1.Efficacy of Cefadroxil Monohydrate (Duricef.

Effect of Cefadroxil monohydrate was evaluated on 37 patients with urinary tract infection in our department during past 6 months periods. Cefadroxil was given Orally at a dose of 1.0 gm - 3.0 gm bid per day for 3 - 15 days and following results were obtained. 1. Excellent or good results was observed in 31 out of 34 patients in whom complete follow up study were done, giving success rate of 91.2%. 2. Therapeutic effects were obtained in 10 out of 11 patients with urethritis and all patients with cystitis and pyelonephritis, but not in 1 with urethritis, 1 with prostatitis and 1 out of 9 urinary tract infections with predisposing factors. 3. In this clinical settings, Cefadroxil was effective against E. coli infection in 10 patients, mixed infection in 2 and Staphylococcus aureus infection in 5. However it was ineffective against Serratia infection in 1 patient and Staphylococcus infection in 1. 4. As to the side effects, only mild dizziness was observed in 2 patient.

3.Intracellular acidosis decreases the outward Na(+)-Ca2+ exchange current in guinea pig ventricular myocytes.

The Na(+)-Ca2+ exchange transport operating in outward mode has been suggested to cause Ca2+ entry during reperfusion or reoxygenation, exchanging extracellular Ca2+ for intracellular Na+ that has accumulated during ischemia or cardioplegia. During cardioplegia, however, an increase in Ca2+ entry via this mechanism can be decreased due to increased intracellular H+ activity and a decrease in cellular ATP content. In this study giant excised cardiac sarcolemmal membrane patch clamp technique was employed to investigate the effect of cytosolic pH change on the Na(+)-Ca2+ exchanger, excluding the effect of ATP, in guinea pig cardiac myocytes. The outward Na(+)-dependent current, which has a characteristics of Hill equation, was decreased as pH was decreased in the range of 7.5-6.5. The current density generated by the Na(+)-Ca2+ exchange transport was 56.6 +/- 4.4 pA/pF (Mean +/- S.E.M.) at pH 7.2 and decreased to 42.9 +/- 3.0 pA/pF at pH 6.9. These results imply that Na(+)-Ca2+ exchange transport, operating in a reverse mode during cardioplegia, decreases due to increased intracellular H+, and further suggest that consequent intracellular Na+ accumulation is one of aggravating factors for Ca2+ influx during reoxygenation or reperfusion.

4.Acquired Pure Red Cell Aplasia due to Anti-Erythropoietin Antibodies in a Patient Undergoing Hemodialysis.

A 63-year-old man was placed on hemodialysis for the end-stage of renal disease secondary to renal artery stenosis. He was also regularly given epoetin, subcutaneously, for anemia associated with his renal disease. Rapidly progressing erythropoietin (EPO) resistant anemia and reticulocytopenia developed after 1 year of hemodialysis. The patient required frequent red blood cell transfusions. The bone marrow examination demonstrated selective erythroid hypoplasia. A detailed search for the cause of the erythroblastopenia revealed nothing, with the exception of anti-EPO antibodies (Ab). Pure red cell aplasia (PRCA) was suspected due to the anti-EPO Ab. With the immunosuppressive agent and change to the epoetin-therapy, the patient recovered his hemoglobin and reticulocyte counts. Particular attention should be paid for the possibility of PRCA due to anti-EPO Ab in patients undergoing rHuEPO therapy, with an unexplained recombinant human erythropoietin (rHuEPO) resistant anemia, especially via the subcutaneous route.

5.A Case of Adrenal Hemorrhage in an Adult.

Acute bilateral adrenal hemorrhage in patients who have been stressed by surgery, sepsis, and /or hypotension, as well as in patients who have been a preexisting coagulopathy, has previously been reported, but acute unilateral adrenal hemorrhage is extremely uncommon. We describe herein a 37-year-old man who had diabetes for 2 years and developed unilateral adrenal hemorrhage in coexisting adrenocortical adenoma.

Type I neurofibromatosis is an autosomal dominant disorder that occurs in 1 of 3, 000 births. It is characterized by multiple cutaneous neurofibromas, cafe-au-lait spots of the skin. Patients with neurofibromatosis are at increased risk of developing malignancies, particularly neural crest and other non-neural crest neoplasms. The term `malignant fibrous histiocytoma' was first introduced in 1963 to refer to a group of soft tissue tumors characterized by a storiform or cartwheel like growth pattern. Malignant fibrous histiocytomas are the most common type of soft tissue sarcoma that occurs in late adult life. Herein, our recent experienced a case of a malignant fibrous histiocytoma in a 28 year-old female with type I neurofibromatosis is reported, with a review of the literature.

7.The Relationship among Eating Traits, Personality Characteristics, and Other Psychopathology in the Korean General Population.

OBJECTIVE: The purposes of this study were to clarify the roles of personality characteristics and general psychopathology, including depression and hypochondriasis, in individuals with eating disorders, and to test a continuum hypothesis of eating disorders. METHODS: Using a multi-stage questionnaire sampling method including area sampling, proportionated stratified sampling, and quota sampling, we surveyed 3,062 subjects(1249 males and 1813 females) from a target of 4,400 Korean adults over the age of 18 in a nationwide area(9 ku's, 10 middle or small cities, and 17 kun's). We used the questionnaire which consisted of three parts:general information, scales for eating traits, and scales for personality characteristics and other general psychopathology. RESULTS: Psychoticism was the only personality characteristics which had a significant relationship with various eating traits including 'eating habits'(r=-0.3195), 'the Eating Attitudes Test'(EAT, r=0.3657), and 'preference for vegetables and fish, and dislike for sweet,tasting foods'(r=-0.2740). Lie scale also had a significant relationship with 'preference for snacks and instant foods'(r=-0.2117). These results were consistent in examining the relationships across genders. In female, there was a significant relationship between 'preference for the traditional Korean foods' and 'psychoticism'(r=-0.2103) in addtion to above relationships. However, there were no significant relationships between any of the eating traits and other psychopathology. Relative to the relationship between personality characteristics and general psychopathology, there were significant correlations between 'depression' and 'interoversion-extraversion'(r=-0.2174), 'depression' and 'neuroticism'(r=0.4510>, and 'hypochondriasis' and 'neuroticism'(r=0.3432). These correlations in female was the same as those of the total group, while among males, 'depression' was significantly correlated with all four personality characteristics, and 'hypochondriasis' was significantly correlated with 'interoversion-extraversion'(r=-0.2265) and 'neuroticism'(r=0.3762). CONCLUSION: These results suggest that psychoticism is the only personaltiy characteristics related to eating-specific psychopathology, while general psychopathology, such as depression and hypochondriasis is not related to eating pathology but may influence eating disorders by their interaction with other personality characteristics, such as interoversion-extraversion and neuroticism. Considering the results of previous studies in the patients with eating disorders and high risk groups, which are similar with our results in the general population, our results support a continuum hypothesis of eating disorders. Our results also suggest that gender differences in the pathology of eating disorders are the result not of difference in eating-specific pathology itself but through difference in influences of concurrent general psychopathology, such as depression and hypochondriasis.

8.The Effect of Second Look Endoscopy in Patients with Peptic Ulcer Bleeding.

BACKGROUND/AIMS: Performing second look endoscopy has been suggested in order to reduce recurrent bleeding. We assessed whether second look endoscopy reduces the risks of recurrent bleeding and the mortality rate for patients suffering with peptic ulcer bleeding. METHODS: From February 2003 to June 2004, we have performed a prospective, randomized, controlled study of 141 patients with bleeding peptic ulcers, and these patients had been admitted to Kyungpook National University Hospital. Seventy patients in the study group were randomized to receive scheduled second look endoscopy. Seventy one patients in the control group were observed closely. RESULTS: Seventeen of the admitted 141 patients were found to have rebleeding after initial therapeutic endoscopy. The overall rebleeding rate was 12.1%. Although the duration of the hospital stay was significantly lower for the study group than for the control group (p<0.05), the rebleeding rate was similar for both groups (p<0.05). The two groups were similar in respect to the mortality during the period of hospitalization, the volume of hypertonic saline epinephrine that was injected and the number of hemoclips that were used. CONCLUSIONS: From these results, we can conclude that scheduled second look endoscopy with retreatment did not reduce the risk of recurrent bleeding for patients with peptic ulcer bleeding. Therefore, scheduled second look endoscopy should be selectively performed for the patients who are at a high risk for peptic ulcer bleeding.

9.Surgical Treatment of Intracranial Arachnoid Cysts.

Eighteen patients with symptomatic intracranial arachnoid cysts underwent 27 surgical procedures between January 1990 and December 1995. These were analysed with respect to their surgical procedures and results, complications and prognosis retrospectively, with an attempt assess the efficacy of each surgical procedure. Thirteen patients underwent excision of the inner and outer membranes with fenestration procedures to the basal cisterns(primary excision group), resulting in excellent or good outcomes in 12 cases, of which three patients were subjected to secondary shunt procedures due to symptom recurrence in two and persistence of the cyst in one during follow-up radiological study. There were no significant complications in this primary excision group. Four patients out of five in the primary shunt group had good or excellent results. But shunt revisions had to be performed in two patients;of these two patients, one died from subdural empyema and sepsis while the outcome of the other one was satisfactory. The results of this study suggested that excision and fenestration should be considered as the primary procedure in patients with symptomatic intracranial arachnoid cyst, especially in the pediatric age group.

Targeted therapies guided by molecular diagnostics have become a standard treatment of lung cancer. Epidermal growth factor receptor (EGFR) mutations and anaplastic lymphoma kinase (ALK) rearrangements are currently used as the best predictive biomarkers for EGFR tyrosine kinase inhibitors and ALK inhibitors, respectively. Besides EGFR and ALK, the list of druggable genetic alterations has been growing, including ROS1 rearrangements, RET rearrangements, and MET alterations. In this situation, pathologists should carefully manage clinical samples for molecular testing and should do their best to quickly and accurately identify patients who will benefit from precision therapeutics. Here, we grouped molecular biomarkers of lung cancers into three categories—mutations, gene rearrangements, and amplifications—and propose expanded guidelines on molecular testing of lung cancers.
