1.Age-related clinical characteristics and differences of refractory nasal bleeding site

OBJECTIVE To investigate the distribution and regularities of intractable nasal bleeding site in different aged patients.METHODS The clinical data of 149 patients with intractable epistaxis were reviewed respectively.RESULTS Of the 149 patients, 110 cases were male and 39 cases were female with a age ranged from 18 to 87 years. They were divided into young group (39y or less), middle age group(40~59 y) and elderly group(60y or older), and there were 36, 52 and 61 cases in each group respectively. Hemostasis was performed with local anesthesia or general anesthesia under nasal endoscope. There were 145 cases with definite bleeding site, and electrocautery or micropacking was used for stop the bleeding. There were 4 cases without definite bleeding site. The bleeding site was on the posterior fornix of inferior meatus in 22 cases(61%), posterosuperior of middle nasal meatus in 13 cases(36.1%) and olfactory cleft of nasal septum in one case(2.7%) in young group; The bleeding site was on the posterior fornix of inferior meatus in 23 cases(44.2%), posterosuperior of middle nasal meatus in 10 cases(19.2%) and olfactory cleft of nasal septum in 17 cases(32.7%) in the middle age group; In the elderly group, the bleeding site was on the olfactory cleft of nasal septum in 37 cases(60.6%), posterosuperior of middle nasal meatus in 15 cases(24.6%) and posterior fornix of inferior meatus in 7 cases(11.5%).CONCLUSION In patients with refractory nasal bleeding, the majority responsible vessel was sphenopalatine artery or its branches in young and middle-aged patient and branches of anterior/posterior ethmoidal artery in elderly patients. Along with the increase of age, the sphenopalatine artery branch bleeding decreased, but the branches of anterior/posterior ethmoidal artery bleeding increased.
2.Survey of first permanent molars of 7 - 12-year-old students in Huairou district, Beijing in 2010

The first permanent molar caries and current status of sealant of 7 - 12-year-old students were surveyed in Huairou district of Beijing.Based on the World Health Organization and national oral health survey standards,cluster and stratified random sampling methods were used.The prevalence rate of deciduous tooth caries was 17.34% with a decayed/missing/filled teeth (dmft) index of 0.30.The caries rate of females was higher than that of males (20.60% vs.14.08% ).The incidence of mandible was higher than that of maxillary.And there was no significant difference around the jaw.The rate of fissure sealant was 51.36%.The current status of first permanent molar caries in this district was in accordance with the characteristics of general epidemiological caries disease.An overall policy of early discovery,early diagnosis and early treatment should be practiced.

Hepatitis B-related acute-on-chronic liver failure (ACLF) is a severe clinical syndrome characterized by jaundice, coagulopathy, ascites and hepatic encephalopathy and with a high mortality rate of 65% to 93%. It involves significant ethical issues when a randomized, double-blinded, placebo-controlled clinical study is conducted to such a serious disease. Therefore, a prospective cohort study design was utilized to explore a new treatment modality of applying integrated traditional Chinese and Western medicine.
5.Early levothyroxine treatment during maternal subclinical hypothyroidism improves spatial learning of offspring in rats

Objective To investigate the efficacy and optimal time of levothyroxine (L-T4) treatment in pregnant rats with subclinical hypothyroidism.Methods Female adult Wistar rats were divided into six groups (n =10 per group):control,hypothyroid (H),subclinical hypothyroid (SCH),and SCH treated with L-T4 starting from the tenth,thirteenth,and seventeenth gestational day (GD10,GD13 and GD17),to restore normal thyroid hormone levels.Spatial learning was assessed in progenies by a water maze test and fEPSPs recording.Results Progenies from the SCH and H groups demonstrated significantly longer mean latency in the water maze test and a lower amplification percentage of the fEPSPs' amplitude and slope compared with the offspring of the control group.L-T4 treatment in the GD10 and GD13 groups significantly shortened mean latency and increased the amplification percentage of the fEPSPs' amplitude and slope as compared with the progeny of rats with subclinical hypothyroidism.However,L-T4 treatment in the GD17 group showed only minimal effect on spatial learning of offspring.Conclusion Maternal subclinical hypothyroidism may impair spatial learning in the offspring; L-T4 treatment started early during pregnancy may alleviate this adverse effect.
6.Effect of acupuncture along affected meridian on the MME gene expression of migraine patients without aura of gan-yang hyperactivity syndrome.

OBJECTIVETo observe the effect of acupuncture along affected meridian on the mem- brane metallo-endopeptidase (MME) gene expression of migraine patients without aura (MO) of Gan-yang hyperactivity syndrome (GYHS).
METHODSTotally 20 MO patients of GYHS were randomly assigned to the acupoint group (acupuncture along affected meridian) and the non-acupoint group, 10 cases in each group. Needling was performed once per day for 10 consecutive days. Gene chip technology was used to obtain two sets of gene expression profiles and analyzed using Gene Ontology (GO).
RESULTSIn the acupoint group, MME gene expression decreased after needling (P = 0.0023).That gene was rich in the beta-amyloid metabolic process (P = 3.16E-05) and the peptide metabolic process (P = 0.009612). Its expression was not seen in the non-acupoint group.
CONCLUSIONThe effect of point selection along affected meridian could be achieved possibly by regulating the MME gene expression.

7.Pharmacokinetics of Qingfengteng cataplasma transdermal delivery by electroporation

Objective To study the effects on electroporation of Qingfengteng cataplasma transdermal absorption and describe the characteristics of animal pharmacokinetics of it.Methods Two-chamber diffusion cell was used and the plasma drug concentration was determined by HPLC.The application of AIC theory to analysis of the compartmentally based model of sinomenine transdermal delivery by electroporation.Results The Cmax,Ka,and AUC0→∞ of electroporation was larger than those of passive diffusion;t1/2(Ka)and tmax of electroporation were reduced compared with passive diffusion.The drug concentration-curve equation were C=2.884?(e-0.056 t-e-0.232 t)and C=2.512?(e-0.058 t-e-0.149 t)for electroporation and passive diffusion,respectively.Conclusion The change of in vivo drug concentration of Qingfengteng calaplasma transdermal absorption by electroporation could be analized in accordance with mammillary one-compartment open model.The etrectroporation technology could sharply enhance the bioavalibility compared with the passive diffusion.
8.Impaction bone grafting with morselized bone and bone paste for acetabular defects in adult Crowe Ⅲ type developmental dysplasia of the hip

BACKGROUND:Considering the risk of lack of bone in revision and the bone coverage of component,it is advisable to use bone grafting to increase bone stock in patients with adult Crowe type Ⅲ developmental dysplasia of the hip (DDH) in the first total hip arthroplasty (THA).OBJECTIVE:To investigate the effectiveness of impaction bone grafting with morselized bone and bone paste for acetabular defects of adult Crowe type Ⅲ DDH in THA.METHODS:Forty cases (45 hips) of Crowe type Ⅲ DDH were enrolled,including 8 males and 32 females,aged 31-68 years.All the components were reconstructed at the anatomic acetabulum combined with deepening acetabulum and putting the centre of rotation medially and distally during THA procedures.The patients were all followed up and evaluated with Harris score,limp length,displacement of the centre of rotation,cup coverage,and postoperative complications.RESULTS AND CONCLUSION:The average follow-up was 32.7 months.Acetabular rotation centers of all the cases were recovered (near) to normal.The incisions healed by first intention and there was no complication such as infection,dislocation,prosthesis loosening.The acetabular cup prosthesis did not displace and was covered well by bone at the last follow-up,the grafted bone particles got radiological osseointegration and the bone sclerosis zone disappeared.No radiolucent lines and screw fracture were detected.The Harris scores of affected hips at the last follow-up were significantly higher than those before surgery(P=0).The average leg length was increased (2.31 ±0.18) cm.The mean cup coverage was 78.1%.The postoperative horizontal and vertical distance of the hip center were shorter than those before surgery (P=0).Reconstructing the acetabulum with autogenous morselized bone graft impaction can effectively restore the acetabular coverage,maintain the stability of acetabular cup and provide better relative bone stock in THA for Crowe type Ⅲ DDH in adult,and moreover,the short-term effect is satisfactory.
9.Curative effect of three-dimensional porous core decompression combined with autologous bone marrow transplantation on phase Ⅰ, Ⅱ avascular necrosis of the femoral head

BACKGROUND: Hip-preserving treatment is advocated in the treatment of early avascular necrosis of the femoral head.OBJECTIVE: To compare the clinical efficacy of simple core decompression and three-dimensional porous core decompression combined with autologous bone marrow transplantation on early avascular necrosis of the femoral head.METHODS: Thirty patients (39 hips) with early avascular necrosis of the femoral head at phase I-II according to the staging criteria of the Association Research Circulation Osseuse (ARCO) who had been admitted to the First Affiliated Hospital of Luzhou Medical University between March 2011 and May 2016 were surgically treated and followed up.Seventeen patients (22 hips, trial group) were treated with three-dimensional porous core decompression combined with autologous bone marrow transplantation, including 10 cases of ARCO stage I and 12 cases of ARCO stage II; the other 13 patients (17 hips, control group) were treated with simple core decompression, including 7 cases of ARCO stage I and 10 cases of ARCO stage II. Harris scores, X-ray and magnetic resonance imaging were used to evaluate the surgical efficacy at 1, 3, 6, 9, 12, 18, 24, 30, 36 months after the operation.RESULTS AND CONCLUSION: The incision healed primarily in all patients, and no infection occurred. The follow-up lasted for 1-3 years (average 2.5 years). At the last follow-up, the Harris scores in both trial group and control group were significantly improved compared with the baseline (P < 0.05); moreover, the Harris scores of ARCO stage I and II patients were significantly higher in the trial group than the control group (P < 0.05). Collapse of the femoral head was observed in one hip (stage I) and one hip (stage II) of the trial group, and in three hips (stage I) and five hips (stage II) of the control group, and then hip arthroplasty was performed. The number of the patients developing collapse of the femoral head in the trial group was significantly less than that in the control group (P < 0.05). Compared with simple core decompression, three-dimensional porous core decompression combined with autologous bone marrow transplantation is more adept to improve patient's function and delay disease progression in the treatment of ARCO stage I and II early avascular necrosis of the femoral head.
10.Clinical and pathological analysis of 2 456 cases of salivary gland tumor

Objective:
To investigate the clinical characteristics of salivary gland tumors and their pathological types.
Methods:
Data from 2 456 patients with salivary gland tumors diagnosed between January 1973 and December 2018 at Sun Yat-sen Memorial Hospital of Sun Yat-sen University were collected, and their gender, age and tumor pathological type, location, and benign and malignant composition ratios were retrospectively analyzed.
Results:
Over the 46-year study period, 2 456 patients with salivary gland tumors were treated; 41.9% were female, and 58.1% were male. The peak incidence was found among the 40 to 60 years of age group, in which 593 (24.1%) patients had malignant tumors and 1 863 (75.9%) had benign tumors. The ratio of benign and malignant tumors was 3.1∶1. The top two most common benign tumors were pleomorphic adenoma (58.7%) and Warthin tumors (33.6%). The top two most common malignant tumors were mucoepidermoid carcinoma (27.7%) and adenoid cystic carcinoma (26.1%). The most common sites of benign pleomorphic adenomas were the parotid glands, palate, and submandibular glands. Mucinous epidermoid carcinomas in malignant tumors were common in the parotid glands and small salivary glands. The incidence of salivary gland tumors in this group has increased each year, and this group accounted for 53.3% of the total cases over the past 10 years.
Conclusion
The number of patients with salivary gland tumors is increasing each year. The total incidence of salivary gland tumors is higher in men than in women. Large salivary gland tumors are mainly benign tumors, and small salivary gland tumors are more common. Polymorphic adenomas, Warthin tumors, and mucoepidermoid carcinomas are the most common tumor types; patients 40~60 years old are most likely to have benign salivary glands and have a high incidence of malignant tumors.
