5.Association of Macrovascular Disease with Serum Lipoprotein(a) Levels in the Elderly .

BACKGROUND : Lipoprotein(a)[Lp(a)] may be an independent risk factor for macrovascular disease in middle aged populations, but in elderly populations, Lp(a) levels to predict macrovascular risk is controversial. The purpose of this study is to evaluate the association of macrovascular disease with Lp(a) levels and lipid change in the elderly. METHODS : We have examined Lp(a) levels, lipid change and clinical, biochemical profile in 114 patients with macrovascular disease and 55 control subjects. Macrovascular disease was defined as ishemic heart disease and/or peripheral vascular disease, peripheral vascular disease was defined as arteriosclerosis obliterans, diabetic foot and/or cerebrovascular disease. RESULTS : Hypertension, diabetes were more prevalent and median Lp(a) levels, fasting blood glucose and HDL-cholesterol were significant higher in patients with macrovascular disease than control subjects, but there were no significant differences in age, body mass index, total cholesterol and triglycerides levels. Lp(a) levels were positively weak correlated with fibrinogen levels, but not correlated with age, body mass index, total cholesterol, HDL-cholesterol, LDL-cholesterol and triglycerides levels. In logistic regression analysis, there was no independent risk factor for ischemic heart disease, whereas diabetes, hypertension and low HDL-cholesterol levels were independent risk factors for peripheral vascular disease. CONCLUSION : Serum Lp(a) levels was not independent risk factor for macrovascular disease, but diabetes, hypertension and low HDL-cholesterol levels were independent risk factors for peripheral vascular disease. Therefore, the relationship between Lp(a) and lipid change and macrovascular disease in the elderly were different from middle aged subjects.

8.Morphological Changes of Somatostation Immunoreactive Neurons in Senile Dementia of Alzheimer Type.

Five brains from established cases of Senile Dementia of Alzheimer Type (SDAT) and six branis from cases of no clinical or neuropathological evidences of SDAT were studied immunocytochemically using anti-somatostatin (SST) antibody. Fifty micrometer sections of immersion fixed brains (autopsy delay: 10-16 hours) were cut and then stored in 0.1 M tris-HCI buffer (pH7.4) until the immunocytochemical study. Lmmunostatinings were performed according to Sternberger's peroxidase-antiperoxidase (PAP) technique. The primary antiserum was rabbit antiserum to synthetic somatostatin (Cambridge Research Biochemicals). Diluted to 1: 1.000. The secondary antiserum was goat anti-rabbit IgG (Sternbeger-Meyer) in dilutions of 1: 400 to 500. Controls followed complete staining protocol. But without incubation in the specific antibody. SST-immunoreactive (IR) neurons in aged normal brains were distributed in all layers of cortices. These cells were multipolar, bitufted, or wramidal in shape. The changes of SST-IR neurons were profound in frontal, parietal, and temporal cortices of SDAT brains, but were absent in the Cal of hippocampus. Neuritic plaque-like structure formation was the most common changes seen in the cortices, especially in the temporal cortex. The morphology of altered SST-IR neuronal elements were similar to that of the neuritic plaques demonstrated by the Bielshowsky's silver impregnation method. Apparently intact SST-IR neurons were also found in SDAT brains, thus giving rise to the speculation that these neurons may have unusual survival and reorganization potential. Unexpectedly, there were no degenerative changes of SST-IR neurons in CA! region of SDAT hippocampus. These results strongly suggest that among SST-IR neurons only neocortical short association neurons are affected in the neuronal degenerative changes of SDAT.

9.A Case of Isolated ACTH Deficiency

solated ACTH deficiency is a very uncommon cause of hypoadrenocorticism, with less than 200 cases reported in the literatures. The clinical presentation can be similar to that of primary adrenal insufficiency, but there is a greater tendency for hypoglycemia and absence of hyperpigmentation. The diagnosis is established by demonstrating hypocortisolism with undetectable serum levels of ACTH, normal adrenal responsiveness to prolonged ACTH infusion, and an absent ACTH response to insulin-induced hypoglycemia. Other endocrine function is normal.We experienced a case of isolated ACTH deficiency in 34 years old female who was admitted due to fever and drowsy mentality.So we present this case with a review of literatures.
