1.Transurethral electrotomy for cystis vesicular seminalis induced by obstruction of the distal end of the ejaculatory duct.

OBJECTIVETo investigate the treatment of cystis vesicular seminalis induced by the obstruction of the distal end of the ejaculatory duct.
METHODSFrom November 2005 to December 2006,12 cases of cystis vesicular seminalis ( [2.3 +/- 1.1] cm) were diagnosed by semen analysis (as on the seminal volume, pH and fructose), transrectal palpation and ultrasonography. All cases were treated by transurethral incision or resection of the obstructive ejaculatory duct till milky semen discharged.
RESULTSThe cysts were significantly reduced ([1.0 +/- 0.8] cm, P < 0.05) in all the 12 cases and no complications were observed during the follow-up 1, 3 and 12 months later.
CONCLUSIONTransurethral electrotomy is a simple and effective method for the treatment of cystis vesicular seminalis induced by the obstruction of the ejaculatory duct.

2.Mean platelet volume predicts early neurological deterioration in patients with acute ischemic stroke treated with intravenous thrombolysis

Objective:To investigate the predictive value of mean platelet volume (MPV) for early neurological deterioration (END) after intravenous thrombolysis in patients with acute ischemic stroke.Methods:Patients with acute ischemic stroke treated with intravenous thrombolysis in the Department of Neurology, Jiangsu Provincial Hospital of Chinese Medicine from September 2016 to December 2019 were enrolled retrospectively. Immediately after admission, hematology analyzer was used to detect the MPV level. END was defined as the National Institutes of Health Stroke Scale (NHISS) score at any time point within 7 d after the admission increased by ≥2 from baseline. Univariate analysis was used to compare the differences in baseline data between the END group and the non-END group, and multivariate logistic regression analysis was used to determine the independent risk factors for END. The receiver operator characteristic (ROC) curve was used to evaluate the predictive value of MPV for END. Results:A total of 103 patients were enrolled. Their age was 68.0±13.3 years old, and 80 were male (77.7%). There were 28 patients (27.2%) in the END group and 75 (71.8%) in the non-END group. The proportion of ischemic heart disease (28.6% vs. 9.3%; χ2=6.065, P=0.014), baseline NIHSS score (8.5 [4.0-16.5] vs. 5.0 [3.0-11.0]; Z=2.198, P=0.028), symptomatic intracranial hemorrhage (1.9% vs. 0%; χ2=5.463, P=0.019), low-density lipoprotein cholesterol (2.9 [2.2-3.4] mmol/L vs. 2.4 [1.8-3.1] mmol/L; Z=2.165, P=0.030), high-sensitivity C-reactive protein (7.0 [1.2-36.0] mg/L vs. 2.1 [1.0-6.0] mg/L; Z=2.159, P=0.031) and MPV levels ( 10.4±1.3 fl vs. 9.5±1.2 fl, t=2.771, P=0.007) of the END group were significantly higher than those of the non-END group. Multivariate logistic regression analysis showed that after adjusting for the confounding factors, the larger MPV was an independent predictor of END (odds ratio 1.668, 95% confidence interval 1.079-2.579; P=0.021). ROC curve analysis showed that MPV had a certain predictive value for END (area under the curve, 0.653, 95% confidence interval 0.533-0.774). Its optimal cutoff value was 102 fl, and the sensitivity and specificity at this time were 57.4% and 70.3%, respectively. Conclusion:A larger baseline MPV had a certain predictive value for END after intravenous thrombolysis in patients with acute ischemic stroke.
3.Dexmedetomidine Receptor Dependently Enhanced Macrophage Phagocytosis

【Objective】To investigate the effect of dexmedetomidine on the phagocytosis of macrophages.【Methods】 RAW264.7 cells were divided into control group,DEX group,and BRL44408 + DEX group. Expression of α2A adrenergic receptor,p-Akt and Akt were detected by Western Blotting;Phagocytosis Assay Kit(IgG PE)was used to measure the phagocytosis of macrophages. 【Results】 α2A adrenergic receptor was detected in RAW264.7 cells;Dexmedetomidine could enhance the phagocytosis of macrophages(P < 0.001),and BRL44408 reversed the enhancement of phagocytic ability of macrophages(P < 0.001);Dexmedetomidine upregulated the expression of Akt in RAW264.7 cells,while the use of BRL44408 inhibited the activation of the Akt pathway(P < 0.01).【Conclusion】Dexmedetomidine could enhance phagocytosis of RAW264.7 by activating the Akt pathway through the α2A adrenergic receptor.
4.Repair of pharynx with acellular dermal matrix.

OBJECTIVETo evaluate the clinical use of acellular dermal matrix (ADM) in the repair of pharynx.
METHODFrom June 2003 to December 2004, 18 cases of laryngopharyngeal carcinoma or oropharyngeal carcinoma accepted treatment of surgery. Four of them had the tumor in the place of posterior wall of laryngopharynx or oropharynx. In the operation, the tumor was resected and the function of larynx was reserved, reconstruction of the posterior wall of laryngopharynx or oropharynx was made with ADM. Three of them had normal function of vocal cords and the tumor was in the place of posterior wall of pharynx and lateral wall of piriform fossa. These patients accepted resection of the tumor and the larynx was reserved. ADM was sutured to the prevertebral fascia to repair the pharynx, reinforcement was made with sternocleidomastoid muscle then. The other 11 patients had the tumor in the pharynx and cervico-esophagus and the vocal cords were fixed. These cases accepted total resection of the larynx and laryngopharynx. ADM was sutured to the prevertebral fascia to repair the posterior wall of laryngopharynx, musculo-cutaneous flap of pectoralis major muscle was made to repair the anterior and lateral walls. All these 18 patients accepted radiotherapy after the operation, the dosage was 60-70 Gy.
RESULTSAmong all these 18 cases, no rejection of ADM or formation of pharynx fistula happened after the operation. The allograft became normal mucosa finally. Two patients had subcutaneous infection. After the treatment of dressing change and antibiotics, the wound healing was satisfying. All the 18 patients had resumption of oral diet finally but 3 of them had light mis-swallowing. Seven patients whose larynx were reserved had the trachea cannula pulled out The follow-up periods after the operation lasted 12 months to 30 months. The middle follow-up period was 19. 38 months. Neither rejection nor scar formation of the allograft happened. The follow-up of 11 patients lasted more than 18 months, 3 of them had relapse of tumor. After the second operation 1 patient was still alive and 2 patients died of distant metastasis.
CONCLUSIONADM can be easily obtained. Its histocompatibility were perfect The operative procedure were easy to perform. The complications of donor part can be totally avoided. The combined use of ADM and musculo-cutaneous flap of pectoralis major muscle or other neck tissue was safe and effective in the reconstruction of pharynx.

5.Mutations of 15 short tandem repeat loci in Chinese population.

OBJECTIVETo explore the mutations of 15 short tandem repeat (STR) loci in PlowerPlex16 System which are world-widely used in parentage testing.
METHODSMutations of 15 STR loci in PlowerPlex16 System were investigated in 1921 parentage testing cases from Chinese population.
RESULTSIn 1921 parentage cases, seventy cases (3.644%) were found to have mutations. Among these were one case with double mutations (D21S11 and PentaD) and another case with two different mutations (D7S820 and D16S539) in two children. The total number of mutated STR loci observed was 72 over 3764 meiosis with a mutation rate of 0.128% +/- 0.1104% x 10(-3). The highest mutation rate was 0.292% at vWA and D21S11. No mutation was observed at TH01 or at TPOX. The mutated alleles coming from father were five times more than those from mother. The majority (98.611%) of mutated alleles were the results of one-step mutation. The ratio of one-step gain versus loss was 1.826:1. There was only one multiple-step mutation with a double-repeat gain observed at PentaD locus. In the PlowerPlex16 System, nine loci, namely D8S1179, Penta D, D13S317, D16S539, D7S820, D5S818, D3S1358, TH01 and TPOX, have lower mutation rates and are more suitable for parentage testing.
CONCLUSIONMutation of STR is relatively common and often makes parentage testing more complicated. Selecting stable STR locus with low mutation rate is more important in parentage testing.

6.Mesenchymal stem cells transplanted in mdx mice differentiate into myocytes and express dystrophin/utrophin.

OBJECTIVETo investigate the differentiation of rat bone marrow mesenchymal stem cells (MSCs) into myocytes and their expression of dystrophin/utrophin after transplantation in mdx mice.
METHODSBrdU-labeled fifth-passage rat MSCs were transplanted in mdx mice with previous total body gamma irradiation (7 Gy). At 4, 8, 12 and 16 weeks after the transplantation, the mice were sacrificed to detect dystrophin/BrdU and utrophin expressions in the gastrocnemius muscle using immunofluorescence assay, RT-PCR and Western blotting. Five normal C57 BL/6 mice and 5 mdx mice served as the positive and negative controls, respectively.
RESULTSFour weeks after MSC transplantation, less than 1% of the muscle fibers of the mdx mice expressed dystrophin, which increased to 15% at 16 weeks. Donor-derived nuclei were detected in both single and clusters of dystrophin-positive fibers. Some BrdU-positive nuclei were centrally located, and some peripherally within myofibers. Utrophin expression decreased over time after transplantation.
CONCLUSIONThe myofibers of mdx mice with MSC transplantation express dystrophin, which is derived partially from the transplanted MSCs. Dystrophin expression from the transplanted MSCs partially inhibits the upregulation of utrophin in mdx mouse muscle, showing a complementary relation between them.

7.Comparative study on gasless endoscopic versus conventional thyroid surgery.

OBJECTIVETo evaluate the efficiency, feasibility and safety of gasless endoscopic thyroid surgery through compared research.
METHODSFrom July 2001 to Oct. 2005, 83 cases that had received gasless endoscopic thyroid surgery were compared with 80 cases that had received conventional surgery and analyzed on operating time, blood loss, cosmetic effects, complications and costs.
RESULTSCompared with conventional thyroid surgery, endoscopic thyroid surgery group had better cosmetic effects. There was only minimal neck wound or even no neck wound. The cost of hospitalization in the endoscopic group was higher than that in the conventional group (10,844 +/- 2373) RMB and (7585 +/- 1081) RMB respectively (t = -7.23, P < 0.001). The endoscopic group had less blood loss than the conventional group (25 +/- 21) ml and (36 +/- 23) ml respectively (t = 3.160, P < 0.01), but had longer operating time (107 +/- 30) min and(79 +/- 19) min respectively (t = -7.225, P < 0.01). However, when the surgeon had got more experiences and applied the advanced harmonic scalpel, the operative time and blood loss reduced obviously.
CONCLUSIONSCompared with conventional thyroid surgery, gasless endoscopic thyroid surgery has the major advantage that it has cosmetic effect and has small or no scar left on neck. It can be concluded that this technique was efficiency, feasibility and safety. It can offer another choice for the thyroid neoplasms patients.

9.Role of interleukin-8 in the progression of estrogen receptor-negative breast cancer.

BACKGROUNDEstrogen receptor (ER) is a very important biomarker of breast cancer. ER deletion has been consistently associated with tumor progression, recurrence, metastasis and poor prognosis, but the biological mechanism is still unclear. ER negative breast cancer expresses high levels of interleukin-8 (IL-8). ER expression can downregulate IL-8 promotor activity. As a multifunctional cytokine, IL-8 has many important biological activities in tumor genesis and development. With the goal of investigating the role of IL-8 in ER-negative breast cancer progression, we applied RNA interference technology to specifically knockdown the IL-8 expression in ER-negative breast cancer cell line MDA-MB-231.
METHODSInterfering pRNA-IL-8 and the control was transfected into ER (-) MDA-MB-231. The proliferation, cell apotosis, and invasive ability were recorded in transfected, untransfected and negative transfected cells. These cells were injected into nude mice to assess tumorigenicity, proliferation, metastasis and microvessel density (MVD).
RESULTSIn vitro, decreased expression of IL-8 was associated with reduced cell invasion (P < 0.001), but had no effect on cell proliferation (P > 0.05). In vivo, neutrophils infiltration was significantly inhibited in pRNA-IL-8 transfected cells compared with untransfected and negatively transfected cells (P = 0.001, P < 0.001). Less metastasis was found in transfected cells compared with negatively transfected cells (0% vs 80%, P = 0.048). Nevertheless, we observed less MVD in transfected cells compared with control in nude mice (P < 0.001).
CONCLUSIONSIL-8 inhibits ER-negative breast cancer cell growth and promotes its metastasis in vivo, which may be correlated with neutrophils infiltration induced by IL-8.

10.Cyclosporine, prednisone, and high-dose immunoglobulin treatment of angioimmunoblastic T-cell lymphoma refractory to prior CHOP or CHOP-like regimen.

Angioimmunoblastic T-cell lymphoma (AITL) is a rare, distinct subtype of peripheral T-cell lymphoma, possessing an aggressive course and poor prognosis with no standard therapy. Twelve patients who have failed at least two initial CHOP or CHOP-like regimens were enrolled in this study and treated with individualized cyclosporine (CsA), prednisone (PDN), and monthly, high-dose intravenous immunoglobulin (HDIVIG). The dose of CsA was adjusted individually based on the blood trough concentration of CsA and renal function. All patients were examined for response, toxicity and survival. The most significant toxicities (≥ grade 2) were infection (16.7%), renal insufficiency (8.3%), hypertension (8.3%), diabetes (8.3%) and insomnia (16.7%). Discontinuation of treatment occurred in one patient (8.3%) due to grade 3 renal toxicity and subsequent grade 4 pulmonary infection. Treatment-related death was not observed. The overall response rate was 75.0% (complete response, 33.3%; partial response, 41.7%). With a median follow-up of 25.5 months, the median duration of response was 20 months (range, 12 to 49 months) and the median progression-free survival (PFS) was 25.5 months (range, 10 to 56 months). The 2-year PFS rate was 81.5%. Our findings indicate the combination of CsA, PDN and HDIVIG is an effective salvage regimen for refractory or relapsed AITL with predictable and manageable toxicity.
