Journal of Shanghai Jiaotong University(Medical Science) 2006;0(09):-
Objective To assess the relationship between GABA_(B) receptor in medulla and trigeminal neuralgia on rats. Methods Twelve SD rats were randomly divided into 4 groups,3 for each group.Group A and B were surgical groups, while Group C and D were sham surgical ones.In the surgical group, right unilateral chronical constriction injury(CCI) of rats was produced by placing loose chromic gut ligature around the infraorbital nerve(ION).In the sham surgical group, the ION was only exposed using the same procedure but not ligated.Mechanical response threshold was observed before operation and 3,6,9,12 and 15 days after operation.Medullas of rats in group A and B were taken to measure the quantity of GABA_(B) receptor by real-time PCR 9 days after operation, and medullas of rats in group C and D were done 15 days after operation. Results Compared with the sham surgical group,an allodynia to mechanical stimulation on the territory of ligated ION was found from the ninth to fifteenth day after operation in surgical group(P

As a non-pungency inhaled agent with less respiratory irritation, sevoflurane has been widely used in various surgeries for induction and maintenance of general anesthesia. However, the incidence of pediatric emergence delirium has been significantly increased with sevoflurane general anesthesia which severely disturbs the surgical effect and increases the requirement of medical staff in post anesthesia care unit. This article systematically reviews the research advance in emergence delirium in children with sevoflurane general anesthesia and introduces the reasonable intervention for the improvement of anesthesia quality.
3.Toll-like receptor 4 and human fibrotic diseases
Journal of Shanghai Jiaotong University(Medical Science) 2009;29(7):873-876
Toll-like receptor 4 (TLR4), the natural receptor of lipopolysaccharide(LPS), plays an important role in the regulation of acute inflammation, intracellular signal transduction and cell apoptosis. TLR4 over expresses in both acute inflammation and fibrotic tissues, which suggests that TLR4 may be essential in acute inflammation and subsequent fibrosis. Nowadays, some progress has been made in the exploration of TLR4 in pathogenesis and treatment of liver cirrhosis and pulmonary fibrosis. TLR4 may become a new target in the treatment of human fibrotic diseases including liver cirrhosis and pulmonary fibrosis. The research status of TLR4 in human fibrotic diseases and its future development are reviewed in this paper.

Objective To evaluate the effects of nutritional support combined with human growth hormone recombinant (rhGH) in patients with mechanical ventilation. Methods One hundred and fifty cases in ICU were randomized into test group (nutritional support combined with rhGH, 80 cases) and control group (nutritional support,70 cases). Both groups received calories according to Harris-Benedict formula [N, 0.25 g/(kg· d)]. In addition, the test group received subcutaneous injections of rhGH at a dose of 0. 1 U/ (kg· d) for 7 days. The different effects on nutritional status indexes, immunological function indexes and clinical indexes were compared. Results Compared with the control group, there were significant improvement in nutritional status indexes and immunological function indexes after the treatment of 7 days. The success rate of once weaning was higher in test group than that in control group. There were significant differences in the level of transferrin, prealbumin, CD3, CD4, NK between two groups [(3.0 ± 0.6)g/L vs. ( 1.9 ± 0.4) g/L, (226.46 ± 27.02 ) mg/L vs. ( 188.68 ± 17.27 ) mg/L, (53.8 ± 3.3 ) % vs. (47.4 ±4.1 ) %, ( 0.41 ± 0.14 ) % vs. (0.25 ± 0.05 ) %, ( 16.1 ± 2.1 ) % vs. ( 10.9 ± 3.0 ) %] ( P < 0.01 ). Conclusion rhGH combined with nutritional support has more effects on improving nutritional status compared with those of single nutritional support. It has significant adjustment effect on immunological function in patients with mechanical ventilation.

BACKGROUND: Many methods involved in measuring energy expenditure of physical action, however, study regarding measurements of upper limber movement energy expenditure are few.OBJECTIVIE: To measure the energy expenditure of limber movement using indirect calorimetry, and to analyze the characteristics of upper limber movement energy expenditure and influencing factors such as age and gender.DESIGN, TIME AND SETTING: The comparison observation was performed at the Jiangsu Institute of Sports Science from January to March 2009.PARTICIPANTS: Totally 108 health adults, including 47 males and 61 females, participated in the study, 65 of them aged 20-39 years, and the remained 43 aged 40-59 years.METHODS: Energy expenditure of 4 upper limb movements were performed, including sitting posture arm swing (60 times/min),elbow flexion (40 times/min), shoulder flexion-extension (60 times/min), and lateral arm raise (30 times/min). At each interval, have a 3-minute rest.MAIN OUTCOME MEASURES: Oxygen expenditure of 4 kinds upper limb movements were measured by MetaMax 3B tester. RESULTS: Rest oxygen expenditure of males were greater than that of females (P < 0.05), greater in 20-39 years old than 40-59 years old (P < 0.05). The greatest oxygen consumption occurred in males with arm swing [(550.9±90.6) mL/min], which was (425.8±75.7) mL/min in females. The lowest oxygen expenditure was elbow bend [male: (440.4±82.7) mL/min, female: (367.0±60.1) mL/min]. Net oxygen expenditure of all 4 kinds of upper limb movements was lower than 250mL/min (1 metabolic equivalent).CONCLUSION: ① The energy expenditure of usual upper limber movements below 1 metabolic equivalent. ② Age and body mass has more effect on resting energy expenditure than the gender. ③ The efficiency of young adults upper limb movements is higher than old adults.
