2.Clinical effects of bronchoscope combined hot and cold ablation technique in benign and malignant tumors in the airway

Objective To analyze the clinical effects of bronchoscope combined with hot and cold ablation in treatment of benign and malignant tumors in primary tracheal and bronchial. Methods From January 2013 to November 2015, 30 cases of pathological types of primary tracheal / bronchial benign and malignant tumors patients include: 8 cases of hamartoma, 1 case of lipoma, 4 cases of granuloma, 1case of clear cell carcinoma, 7 cases of small cell lung cancer, 9 cases of squamous cell carcinoma, were treated by using carbon dioxide cryotherapy combined hot ablation under bronchoscope. Then investigated the outcomes. Results 30 patients, 22 males, 8 females, suffered from 1 month to 2 years, underwent surgery bronchoscope interventional treatment under local anesthesia combined with intravenous anesthesia, are taking joint cold ablation technique. All patients airway is completely opened, the lifting of the airway obstruction. Including 19 cases of disposable surgical removal of the lesion; 8 cases of lesions due to wide distribution, 2 times surgery; three cases three times surgery, no serious complications, follow-up without recurrence, blood gas analysis and dyspnea index were significantly improved. Conclusion Bronchoscope cold joint ablation techniques in the treatment of primary tracheal, bronchial benign and malignant tumors is effective, simple, minimally invasive and safe.
3.Neuroprotection of erythropoietin and methylprednisolone against spinal cord ischemia-reperfusion injury.

Recent research based on various animal models has shown the neuroprotective effects of erythropoietin (EPO). However, few studies have examined such effects of EPO in the clinic. In this study we enrolled patients with spinal cord ischemia-reperfusion (I-R) injury to investigate the clinical application of EPO and methylprednisolone (MP) for the neuroprotection against spinal cord I-R injury. Retrospective analysis of 63 cases of spinal cord I-R injury was performed. The Frankel neurological performance scale was used to evaluate the neurological function after spinal cord injury (SCI), including 12 cases of scale B, 30 cases of scale C, and 21 cases of scale D. These cases were divided into 2 groups: group A (27 cases) got treatment with both EPO and MP; group B (36 cases) got treatment with MP only. The neurological function of patients after treatment was evaluated by American Spinal Cord Injury Association (ASIA) index score, and activity of daily living (ADL) of the patients was also recorded. All patients got follow-up and the follow-up period ranged from 24 to 39 months (mean 26 months). There was no significance difference in neurological function between groups A and B before the treatment (P>0.05). However, the neurological function and ADL scores were significantly improved 1 week, 1 year or 2 years after the treatment compared to those before the treatment (P<0.05), and the improvement was more significant in group A than in group B (P<0.05). It is suggested that the clinical application of EPO and MP provides the neuroprotection against spinal cord I-R injury.
4.Curved vertebroplasty device for thoracolumbar osteoporotic vertebral compression fractures

BACKGROUND:Currently, the cement delivery device used in vertebral augmentation vertebral augmentation (vertebroplasty and kyphoplasty) has flat tips that cannot be bent. During surgery, we always choose injection cement by symmetric way to keep the balance of power on the biomechanics of the vertebral body, while the traditional cement delivery tube can only be used in ipsilateral vertebral body.
OBJECTIVE:To evaluate the feasibility of curved vertebroplasty device for the treatment of thoracolumbar osteoporotic vertebral compression fractures.
METHODS: Sixty patients (72 vertebral bodies) with osteoporotic vertebral compression fractures were enroled, including 8 cases of double-level vertebral fractures and 2 cases of three-level vertebral fractures. Al operations applied curved delivery device in percutaneous vertebroplasty. We recorded operation time, intraoperative blood loss, and X-ray imaging data through prospective self control study. Treatment effect was evaluated with modified Oswestry disability index, Visual Analogue Scale scores and the World Health Organization Quality of Life assessment. The patients were folowed up for 6 months to 2 years.
RESULTS AND CONCLUSION: (1) Surgery was performed successfuly on al 60 patients. The average operation time was 27 minutes (20-45 minutes) for 50 cases by unilateral transpedicular approach. (2) The mean amount of bone cement in every single vertebral of al 72 vertebrae was averagely 6.4 mL (4.2-9.5 mL). Bone cement distribution had not been found to be biased one-sided without severe leakage. (3) Visual Analogue Scale score and Oswestry disability index were significantly improved at 1 day after treatment (P < 0.05). During the final folow-up, no significant difference was detected as compared with 1 day postoperatively (P > 0.05). (4) The ratio of good quality of life in 1 month than postoperatively (87%) and in the last folow-up (92%) was significantly greater compared with the preoperative ratio (27%) (P < 0.05). (5) These findings confirmed that curved vertebroplasty device in unilateral pedicle puncture could ensure bilateral symmetric distribution of bone cement. Al patients are satisfied with treatment. It is proved to be a feasible simple and safe method.
5.Clinical study on the second surgical procedure of adjacent segment degeneration after the first time spinal fusion

Objective To discuss the effectiveness of the second surgical procedure of adjacent segment degeneration after the first spinal fusion.Method 35 patients who had been performed spinal fusion in our hospital or had symptoms recurred or aggravated after 12 ~ 114 (42 ±35) months of the prior surgery were enrolled in this study.A second surgery was performed and intraoperative the intradiscal pressure of adjacent segments of degeneration and normal segments was measured.The VAS score systems were compared among prior surgery,3 months later and 2 years after the second surgery.Result The intradiscal pressure of adjacent segments after the cervical vertebra and lumbar vertebra fusion were [ (15 ± 4.6)cmH2O,(23 ±5.2)cmH2O],much higher than normal segments [ (3 ±2.3)cmH2O,(8 ±4.1)cmH2O](P <0.01).The VAS score systems of 3 months later and 2 years after the second surgery were [ (2.9 ±0.7),(2.0 ± 0.6) ],which were dramatically lower than the prior (7.8 ± 1.2) (P < 0.01).In 12 ~ 46(31 ± 12) months of follow-up after the second procedure,X-ray and MRI showed that fusion segments reached nearly bone fusion,well decompressed and without nerve compression or other complications.Conclusion If recurrence of symptoms after spinal fusion were caused by adjacent segment degeneration,reoperation would guarantee good clinical outcome.
6. Combined analysis of CRMP4 methylation levels and CAPRA-S score predicts metastasis and outcomes in prostate cancer patients

The present study analyzed the predictive value of combined analysis of collapsin response mediator protein 4 (CRMP4) methylation levels and the Cancer of the Prostate Risk Assessment (CAPRA-S) Postsurgical score of patients who required adjuvant hormone therapy (AHT) after radical prostatectomy (RP). We retrospectively analyzed 305 patients with prostate cancer (PCa) who received RP and subsequent androgen deprivation therapy (ADT). Two hundred and thirty patients with clinically high-risk PCa underwent immediate ADT, and 75 patients with intermediate risk PCa underwent deferred ADT. CRMP4 methylation levels in biopsies were determined, and CAPRA-S scores were calculated. In the deferred ADT group, the values of the hazard ratios for tumor progression and cancer-specific mortality (CSM) in patients with ≥15% CRMP4 methylation were 6.81 (95% CI: 2.34-19.80) and 12.83 (95% CI: 2.16-26.10), respectively. Receiver-operating characteristic curve analysis indicated that CRMP4 methylation levels ≥15% served as a significant prognostic marker of tumor progression and CSM. In the immediate ADT group, CAPRA-S scores ≥6 and CRMP4 methylation levels ≥15% were independent predictors of these outcomes (uni-and multi-variable Cox regression analyses). The differences in the 5-year progression-free survival between each combination were statistically significant. Combining CAPRA-S score and CRMP4 methylation levels improved the area under the curve compared with the CRMP4 or CAPRA-S model. Therefore, CRMP4 methylation levels ≥15% were significantly associated with a poor prognosis and their combination with CAPRA-S score accurately predicted tumor progression and metastasis for patients requiring AHT after RP.

This review summarizes the present advance of effects of stress on hippocampal structure and function and the role of hippocampus in feedback regulation of thalamic-pituitary-adrenocortical (HPA) axis during stress. It shows that stress can affect hippocampal structure and function, on the other hand, the hippocampus can also suppress the stress reaction through the feedback regulation of HPA axis, but chronic stress can attenuate this regulation, then significantly impair its structure and function.

8.Study on Material Basis of Miao Medicine Bi-Ning Spray in Allergic Rhinitis Treatment

This paper was aimed to study the material basis for the efficacy of Miao medicine Bi-Ning spray in the treatment of allergic rhinitis,and to lay a foundation for improvement of its quality standard.Solidago and Centipeda minima were weighed according to the prescription and then soaked in water.Reflux extraction was conducted to the liquid.Petroleum ether,ethyl acetate,and n-butanol were used in the extraction,respectively.Each received composition was dried and made into sample solution for the experiment.A total of 60 healthy adult SD rats,with the ratio of half male and half female,were selected and randomly divided into 6 groups according to the weight with 10 rats in each group.There were the normal group,the model group,the petroleum ether (SYM) group,the ethyl acetate (YSYZ) group,the nbutyl alcohol (ZDC) group and the water group.The 20% xylene olive oil solution was used in the model establishment of rats from different groups except the normal group.Intranasal administration of drug was given to different dosage group after the models were successfully established.Normal saline was given to the normal group and the model group.Serum of each experimental rat was collected at the end of the experiment.The contents of IgE,IL-4 and HT in serum were detected by ELISA.The pathological morphology of the nasal mucosa was analyzed.The results showed that compared with the normal group,the contents of IgE,IL-4 and HT in serum of the model group increased significantly (P < 0.05).Compared to the model group,the contents of IgE,IL-4 and HT in serum of the water group decreased significantly (P <0.05).Contents of IgE,IL-4 and HT in other groups presented decreasing tendency.However,there was no statistical significance.The pathological morphology of the nasal mucosa results showed that compared with the normal group,the cell inflammatory response of the nasal mucosa tissues significantly increased in the model group (P < 0.01).Compared with the model group,the cell inflammatory response of nasal mucosa tissues significantly decreased in the ZDC group and water group (P < 0.05).There was no statistical significance in other groups.It was concluded that components in water part were the main material basis for efficacy of Bi-Ning spray in the treatment of allergic rhinitis.

Oxidative stress is the key determinant in the pathogenesis of noise-induced hearing loss (NIHL). Given that cellular defense against oxidative stress is an energy-consuming process, the aim of the present study was to investigate whether increasing energy availability by glucose supplementation protects cochlear hair cells against oxidative stress and attenuates NIHL. Our results revealed that glucose supplementation reduced the noise-induced formation of reactive oxygen species (ROS) and consequently attenuated noise-induced loss of outer hair cells, inner hair cell synaptic ribbons, and NIHL in CBA/J mice. 10.Hidden hearing loss in tinnitus patients with normal audiograms: implications for the origin of tinnitus.

OBJECTIVE:
To investigate hidden hearing loss in tinnitus patients with normal audiograms by means of auditory brainstem response (ABR) and explore the origin of tinnitus.
METHOD:
Pure tone thresholds, ABR thresholds, amplitude of wave I and wave V of ABR were analyzed in 40 tinnitus patients and 15 controls.
RESULT:
There was no significantly difference in pure tone thresholds and ABR thresholds between those tinnitus patients and controls while a reduced amplitude of wave I and normal amplitude of wave V of ABR in the tinnitus patients became evident.
CONCLUSION
Tinnitus patients with normal audiograms have hidden hearing loss at the level of primary auditory nerve and the generation of tinnitus is likely attributed to a homeostatic response of neurons in brainstem.
