1.Bacteriological Study of Pyodermas.

Bacteriological study, including antibiotic sensitivity tests, of 81 patients with pyodermas such as impetigo, folliculitis, furuncle, carbuncle, cellulitis and acuete infectious eczematoid dermatitis, was carried out during 4 months period from June, 1980 to September, 1980 at the department of dermatology, Han Il Hospital. The results were as follows: 1) The causative agents of impetigo in 42 patients, were coagulase positive Staphylacoccus aureus in 33, p-hemolytic Streptococcus in 6 and both organisms in R (S. aureus and 3-hemolytic Streptococcus). Coagulase negative Stgaphylococcus was not found. 2) The causative agents of superficial and deep follicultis in 23 cases, were coagulase positive S. aureus in 12, coaulase negative Staphylococcus in 5, p-hemolytic Streptococcus in 4 and S. aureus and p-hemolytic Streptococcus in 3. 3) The causative agents of cellulitis in 2 cases, were coagulase positive S. aureus in 1, and coagulase negative Staphylococcus in 1. 4) The causative agents of acute infectious eczematoid dermatitis in l4 cases, were coagulase positive S. aureus in 6, coagulase negative Staphylococcua in 1, p-hemolytic Streptococcus in 2, and two organisms in 5 (4 cases were mixed).

2.Hyperkeratosis of the Nipples Developed during Pregnancy: report of a case.

Hyperkeratosis of the nipple and areola is rare dermatosis that may be divided into three categories: a type of epidermal nevus, a type associated with ichthyosis and a type of nevoid form seen in voung women. A 23-year-old woman was seen for skin lesion involving both nipples. These changes were first noted when the patient was in the sixth month of gestation. Physical examinations revealed papillomatous thickening of both nipples with brownish black discoloration. Histopathological changes were mild hyperkeratosis, keratotic plugging and telangiectasia. It was interesting to note that the skin lesions disappeared spontaneously 7 days after parturition without treatment. A possible correlation between pregnancy and acquired form of this rare skin disorder is suggested.

3.A Case of Hypereosinophilic Syndrome.

The hypereosinophilic syndrome consists of peripheral blood eosinophilia of 1500/ mm3 or more without a known cause, plus signs and symptoms of organ eosinophilia. We report a case of hypereosnophilic syndrome with cutaneous manifestation. A 31 year-old female had intermittent fever, sbdominal pain, anemia, systolic murmur, hepatosplenomegaly, cheat discomfort, dry cough without rales, and skin-colored wheal on the trunk. Diagnoais of hypereosinophilic syndrome wss established by clinical findings, marked blood eosinophilia without a known cause, bone marrow aspiration and biopsy, liver scan, and histopsthologic findings of the skin.

4.A Case of Herpes Zoster with Generalized Varicella like Eruption.

Generalized varicella-like eruption occurs in 2 to 10 percent of unselected patients with localized zoster, the majority of whom are patients with immunologic defects due to underlying malignanies (particularly rnalignancy of hematopoietic system) or immunosuppressive therapy. A 25-year old male noted onset of pain and burning sensation on the right side of cheet nine days prior to admission. The following day multiple vesicles were noted on right side of the chest. Three day before the admisson erythernatous maculopapules were noted on the abdomen. One day prior to admission vesicular lesions appeared over the entire hody. The patient was treated with topical application of calamin lotion, and with kanamycin and Cephalexin to prevent secondary infection. The secondary vesicles had disappeared by the fifth hospital day, but the initial lesion had not improved. On the fifteenth hosptital day the initia1 lesions has disappeared.

6.Levels of Insulin - like Growth Factor 1 ( IGF - 1 ) , Insulin - like Growth Factor Binding Protein 3 ( IGFBP - 3 ) , Osteocalcin and Deoxypyridinoline biochemical markers in either surgical menopause or natural menopause women.

OBJECTIVE: The purpose of this study was to investigate possible menopause related changes in circulating insulin-like growth factor binding protein 3 (IGFBP-3) levels and their relationship with insulin-like growth factor 1 (IGF-1) plasma levels, osteocalcin(Ost) and urinary deoxypyridinoline(Dpd) in either surgical menopause or natural menopause, METHOD: Seventy-two postmenopausal women (surgical menopause 48, natural menopause 24) were invited to participate in this study. In all subjects plasma IGF-1 and IGFBP-3 levels were measSURED by radioimmunoassay and Ost and Dpd were measured by enzyme linked immunosorbent assay(ELISA). RESULTS: No difference was found between mean IGFBP-3 plasma levels in the two groups studied(3,522 +/- 926 vs 3,854 +/- 569 ng/ml), while mean IGF-1 levels were significantly lower in natural menopause as compared with surgical menopause (natural 126 +/- 44 vs surgical 163 +/- 66 ng/ml, p=0.007). No difference was found between mean Ost levels in the two groups studied (natural menopause 8.0 +/- 2.9 vs surgical menopause 8,9 +/- 2.1 ng/ml, p=0.113) and mean Dpd levels in the two studied (natural menopause 6.8 +/- 2.3 vs surgical menopause 7.8 +/- 3.4 mM, p=0.213). CONCLUSION: IGF-1 was significantly lower in natural menopause as compared with surgical menopause, but no significant difference was found in IGFBP-3, Ost, and Dpd levels

7.Myxoma of the Breast: A case report.

A case of myxoma of the breast was reported. A 25-year-old woman showed a lump in the left breast which was suddenly enlarged after the delivery of a baby. A tumor measuring 73.53cm was located in the upper outer quadrant of the breast. Fine needle aspiration cytology revealed scattered benign spindle shaped mesenchymal cells with abundant myxoid material. On excision, the tumor was partly solid, partly cystic and multilocular. Each nodule consisted of an abundant mucoid material with a few spindle or stellate mesenchymal cells. Pleomorphism and mitosis were absent. The mesenchymal cells showed immunoreactivities for S-100 protein, smooth muscle actin, and CD34. The myxoid stroma was positively stained with alcian blue and mucicarmine. These findings suggest that constituent cells derive from totipotential primitive mesenchymal cells.

8.Laparoscopic transabdominal extraperitoneal mesh repair of lumbar hernia.

Lumbar hernias are rare posterolateral abdominal wall defects that may be congenital or acquired. There are two types of lumbar hernia, the superior lumbar hernia through Grynfeltt triangle, and the inferior lumbar hernia through Petit triangle. Many techniques have been described for the surgical repair of lumbar hernias including primary repair, local tissue flaps, and conventional mesh repair. But these open techniques require a large skin incision. We report a case of superior lumbar hernia, which was successfully repaired using a laparoscopic approach.

9.Rhabdomyolysis in Doxylamine Succinate Overdose.

BACKGROUND: Doxylamine succinate(DS) is an antihistamine commonly used as an over-the-counter medication to relieve insomnia and frequently involved in overdoses. Its overdoses are dominated by anticholinergic effect. Recently it was revealed that DS had a direct effect on muscle, while its exact mechanism is not clear yet. We evaluated the patients with rhabdomyolysis induced by DS overdose for patients disposition based upon clinical decision, especially by creatinine phosphokinase(CPK). METHODS: We reviewed retrospectively the medical records of patients admitted by DS overdose from Jan. 1998 to Oct. 1999. Seventy and nine cases of DS overdose were evaluated with respect to age and sex distribution, amount ingested, clinical symptomatology, time from ingestion to visit, pattern of CPK, amount of bicarbonate used as therapy, complication and prognosis, especially in patients complicated rhabdomyolysis. RESULTS: Rhabdomyolysis, diagnosed as more than 1,000I. U/L of CPK, has been noted in 25(31.6%) of 79 cases of DS overdose visited to our emergency department(ED). In patients diagnosed rhabdomyolysis, the number of man was 10 cases(40%) and the number aged between 20 and 40 years was 22 cases(88%). The average time from DS ingestion to ED visit was 459 minutes. The amount of DS ingested was 500-5,000mg(mean, 1,980mg). 13(52%) cases ingested less than 2,250mg of DS. The initial levels of CPK(range, 48-14900I. U/L; normal range, 26-200I. U/L) after admitting to our emergency department were normal in 15 cases(60%) of rhabdomyolysis patients. The range of peak CPK levels after ingestion was 607 to 412,500I. U/L(mean, 33,550I. U/L). Its peak time was 6 to 96 hours(mean, 28.96 hours). In 14 cases(67%) of 21 visiting within 24 hours after ingestion, peak time of CPK ranged 12 to 24 hours after ingestion. The amount of bicarbonate used as therapy of rhabdomyolysis ranged 100 to 2,740mEq(mean, 656mEq) and all patients was discharged after improvement without other complication including acute renal failure. CONCLUSIONS : Although patients ingested less than 2,250mg of DS, emergency physicians should observe them more than 24 hours after DS ingestion with CPK follow-up after gastric irrigation and charcoal administration.

10.Imaging Feature of Radiation Induced Lung Disease.

BACKGROUND AND PURPOSE: Radioopaque lesions are commonly seen in patients who received thoracic radiotherapy for various kinds of thoracic neoplasm, But therir exact diagnos are sometimes uncertain. PATIENTS AND METHODS: We examined simple chest radiograph and computed tomogram(CT) of 69 patients who received thoracic radiotherapy for lung cancer and were follow up at least 6 months in Yeungnam University Medical Center. RESULTS: Of the 69 patients. thirty-eight patients showed radioopaque lesions in their chest radiographs except radiation fibrosis; radiation pneumonitis was witnessed in 24 patients. infectious pneumonia in 8 patients, and recurrence in 6 patients. In radiateionpneumonitis patients, the pneumonitis occurred usually between 50 to 130 days after receiving radiation therapy, and interval between pneumonitis and fibrosis is 21 to 104 days. Simple chest radiographs of radiation pneumonitis(24 patients) represented ground glass opacities or consolidation in 4 cases(type I, 17%), reticular of reticulonodular opacities in 10 cases(type II, 42%), irregular patichy consolidations in 2 cases( type III, 8%), and consolidation with fibrosis in 8 cases(type IV, 33%), CT represent ground glass opacities or consolidation in 5 cases(type I, 29%), irregular nodular opacities in 3 cases(type II, 19%), irregular opacity beyond radiation fields in 3 cases(type III, 18%), and consolidation with fibrosis in 6 cased(typeIV, 35%). The CT of four patients who represented type II on simple chest radiographs reveal type I and III, and CT of two patients with clinical symptoms who had no abnormal finding on simple radiograph revealed type I. CONCLUSIONS: In conclusion, computed tomogram is superior to the simple radiograph when trying to understand the pathologic process of radiation pneumonitis and provide confidence in the diagnosis of radiation induced lung disease.
