3.The relationship between insomnia severity and depression, anxiety and anxiety sensitivity in general population.

The objective of this study was to investigate the relationship between insomnia severity, depression, anxiety, and anxiety sensitivity and to find out the explanatory variables that account for the insomnia severity among depression, anxiety, and anxiety sensitivity in general population. 95 mentally healthy volunteers who visit health promotion center of Kangbuk Samsung hospital for their regular medical examination were requested to complete Athens Insomnia Scale, Beck Depression Inventory, State-Trait Anxiety Inventory, and Anxiety Sensitivity Index. Association between total scores of Athens Insomnia scale and other variables (total scores of Beck Depression Inventory excluded item 16, total scores of State Anxiety, total scores of Trait Anxiety, and total scores of Anxiety Sensitivity Scale) was assessed individually with partial correlations adjusted by age and then together using multiple regression analysis. The total scores of Athens Insomnia Scale were significantly associated with total scores of Beck Depression Inventory excluded item 16(r = 0.541, p<0.001), total scores of Trait Anxiety(r = 0.642, p<0.001), total scores of State Anxiety(r = 0.267, p<0.05), and total scores of Anxiety Sensitivity Index(r = 0.312, p<0.01). Total scores of trait anxiety showed the highest correlation with the total scores of Athens Insomnia Scale and was the significant predictor to total scores of Athens Insomnia Scale among the other predictor variables (p<0.001). These results show that insomnia severity is positively correlated with depression, anxiety, and anxiety sensitivity. The correlation was strongest with trait anxiety. In addition, our results suggest that trait anxiety is associated with insomnia severity in general populations.

In 1868 Charcot described the join in tabes dorsalis, which since then has been designated as Charcot neuroarthropathy. It is characterized by painless swelling and abnormal mobility of the affected joint. Arthropathy of the type described by Charcot has been associated with various other conditions, such as diabetes mellitus, syringomyelia, injury to nerve root, congenital indfference to pain, leprosy, and intra-articular use of steroid, etc. Authors present a case of Charcot joint which involved the left knee joint associated with pathologic fracture of the left tibia and fibula.

6.Suicide prevention by education.

The best and only solution for suicide is prevention. Suicidal behavior can be divided into 3 phases: suicidal ideation, suicidal planning, and suicide attempt. To prevent suicide more effectively, it is better to intervene at an earlier phase of suicidal behavior before suicide attempts occur. It is essential to educate the public, gatekeepers, and mental health professionals about suicide prevention. The authors suggest that the role of the public and the gatekeepers-identifying signs of suicide earlier and referring to professionals-is very important because suicidal behavior is common in Korea. Although many public institutions or private organizations that are working for suicide prevention in Korea have made an effort to educate the public and the gatekeepers, the outcome has not yet been satisfactory due to a lack of structured programs for education. There are several kinds of well-made educational programs for gatekeepers. However, most of these programs were developed in foreign countries which have social and cultural backgrounds different from Korea. It is essential to develop a structured, systemized educational program that reflects the cultural and social characteristics of Korean society for the public and the gatekeepers.

8.The Effects of Ephedrine and Prehydration on the Cardiovascular Response during Spinal Anesthesia.

BACKGROUND: This study was designed to compare the efficacy of an ephedrine intramuscular injection with crystalloid or colloid administration for the prevention of cardiovascular change during spinal anesthesia undergoing Transurethral Resection of Prostate (TURP). METHODS: Eighty ASA I-II patients scheduled for TURP under spinal anesthesia were randomly allocated to receive non of the vasopressors or fluids (control group), 40 mg of i.m. ephedrine (ephedrine group), 15 ml/kg of i.v. lactated Ringer'solution (crystalloid group), and 8 ml/kg of i.v. pentastarch(colloid group). 12 mg of 0.5% hyperbaric tetracaine was injected through a 24 gauge Quinke needle at the L4-5 interspace in all patients. All patients were placed in the supine position and blood pressure (BP) and heart rate were recorded at 1-min intervals for 10 min, 2-min intervals for the next 10 min and subsequently at 5-min intervals. The sensory level was determined by pin prick test using 27 gauge needle. RESULTS: In the ephedrine group, systolic blood pressure was significantly greater than the other groups between 1 and 40 min after spinal anesthesia. (p<0.05) The changes in diastolic BP, mean BP and heart rate in the all groups were similar. CONCLUSION: The result of intramuscular injection of ephedrine may be more effective than crystalloid or colloid adminstration for the prophylaxis of hypotension during spinal anesthesia undergoing TURP.

10.Percutaneous Automated Gun Biopsy of Localized Pulmonary Lesions.

PURPOSE: Percutaneous needle aspiration biopsy under the imaging guidance in the diagnosis of intrathoracic diseases has become a standard practice. Core tissue obtained by automated gun biopsy(AGB) is believed to be more diagnostic than materials from fine needle aspiration(FNA) by many cytopathologists. The authors evauated the role of automated gun biopsy in localized pulmonary lesions with respect to the its accuracy and complications. MATERIALS AND METHODS: We performed 107 percutaneous biopsies in localized pulmonary lesions under fluoroscopic guidance among 107 patients:AGB using 18G or 20G was done in 81 patients, and both AGB and FNA was done in 26 patients. RESULTS: Biopsy samples sufficient for histologic diagnosis were obtained in 94(87.9%) of 107 patients, AG8 in 73(90.1%) of 81 patients and AGB and FNA in 21(80.8%) of 26 patients. 11 of 13 negative results were due to inadeguate specimen size for the histologic diagnosis or were composed of necrotic areas, and 2 of 13 negative results were obtained through nontarget tissue. 10 patients developed pneumothorax and 4 of them required chest tube insertion. Minimal hemoptysis was found in 3 patients. CONCLUSION: Percutaneous automated gun biopsy under fluoroscopic guidance was easy and simple method of obtaining specimens of good quality and quantity. It may be an useful procedure in the diagnosis of localized pulmonary lesions.
