1.Intestinal perforation due to infection of Sparganum mansoni.

In May 1974, authors encountered a 37 year old Korean male who was suffering from very serious condition of acute abdomen. On exploratory laparotomy, a ruptured granulomatous mass in the proximal portion of the ileum showing extensive inflammatory and gangrenous changes was found and about 4 ft. long of the bowel was resected. From the honey-combed fibrous capsules in the mass, four plerocercoid larvae, spargana, measuring about 3 to 7 cm in lengths were extirpated. The patient had a past history of having eaten the raw flesh of a snake as a tonic about 7 months prior to admission. Four cases of intra-abdominal sparganosis reported previously in Korea and the present case were discussed briefly. Snakes and frogs in Korea are very important second intermediate hosts for the 1arva, Sparganum stage. It is most preferable that the habitual ingestion of the raw fleshes of them should be avoided in this country.

2.Postural Response of Intraocular Pressure in Primary Open-angle Glaucoma Following Trabeculectomy.

Forty-six eyes with primary open-angle glaucoma(POAG) and 34 eyes which had previously undergone trabeculectomy for POAG were studied and the postural response of the intraocular pressure(IOP) compared with that of 80 normal eyes, In all groups the increment in IOP occurred when the subject changed from the sitting to the lying position. The pressure increment when changed from the sitting to lying position, averaged 2.4 mmHg in normal eyes, 3.6 mmHg in the group who were treated medically and 3.8 mmHg in the group who had undergone trabeculectomy. There was greater postural response in primary open-angle glaucoma patient compared to normal subject. And also there was no statistically significant difference of postural response in intraocular pressure between the group who were treated medically and the group who had undergone trabeculectomy. It is suggested that abnormal postural response of intraocular pressure in POAG appears to be retained and to be unaffected in those eyes which had undergone trabeculectomy.

3.MRI findings of acute disseminated encephalomylitis.

Acute disseminated encephalomyelitis (ADEM) is a demyelinating disease of probable autoimmune etiology. The MR images of patients with clinically suspected ADEM were retrospectively reviewed. The clinical symptoms occurred 5 days to 1 month after viral upper respiratory infection (4) and Coxsakie viral infection(1). The symptoms had begun with fever(3), headache(3), sore throat(1), and drowsy mental state (1), which progressed with monophasic course to altered mental change(2), extremity weakness(2), seizure(1) and/or cerebellary symptom(I) MRI findings of ADEM showed patchy(4), nonhemorrhagic(5), asymmetric(5) high signal intensity lesions on T2-weighted images. The number of the lesions was mostly multiple(4). The lesions mainly involved the brain stem(3) and subcortical white matter(3). Follow-up MR images of 13 days to 20 days after high dose steroid therapy showed marked improvement in two of three, which well correlated with clinical manifestations. MR findin of multiple, patchy, nonhemorrhagic and asymmetric lesions in subcortical white matter and brain stem on T2-weighted images seem to be characteristic features of ADEM, but nonspecific. Therefore, clinical correlation is required in evaluating ADEM.

4.The Changes in Myosin Heavy Chain Isoforms After Extraocular Muscle Recession in Rabbits.

PURPOSE: To study the changes in the amount and isoform pattern of the myosin heavy chain (MyHC) in rabbit extraocular muscle (EOM) fibers after recession. METHODS: Sixteen New Zealand white rabbits were used. Recession surgery was performed on the right superior rectus (SR) muscle by 3 mm in eight rabbits, and performed by 8 mm in other eight rabbits. The left SR muscles were left intact as the control groups. The SR muscles in both eyes were harvested from two rabbits from each recession group at 3 days and 1, 2, and 4 weeks after surgery. The changes in MyHC amount and isoform pattern were analyzed by gel electrophoresis. RESULTS: Total MyHC content decreased from 1 week after surgery in the 3-mm recessed group and from 3 days in the 8-mm group. The type IIb MyHC (MyHCIIb) plus EOM-specific MyHC (MyHCeom) showed similar proportional changes to the total MyHC at the different time points after surgery. CONCLUSIONS: The fast MyHCIIb plus the superfast MyHCeom decreased after EOM recession, and these results appear to be related to the changes in the global layer rather than in the orbital one. This suggests that the global layer might be the fast and the superfast twitch portions of rabbit EOM, which perform the fast saccades in ocular movements.

6.The effects of intraoperative mitomycin-C or 5-fluorouracil on glaucoma filtering surgery.

We compared the success rate of filtering surgery of a single 5-minute intraoperative application of mitomycin-C (MMC) or 5-fluorouracil (5-FU). Animal experiment and clinical study were done. In animal study, thirty pigmented rabbits (60 eyes) weighing 2.0 to 2.5 Kg were enrolled. We divided into 4 groups, such as BSS, 5-FU 50 mg/ml, MMC 0.2 mg/ml and MMC 0.4 mg/ml subconjunctival soaked group. Each group consisted of 15 eyes. In each group, 10 eyes was for examination of bleb survival and complications, 3 eyes for light microscopic examination and 2 eyes for electron microscopic examination. Bleb was survived 6.1 days (3 to 13 days) for BSS treated group, 16.3 days (9 to 23 days) for 5-FU 50mg/ml treated group, 32.7 days (17 to 55 days) in MMC 0.2 mg/ml treated group, and 64.4 days (49 to 84 days) in MMC 0.4 mg/ml treated group. Duration of bleb survival was significantly prolonged in 5-FU, MMC 0.2 mg/ml and MMC 0.4 mg/ml group respectively. In clinical study, fifty-five eyes of 40 patients were enrolled. 29 eyes of 21 patients were treated with MMC 0.2 mg/ml and 26 eyes of 19 patients were treated with 5-FU 50 mg/ml. The success rate was 89.7% in MMC 0.2 mg/ml treated group and 84.6% in 5-FU 50 mg/ml treated group at postoperative 3 months, and postoperative 6 months 89.7% in MMC 0.2 mg/ml treated group and 76.9% in 5-FU 50 mg/ml treated group. There was no statistical significance at postoperative 3 months (P > 0.05), but statistical significance at postoperative 6 months (P < 0.05). There was no serious complications using a MMC 0.2 mg/ml or 5-FU 50mg/ml.

8.Innervated Myotendinous Cylinders Alterations in Human Extraocular Muscles in Patients With Strabismus.

PURPOSE: To analyze innervated myotendinous cylinders (IMCs) in the extraocular muscles (EOMs) of normal subjects and strabismic patients. METHODS: The rectus muscles of 37 subjects were analyzed. Distal myotendinous specimens were obtained from 3 normal subjects, 20 patients with acquired strabismus, 11 with infantile strabismus, and from 3 with congenital nystagmus, and were studied by using light microscopy. Some specimens (6 rectus muscles) were also examined by transmission electron microscopy. RESULTS: IMCs were found in the distal myotendinous regions of EOMs. The IMCs of patients with acquired strabismus showed no significant morphological alterations. However, significant IMCs alterations were observed at the distal myotendinous junction of patients with congenital strabismus and congenital nystagmus. CONCLUSIONS: This study supports the notion that IMCs in human EOMs function mainly as proprioceptors, along with effector properties, and a disturbance of ocular proprioceptors plays an important role in the pathogenesis of oculomotor disorder. We suggest that a proprioceptive feedback system should be stimulated and calibrated early in life for the development of binocular vision.

9.Hand, foot and mouth disease associated with the aseptic meningitis in Seoul, 1990.

We performed clinical and virological studies on 79 children with hand, foot and mouth disease (HFMD) who had been admitted or visited to the department of Pediatrics, Asan Medical Center from February to August, 1990. The results were as follows; 1) There were total of 79 cases with hand, foot and mouth disease during this period. The aseptic meningitis was combined in 18 cases (22.8%). 2) The male-to-female ratio of HFMD was 2.03 to 1, but that of aseptic meningitis was 8 to 1. 3) Hand, foot and mouth disease occurred more frequently in under the age of 5 years, and the aseptic meningitis in under the age of 1 year. 4) Hand, foot and mouth disease and the aseptic meningitis occurred mostly in May and June. 5) In addition to the skin manifestations characteristic in hand, foot and mouth disease, some of the patients with aseptic meningitis had the following symptoms: change of consiousness, seizure, monoparesis in the upper extremity, paraparesis in the lower extremity. 6) The causative agent of hand, foot and mouth disease associated with the aseptic meningitis in Seoul in 1990 seemed to be enterovirus 71 according to the serologic study.
