Lumbar hernias are rare posterolateral abdominal wall defects that may be congenital or acquired. There are two types of lumbar hernia, the superior lumbar hernia through Grynfeltt triangle, and the inferior lumbar hernia through Petit triangle. Many techniques have been described for the surgical repair of lumbar hernias including primary repair, local tissue flaps, and conventional mesh repair. But these open techniques require a large skin incision. We report a case of superior lumbar hernia, which was successfully repaired using a laparoscopic approach.

3.Human Cysticercosis in Subcutaneous Soft Tissues

80 cases of Cysticercosis were studied and analysed, which were diagnoses by biopsy at Seoul National University in the past three years from January-1978 to March-1980. The results were summerized as follows. 1. The relative incidence ratio of cysticercosis among 18,941 surgical specimens was 0.42%. 2. The age group between 11 and 50 years old occupied 87.5%. 3. Of the 80 cases, there were 38 males and 42 female patients, without significant sex predominence. 4. The common sites of cysticercosis were upper extremities (35.0%) and anterior surface of the trunk (37.5%) 5. Epidemiologically, there was marked predominence of rural area over urban area. 6. There were 8 cases (10%) of definite cerebral cysticercosis and 4 cases (5%) of suspicious cerebral cysticercosis. 7. In the cases of nodular masses of extremities, the parasitic infection, especiaily cysticercosis celluiosae, must be included in differential diagnosis distinguished from other soft tissue tumors.

4.The Absorption of Intraocular Gas and Its Relationship with IOP.

We performed the intravitreal injection of air and pure perfluoropropane(C3F8) gas in the pigmented rabbits and observed the changes of gas levels by the ultrasonography. We also observed the changes of the intra-ocular gas levels by the ultrasonography and measured the intraocular pressures(IOPs) in 9 patients who had undergone vitrectomy and fluid-gas exchange with 20% sulfur hexafluoride(SF6) and 14% perfluoropropane(C3F8) gases. In the pigmented rabbits, the air was not expanded and completely absorbed within 2 days, and the C3F8 gas was expanded maximally at 3 to 7 days and completely absorbed after 3 weeks. Among nine human eyes treated with vitrectomy and fluid-gas exchange, the intraocular pressures were elevated above 25mmHg at 1 day after operation in 2 eyes, which were controlled with medical therapy. The lOPs were significantly correlated with the amount of the intraocular gases(r=0.3476, p<0.05). The assessment of intraocular gas level by ultrasonography seems to be easier and more objective method than others previously reported.

6.Development of a Semi-Quantitative Food Frequency Questionnaire for Pre-school Children in Korea.

The purpose of the study was to develop and evaluate the reliability of a semi-quantitative food frequency questionnaire (SFFQ) intended to measure mainly nutrient intake (energy, protein, fat, calcium and iron) related to growth in pre-schoolers in Korea. Based on foods with up to 90 cumulative percent contribution of the 5 nutrients, we developed a SFFQ for 86 foods. In order to evaluate the reliability of both nutrient and food intakes, caregivers of a sample of 101 children aged 4 to 6 years completed the SFFQ repeatedly at 3 month intervals. There were small differences in the nutrient intakes assessed by the SFFQ ranging from 0.55% to 9.91%. There were no significant differences in nutrient values calculated from the repeat questionnaires, except in the case of niacin and vitamin C. The Pearson correlation coefficients of most of the nutrients ranged form 0.54 to 0.75 (mean = 0.66). When energy intake was adjusted, there was approximately a 0.2 decrease in the correlation coefficient of most nutrients, due to the high correlation of energy intake with other nutrients (r=0.7-0.9). The amount of food intake differed by 0.1 to 66.4% (mean = 17.5%), depending on the food item. Out of 86 foods, 74 foods (86%) showed less than a 30% difference in intake and 30 foods (35%) showed less than a 10% difference. Only 6 (7%) out of the 86 foods presented statistically significant differences in intake. The Spearman correlation coefficients of most food intakes assessed repeatedly by the SFFQ ranged from 0.4 to 0.7. Reproducibility of the nutrient and food intakes found in this study was better or similar to those found in cases of Korean adults. Therefore, the SFFQ developed in the present study can be a useful tool to assess the dietary intake of pre-schoolers in Korea.

7.Effects of a Protein Synthesis Inhibitor on Hippocampal Neuronal Damage of Rats in the Ventricular Fibrillation Cardiac Arrest Model.

BACKGROUND: The goal of successful resuscitation is not only to stop the process of ischemia as soon as possible but also to overcome the secondary injury process after resuscitation, which involves a complex interplay of mechanisms. Brain damage accompanying cardiac arrest and resuscitation is frequent and devastating. Cells die by one of two mechanisms: necrosis or delayed neuronal death. Delayed neuronal death may require protein synthesis. Neurons in the CA1 subfield of the hippocampus are selectively vulnerable to death after injury by ischemia and reperfusion. Death of these neurons occurs after an interval of 1 or 2 days. We assessed the effects of a protein synthesis inhibitor, cycloheximide(CHX), on hippocampal neuronal death of rats by using the ventricular fibrillation cardiac arrest(VFCA) model. METHODS: The effect of CHX(3mg/kg, s.c.) on hippocampal neuronal death was studied in two groups of 18 rats each, one group being subjected to a 2-min VFCA and the other to a 3-min VFCA. Each group was divided into three subgroups: control(group I,II) without subcutaneous injection of CHX, 'exp-12' of group I/II treated with CHX 12 hours after return of spontaneous circulation (ROSC), and 'exp-24' of group I/II treated with CHX 24 hours after ROSC. The coronal sections of the hippocampus levels were stained with hematoxylin-eosin after 72 hours of survival. The histologic damage score(HDS) was used to assign a score to the total number of damaged neurons counted in each of the hippocampal CA1 subfields. RESULTS: 1. There were not significan differences in heart rates, blood pressures, blood sugar, and blood gas in group I & II during the pre-arrest steady state or at 5 min and 30 min after ROSC. 2. In group I & II, the HDS, were significantly reduced in rats(I exp-12, 1.1+/-0.6; I exp-24, 1.3+/-0.5; II exp-12, 1.4+/-0.7; and II exp-24, 1.8+/-0.8) treated with CHX 12 hours or 24 hours after ROSC than control rats(I, 2.5+/-0.9, II, 2.9+/-0.8)(p<0.05). CONCLUSION: These results suggest that delayed hippocampal neuronal death from ischemic insult after ventricular fibrillation cardiac arrest followed by resuscitation can be prevented by a protein synthesis inhibitor, CHX. Further experimental studies of the action mechanism of protein synthesis inhibitors to delayed neuronal death and clinical applications are required.

8.A Case of Chronic Pneumonitis of Infancy in Pediatric Interstitial Lung Disease.

The interstitial lung diseases(ILD) comprises a large, heterogeneous group of disorders characterized by derangement of alveolar walls and alveolar capillary unit causing restrictive pulmonary dysfunction and disordered gas exchange. It is rare, especially in children, and the opinions on investigation and treatments are controversial. The classification of adult ILD is applied to that of children. But considering occurrence during the developing state of lung and immune systems, we should be more cautious because the clinical manifestations and prognosis will be different from those of adults. In 1995, Katzenstein named a unique type of ILD, chronic pneumonitis of infancy (CPI) for the first time. CPI is characterized by marked alveolar septal thickening, striking type II pneumocyte hyperplasia, and eosinophilic alveolar exudate containing numerous macrophages. We report a 14 month-old girl, who had continuing respiratory difficulty and cyanosis after upper respiratory infection, could not sustain oxygen saturation and was finally diagnosed as CPI by histologic examination.

9.Interpersonal Relations, Hope, Professional Self-concept and Turnover Intention according to Adult Attachment Styles in Early Stage Nurses.

PURPOSE: This study was done to examine interpersonal relations, hope, professional self-concept and turnover intention according to adult attachment styles in early stage nurses. METHODS: A survey using a structured questionnaire was conducted with 245 nurses who work in one of six general hospitals. Additionally, a qualitative method was used to obtain data on experiences in interpersonal relations. Data were analyzed using SPSS/WINdows 14.0 and qualitative data from open question in the survey questionnaire were analyzed using content analysis. RESULTS: There was a significant difference in interpersonal relations, hope, and professional self-concept according to adult attachment styles. In the interpersonal relation of early stage nurses, 8 themes were identified, of which the most frequent were: a comfortable relationship with my co-workers (43%) and stress from the relationship (34%). CONCLUSION: Findings from this study should be useful in the development of programs to promote adult attachment and professional self-concept for early stage nurses.

10.Within-and between-person Variation in Nutrient Intakes by the Korean Elderly.

This study examined within- and between-person variation in nutrient intakes in order to estimate the degree of precision in dietary assessment among 37 males and 46 females whose mean age was 70.4 years old. To collect dietary data, each subject was interviewed 5 or 6 times using a 24-hour recall method during a 3 month period. Within-person variation ranged from 23.5% to 101.2%. Lower values of within-person variation were observed in energy, carbohydrated, protein and phosphorous while higher value was observed in vitamin A. Between person variations of nutrient intakes ranged 12.6-23.5% in most nutrients. With 1 day dietary data, observed nutrient intakes were estimated to within 6-25% of the group's usual9true) intakes and 52-198% of the individuals' usual intakes. The values of these maximum percentage deviations became smaller when the number of dietary recalls increased. The results of this study suggest that the Korean elderly subjects appear to have a slightly less diverse diet compared to young Korean women. Within persons, intakes of nutrients largely from animal sources were more variable for the Korean elderly than for their counterparts in Western countries. This study also implies that commonly used 1 day dietary study may be appropriate for assessing group means of nutrient intakes, but clearly not appropriate for assessing individual's nutrient intakes.
