

6.Relationship between mutations of epidermal growth factor receptor gene and clinicopathologic features of non-small cell lung cancers.

OBJECTIVETo explore the relationship between the mutations of epidermal growth factor receptor (EGFR) gene and clinicopathological characteristics in patients with non-small cell lung cancers (NSCLC).
METHODSParaffin-embedded tissue specimens were obtained from 1444 patients with NSCLC. The genomic DNA was extracted. Mutations of EGFR gene (exons 19 and 21) were detected by real-time PCR.
RESULTSDNA was available in 1410 cases. Somatic mutations of the EGFR gene were identified in 401 cases (27.8%). Among patients with EGFR mutations, 41.4% (n=166) had del E746-A750 of exon19, 6.7% (n=27) had del L747-P753insS of exon 19, 50.3% (n=201) had L858R of exon 21, and 1.5% (n=6) had L861Q of exon 21. Woman, non-smoker and adenocarcinoma showed a higher percentage of EGFR mutation (43.2%, 37.6%, and 33.5%, respectively). However, there was no association among age, grades, lymph node metastasis, and TNM stages (P>0.05). The mutation rate of BAC subtype (61.3%, 19/31) and adenocarcinoma with BAC features (48.0%, 12/25) was significantly higher than that of conventional adenocarcinoma (32.4%, 336/1038). A further assess of the smoking status found a trend that the more increased smoking exposure, the lower the incidence of EGFR mutations. A multivariable analysis revealed that adenocarcinoma, never smoking, and female were independently associated with EGFR mutations (odds rations=3.381, 2.393, and 1.727, respectively).
CONCLUSIONSThe detection rate of EGFR mutation is higher in Chinese patients, especially in non-smoking female patients with adenocarcinoma. Real-time PCR is a sensitive and accurate method to detect the mutations of EGFR gene and can therefore provide useful information for clinical treatment.

Objective To analyze the clinicopathological characteristics and prognosis of oligodendroglioma with IDH mutation and 1p/19q codeletion. Methods We collected the data of 54 oligodendroglioma patients with IDH mutation and 1p/19q codeletion.The patients'clinicopathological data, including age, histological grade, and tumor site, were analyzed for the effects on progression-free and overall survival. 8.Surgical treatment of obstructive azoospermia: a report of 56 cases.

OBJECTIVETo evaluate the diagnosis and surgical treatment of obstructive azoospermia.
METHODSWe analyzed the clinical data of 56 cases of obstructive azoospermia, 43 of them with ejaculatory duct obstruction (EDO), and the other 13 suspected of epididymal obstruction. The diagnostic methods included semen analyses, measurement of fructose and neutral alpha-glucosidase in the seminal plasma, transrectal ultrasonography (TRUS), and vasography when necessary. The 43 patients with EDO were treated by transurethral resection of the ejaculatory duct (TURED), and 11 of the 13 cases of suspected epididymal obstruction were confirmed by scrotal exploration and underwent either bilateral or unilateral vasoepididymostomy. The patients were followed up for 3 -51 months for postoperative semen quality and impregnation.
RESULTSOf the 43 azoospermia patients with EDO treated by TURED, 36 (83.7%) showed improved semen parameters and 11 (25.6%) achieved pregnancies. Among the 11 cases of azoospermia with confirmed epididymal obstruction treated by vasoepididymostomy, 6 (54.5%) had sperm in the semen assay and 3 (27.3%) achieved pregnancies.
CONCLUSIONSemen analyses, measurement of fructose and neutral alpha-glucosidase in the seminal plasma, TRUS and vasography are important diagnostic methods for obstructive azoospermia. TURED is effective for azoospermia with EDO, while vasoepididymostomy is preferable for the treatment of azoospermia with epididymal obstruction.

9.Relationship between serum bilirubin levels and optic neuritis.

BACKGROUNDBilirubin is the end product of heme catabolism and has strong antioxidant properties. Serum bilirubin levels are reported to be reduced in patients with multiple sclerosis (MS) and neuromyelitis optica (NMO). The pathophysiology of optic neuritis (ON) resembles that of MS; however, the role of endogenous bilirubin in ON is unclear. The aim of this study is to measure serum bilirubin levels in patients with ON, and to investigate the correlation between ON and serum antioxidant status of bilirubin.
METHODSSerum levels of bilirubin were measured in 42 patients with ON, 50 patients with multiple sclerosis (MS), 48 patients with neuromyelitis optica (NMO) and 48 healthy control subjects.
RESULTSSerum total bilirubin (Tbil), direct bilirubin (Dbil) and indirect bilirubin (Ibil) levels in patients with ON were significantly lower than those in the healthy controls. However, no statistical significance was found between levels in the ON and MS, ON and NMO, and MS and NMO groups. In patients with ON, serum Tbil, Dbil, and Ibil levels were lower in those with recurrence or those with ON for a longer duration (≥ 1 year). Moreover, Tbil, Dbil, and Ibil concentrations were lower in patients with papillitis than in those with retrobulbar type ON, but the differences were not statistically significant.
CONCLUSIONSLow antioxidant status may exist in patients with ON. But serum levels of Tbil, Dbil, and Ibil did not correlate with clinical presentations, such as recurrence, duration of disease and subtypes of ON. Low antioxidant status already existed in MS or NMO patients before systemic symptoms appeared.

10.Diagnostic performance of prospective versus retrospective electrocardiogram-gated 64-slice computed tomography coronary angiography.

OBJECTIVETo compare the diagnostic performance of prospective electrocardiogram (ECG)-triggered computed tomography coronary angiography (CTCA) versus retrospective ECG-gated CTCA.
METHODSPatients with suspected coronary artery disease divided into two groups which underwent 64-slice CTCA with prospective ECG-triggered or retrospective ECG-gated scanning (n = 100 each, HR < or = 65 bpm). Multi-planar reconstruction (MPR), curved-planar reconstruction (CPR), maximum intensity projection (MIP) and volume rendering (VR) were made to demonstrate the coronary arteries. The image quality was defined as excellent, good and poor by motion and stair-step artifacts. Individual radiation exposure dose was estimated from the dose-length product.
RESULTSThe mean effective radiation dose of prospective ECG-triggered CTCA [(2.81 +/- 0.48) mSv] was significantly lower than that of retrospective ECG-gated CTCA [(10.16 +/- 1.09) mSv, P < 0.01]. Segments of diagnostic image quality (95.2%, 1165/1224) and non-diagnostic coronary segments (4.8%, 59/1224) in prospective ECG-triggered group were similar as those of retrospective ECG-gated group (94.1%, 1186/1261 and 5.9%, 75/1261, all P > 0.05).
CONCLUSIONThough the radiation exposure dose required is significantly lower, the diagnostic performance of prospective ECG-triggered 64 slice CTCA is comparable with that of retrospective ECG-gated 64 slice CTCA on patients with stable heart rates up to 65 bpm.
