3.Proposal of Photographic Standards in Plastic Surgery.

Photographic standards in plastic surgery is an important problem that has been stressed in the discipline over the past several years. Clinical photographs should always be taken with the same camera lens, lens setting, lighting, film, and patient position to ensure reproducibility and to enable valid pre- and postoperative comparisons. However, no photographic standards in plastic surgery were established in Korean Society of Plastic and Reconstructive Surgeons. We analyzed that inappropriate photographs in the total publishing photographs were counted on the Journals of the Korean Society of Plastic and Reconstructive Surgeons from 1995 to 2000. The causes of inappropriate photographs were categorized as six groups. We found three characteristics from photograph published on this journals. First, the ratio of inappropriate photographs were not decreased for recent six years - 30.6% in 1995 and 29,7% in 2000. The mean proportion of poorly qualifying photographs in total photos was 29.3% during six years. Second, the subject malposition due to incorrect anatomic landmark and technical errors were 34.8%., 20.0%, respectively. The markedly different size of pre-and postoperative subject, inappropriate orientation of camera (horizontally or vertically), inadequate patient preparation, others were 15.3%, 14.9%, 7.9%, 7.1% in order. Third, the most patient identity in this publication was recognizable regardless of patient consent(1673/1761, 95.0%). In our conclusion, first, the ease photographic standards for plastic surgeon in Korea was guided in our study. Second, we thought that the ease of using digital photography may be able to correct the inappropriate photographs due to technical errors of amateur. Third, we proposed that new format of permission for patient photographic authorization and release will be needed because of prevention against sue for invasion of privacy or release of identity in Korea.

5.Treatment of the tibia shaft fractures with ender nails.

Fractures of the tibial shaft are the most common among the long bone fractures, and have much difficulty in treatment due to their numerous complications. Thirty patients with fractures of the tibial shaft were treated with Ender nails under the image intensifier at the Department of Orthopedic Surgery, Yeungnam University Hospital from December 1986 to November 1991. The following results were observed. 1. The average age was 37.3 years and the number of male was three times of the female. The most common cause was traffic accident. 2. Twenty cases out of thirty one were closed fracture and the remaining 11 were open. The comminuted and segmental fractures were 18 (57.7%) in number and the most common fracture site was the middle one third (53%). 3. Average interval from injury to operation was 7.6 days and 19 cases showed associated injuries. 4. The mean duration of the bone union was 18.9 weeks and 2 cases showed the delayed union. 5. Twelve complications were noted such as shortening, varus deformity, delayed union, ankle motion limitation, nail irritation, and soft tissue infection.

6.Experimental Scoliosis Induced by Electrical Stimulation

Animal experiment was carried out to measure the physiologic changes of thoracic or thoracolumbar spine that occurred as a result of electrical stimulation, and to evaluate the usefulness of electrical stimulation as a possible treatment for scoliosis. Unilateral electrical stimulations were applied to the back muscle of immature rabbits.They were subgrouped into medial, intermediate, and lateral muscle stimulated groups in order to observe the effects of various electrode placements. The radiograms were taken every other week to observe the changes of the spinal curvatures for the period of 12 weeks. The histological studies of the muscles, which were obtained from the stimulated(right) and the nonstimulated(left) sides at the same level, also were carried out to determine the response of the muscles to the electrical stimulation. The results were as follows. l. A significant scoliotic curvature was observed two weeks after electrical stimulation, and those curvatures did not change more for the next six weeks in spite of prolonged stimulation. 2. Scoliotic curvature induced by electrical stimulation for eight weeks remained unchanged for another four weeks after stopping stimulation. 3. There were no statistical differences in the induced scoliotic curvature among the three groups stimulated at different sites of the medial, intermediate, and lateral regions respectively. 4. A prolonged unilateral electrical stimulation could induce the scoliosis, and may be used as a method for treatment of scoliosis.

8.Anesthetic Experience of the Patient with Nemaline Myopathy: A case report.

Nemaline rod myopathy is an autosomal dominant disease characterized by nonprogressive symmetric skeletal muscle weakness affecting principally proximal skeletal muscles. Anesthesia of the patient with this disease is known to present some problems:difficult intubation due to facial dysmorphism, delayed onset of succinilcholine, reduced vital capacity on pulmonary function test and skeletal deformity. We anesthetized 19 year old male patient with nemaline myopathy for the operation of cryptochidism. Because of the possibility of difficult intubation, we produced epidural anesthesia with 20 G Tuohy needle at L5-S1 intervertebral space successfully. There were no problems in the course of operation and recovery except transient airway obstruction due to sedative.

9.A Case of Gastric Teratoma Combined with Congenital Syphilis.

Gastric Teratomas are exetremely rare in infancy and childhood. About 50 cases of gastric teratomas are reported in the world's literature. Most of them were male. Few were dignosed prior to surgery. The mos thelpful diagnostic aid was of teeth or bone in radiologic examinations. Treatment is surgical excision and all of them were benign. Recently, we experienced a case of gastric teratoma combined with congental syphilis which was confirmed by explo-lapa and biopsy in 2month old male infant, who had large intraabdominal mass, vomiting, respiratory distress and skin lesions. The literature on this subject was reviewed briefly.

Coronary artery fistula draining into lung parenchymal vasculature has not been reported. Herein, we describe a case of an 81-year-old woman who presented with a localized pulmonary edema on right upper lobe associated with coronary fistula emptied into vasculature in right upper lobe. She underwent transcatheter closure of the fistula with an Amplatzer Vascular Plug 4, which resulted in complete occlusion and improved localized pulmonary edema.
