Prediction of individual outcome after cardiopulmonary resuscitation is of major medical, ethical, and socioeconomic interest but uncertain. We experienced the case thats the patient got complete neurologic recovery after the 123th day firm cardiac arrest, who had been suspected to go with poor prognosis because she got the findings of Glasgow Coma Scale 4, severe diffuse encephalopathy on encephalogram and generalized tonic-clonic seizure at the 4th day. Recently, a 29 year-old women who sustained from respiratory arrest induced presumably by sedative and anticonvulsant therapy for control of seizure that happened during local lidocaine anesthesia far mamoplasty was transfered to our emergency medical center from local private plastic office. Arrest time was about 20 minutes. On hospital arrival, she had a pulseless bradyasystole and no respiration, but spontaneous circulation was restored at 10 minutes artier CPR started. We started cerebral oriented resuscitation including mild hypothermia(34degrees C), hemodilution, calcium channel blocker infusion. On hospital day 4, patient's glasgow coma scale(GCS) was 4. On hospital clay 7, Brain Magnetic Resonance Imaging(MRI) showed high signal intensity on T2WI, involving the bilateral basal ganglia. After contrast administraton, marked enhancement can be seen at the lesion site. Patient's glasgow coma scale(GCS) increased step by step to 5 on 8th day, 7 on 14th day, 10 on 15th day, 13 on 17th day, 15 on 20th day. 40 days later the patient was discharged with minor neurologic abnormality including hand tremor, dysphonia, amenorrhea and Mini Mental State Examination(MMSE) score(26). Long-term Follow up revealed that all neurologic functional abnormality inducting hand tremor, dysphonia, amenorrhea and MMSE score(26) is completely recovered on 123th day after episode of cardiopulmonary arrest.

To evaluate the intractability of partial epileptic patients by variables, the author studied 113 patients (uncontrolled: 45, controlled: 68) who were admitted to the Department of Neurology, College of Medicine, Yeungnam University from January, 1991 to August, 1993. The results were as follows. The items related to complex partial seizures, multiple seizure types and a histories of status epilepticus or clusters of seizures were significantly associated with drug-refractoriness (p<0.01). A high frequency of seizures before evaluation was associated with a poor outcome(p<0.01). The presences of known etiology of seizures, neurologic abnormalities and psychiatric disturbance were associated with limited treatment responses(p<0.01, p<0.05, p<0.01). An abnormal EEG findings such as background slowing, focal slowing, epileptiform discharges or secondarily bilateral synchrony were statistically significant (p<0.01). Age at onset, sex, distribution of epileptic foci, duration of seizure before evaluation, family history and abnormal neuroradiologic findings were not statistically significant. By these results, it was suggested that having at least four factors of the above variables were associated with limited treatment response.

The author studied 20 healthy adults (20 hands) as a control and 30 patients (40 hands) with carpal tunnel syndrome to evaluate the clinical usefulness of measuring nerve conduction velocity after wrist flexion in diagnosis of carpal tunnel syndrome. The median nerve conduction velocity over wrist to finger segment was measured before and after wrist flexion for 1, 2 and 5 minutes, using belly-tendon method for motor nerve distal latency (MNDL) and antidromic method for sensory nerve conduction velocity (SNCV). The results were as follows: 1. In control group, MNDL increased in 1 hand and SNCV decreased in 2 hands after wrist flexion. In patient group, MNDL increased in 2 hands and SNCV decreased in 3 hands after wrist flexion. 2. In both control and patient group, there were no significant changes in mean values of SNCV and MNDL between before and after wrist flexion. 3. Phalen's wrist flexion test was positive in 5 percent of control and 60 percent of patient group. 4. Tinel's sign was present in 10 percent of control and 33 percent of patient group.

A 25-year-old 7 month pregnant woman with mitral stenosis was hospitalized because of life threatening hemoptysis dispite intensive medical treatment. Emergency mitral commissurotomy resulted in prompt cessation of hemorrhage and at nine months follows-up, the hemoptysis has not recurred. Mitral valve surgery appears to be consitently associated with rapid and sustained cessation of hemoptysis. Therefore, surgery should be considered in hemoptysis due to mitral stenosis, particulary when the danger of asphyxiation exists.

Purpose: This study developed a Korean sentiment questionnaire by adapting an existing English survey using focus group interview (FGI)-based cognitive interview (CI) techniques to investigate the utilization of outcome measures (OMs) among Korean physical therapists. Methods: The existing OMs survey questionnaire was adapted by dividing eight physical therapists into two groups: mid-experienced (n=4) and high-experienced (n=4). Each group participated in a 120-minute FGI-based CI session. All interviews were recorded, and the researcher transcribed the data immediately after each interview. The data were then organized and categorized into themes using Excel 2021 and verified with the participants. Results: FGI-based CI sessions were conducted with Korean physical therapists to revise the English version of the questionnaire, tailoring it to the local context. Four main themes emerged from the interviews: inappropriate items or translations, questionnaire length and organization, questionnaire improvements, and additional items. The questionnaire was revised based on the feedback obtained during these interviews. Conclusion The questionnaire was modified according to the themes derived from the interviews. The questionnaire was developed to represent the clinical environment of Korean physical therapy accurately by removing elements of the questionnaire unsuitable for the Korean sentiment and incorporating the perspectives of Korean physical therapists.

We report a rare case of subepithelial hematoma of the renal pelvis (AntopoI-Goldman Lesion). A 55-year-old women visited our hospital because of gross hematuria. Ultrasonogram showed a 4cm well-defined solid mass of the left renal pelvis. Intravenous pyelography revealed compression of the left upper pelvicocalyceal system by the mass with contrast filling within the mass. CT scan revealed Iobulated well defined mass in the left renal pelvis extending into renal parenchyme. A transitional cell carcinoma or renal cell carcinoma was suspected radiologically, and the patient underwent left total nephrectomy. In pathology, the lesion turned out to be a subepithelial hemaroma. In the differential diagnosis of renal malignancy, a subepithelial hematoma of the renal pelvis may be included.
