3.Efficacy and Safety of Nifedipine Gastrointestinal Therapeutic System(Adalat OROS) in Patients with Mild to Moderate Essential Hypertension.

BACKGROUND: This study was designed to evaluate the antihypertensive efficacy and safety of nifedipine gastrointestinal system push-pull osmotic pump formulation in patients with mild to moderate essential hypertension. METHODS: After 2 weeks placebo run-in period, nifedipine 60 mg was administered once daily for 8 weeks in thirty-two patients with mild to moderate essential hypertension. RESULTS: At the end point of treatment, sitting blood pressure decreased as much as 33/16mmHg in average and rewarded 88% of efficacy and 69% of normalization. The ambulatoy blood pressure monitoring in 10 patients also revealed 11% decrease in 24 hr mean diastolic blood pressure and 32% decrease in % elevated blood pressure. The most frequent side reactions were constipation in 7 patients and polyuria in 6 patients which were tolerated during entire trial period. There were no significant changes in biochemical parameters and hematologic data, thus making the rate of safety 91% and overall rating of usefulness was 84%. CONCLUSION: Nifedipine GITS 60 mg once daily regimen is well tolerated and effective in the treatment of mild to moderate essential hypertension.

4.Identification of the CDR3 Gene Sequence on the beta Chain of the T Cell Receptor in T Cell Leukemia Cell Line.

In order to develop a method for the detection of minimal residual leukemic disease (MRD) in T cell acute lymphocytic leukemia (T cell ALL), T cell leukemia cell line was used to detect clonal TCR p chain mRNA and to synthesize CDR3 specific oligonucleotide probe. For the Jurkat cell line clonal TCR p chain cDNA was amplified by using RT-PCR with oligonucleotide primer, Vp universal primer. As the result of RT-PCR an approximate 300 bp fragment of the TCR chain was obtained, and the partial identification of the TCR p chain gene and the amino acid sequence of the fragment were done by gene cloning and sequencing. The gene sequence of TCR p obtained was identified as Vp8-Jp1.2-Cp2. Diversity gene segment could not be found. Within the p chain, the CDR3 region was identified as 12 amino acids (SFSTCSANYGYT). It is kown that TCR is expressed in about 40% of the all T cell ALL. However it is not kown what percentage of the TCR p chain mRNA expression translates into the actual TCR molecule. It is not certain how many patients with MRD can be detected by this method used in this study, but this technique might be useful to detect MRD in at least 40% of the patients with T-cell ALL.

5.Pathologic Diagnosis of Intestinal Tuberculosis in Endoscopic Biopsied Material.

The clinicopathologic features and the comparative analysis of diagnostic methods in 42 patients having intestinal tuberculosis were studied. In all the cases, clinical and colonoscopic diagnosis was confirmed by histological examination. Abdominal pain was the most common symptom (54%). Twenty nine patients had active pulmonary tuberculosis which was confirmed by a chest X-ray, or an AFB smear and a culture of sputum. A transverse ulcer with surrounding hypertrophic mucosa and multiple erosion was the usual colonoscopic findings. The granulomas were usually located in the just upper and lower portion of muscularis mucosa. The direct smear and culture of the fresh biopsy material showed AFB in 11 (32.4%) and 12 cases (36.4%) respectively. Ziehl-Neelsen staining in serially sectioned slides from formalin-fixed, paraffin- embedded tissue revealed AFB in 15 cases (35.7%). An immunohistochemical stain for Mycobacterium bovis was done in all cases and 13 cases were positive (31%). A polymerase chain reaction (PCR) was done and showed positivity in 4 out of 20 cases of fresh biospy material and 12 out of 40 cases in paraffin embedded tissue. For the conclusive diagnosis of intestinal tuberculosis, a Ziehl-Neelsen stain is the most sensitive, fast, and cost-effective method. The diagnostic accuracy will be increased when other diagnostic methods such as tissue culture and PCR are coupled with this simple staining method.

8.Two Cases of Acute Digitalis Poisoning by Accident and Suicidal Attempt.

Two cases of acute digitalis poisoning were presented. The first patient was 20-year-old male with asymptomatic ventricular septal defect (maladie de Roger) who attempted suicide by ingestion of degitalis leaf about 6.0 g (60 tablets). He developed severe epigastric pain and vomiting one hour after ingestion. Syncopal attack was developed once for a few minutes and recovered. Electrocardiogram revealed 2:1 A-V block. By conservative management he was recovered. The second patient was 2-year-old, healthy baby who ingested digoxin about 2.5 mg(10 tablets) by accident. He developed severe vomiting and electrocardiogram revealed 1degrees, 2degreesand 3degrees A-V block in series. He was recovered by conservative management without sequele.

10.A Study on Fatigue Management of Aviation Maintenance Mechanics-Focusing on Shift Workers

In the case of aviation maintenance work, several kinds of shift works are performed during day and night, and a lot of works are performed on the ramp due to the characteristics of the aircraft flight schedule. Maintenance workers are often exposed to the sun or in cold temperatures for aircraft maintenance works. The fatigue risk of the maintenance workers will be weighted. In particular, the work at night shift under normal biological rhythms can make maintenance workers feel sleepy during daytime work. It can also affect the safety of aircraft and individual workers.Accordingly, this study will consider fatigue-related previous researches in the field of aviation maintenance, review the effects of shift work, and how fatigue affects aviation mechanics during the dayight shifts. Considering these factors, we tried to present a plan that can minimize the fatigue of aviation workers through this study.
