BACKGROUND AND OBJECTIVES: The development of coronary intervention registry system is required for analysis of current status and more efficient management of patients with coronary artery disease in Korea. We developed database system and registry system in internet environment to confirm the possibility of nationwide system construction and of useful information providing on coronary intervention. MATERIALS AND METHOD: Database for coronary intervention and web-based registry system were developed and installed on internet server. Data elements consist of demographics, coronary risk factors, initial diagnosis, lesion characteristics, type of interventional procedures, and post-procedure outcomes. RESULTS: Total 734 patients were registed by 12 intervention centers from November 1998 to October 1999. Patients with angina pectoris and myocardial infarction were 78.5% and 21.5%, respectively and 14.6% of all patients had history of previous coronary intervention. Major coronary risk factors of the patients were smoking (38.6%), hypertension (33.8%), and diabetes (18.0%). Proportions of the patients with single-vessel disease, two-vessel disease, and triple-vessel disease were 64.7%, 26.2%, and 9.1%, respectively. Left main disease was detected in 2.0%. Balloon PTCA only was performed in 209 patients and 530 patients underwent coronary stent insertion. CONCLUSION: Multicenter data of coronary intervention were collected via internet-based system, and it was possible to construct intervention database. These web-based systems linked with database will provide status of Korean interventional procedures and be able to support multicenter study efficiently by expanding the participating hospitals and including more detailed database fields.

Clinical observation of thirty-eight cases of pyogenic osteomyelitis of the spine for the period of eleven years, from 1965, at Seoul National University Hospital was carried out with the following results. 1. The incidence of pyogenic osteomyelitis of the spine among total cases of the pyogenic osteomyelitis was 6.9% and the ratio between the spine tuberculosis and the pyogenic osteomyelitis of the spine was 1:0.067. 2. The sex ratio between male and female wss 3.8: 1 The incidence was higher ia the second and third decade and the average prevalent age was 30.8 years of age. 3. The site of infection was in the lumbar vertebrae in 25 cases(66%), thoracic in seven(19%), lumbosacral in three, sacral in two and cervical in one. In the region of a vertebra, the vertebral body was affected in 33 cases (87%) and the neural arch and processes in other 5 cases (13%) 4. The causative micro-organism was isolated in 27 cases from the diseased focus; coagulase positive staphylococcus in 18 cases (67%), streptococcus in one, salmonella in one and the mixed infection in two. 5. Frequent clinical symptoms and signs were back pain, tenderness, muscle spasm, fever and chillness, and limited motion of the spine. 6. The roetgenographic findings were in order of disc space narrowing, destruction of the vertebral body, kyphosis or loss of lordosis, erosion, bony bridging or spur formation, sclerosis and paravertebral abscess. 7. The conservative treatment was performed in 14 cases. Among 7 cases in which the follow-up was possible, only 3 cases obtained the satisfactory result, four cases were readmitted for the recurrence and two cases among them needed the operative treatment later. The operative treatment was peformed in 26 cases; The focal curettage with anterior interbody fusion through anterior approach was carried out in 13 cases with the satisfactory result in almost all cases. 8. The advantages of the operative treatment were that the isolation of the offending organism was efficient and the definite diagnosis was feasible, and especially in cases of anterior spinal fusion the healing and union of the infected spine was better clinically and roetgenographically in comparision with those of conservative treatment, in which the expected spontaneous fusion was not found in any case and the recurrence was also frequent.

Apert described acrocephalosyndactyly as a clinical entity in 1906. At first he included a patient with craniofacial deformities combined with syndactyly of both the upper and lower extremities. This is a rare congenital disturbance in the growth of bone and soft tissue affecting principally the head, the hands and the feet. More than 200 cases have been reported in the world literature, and three cases in Korea. Authors experienced one case of the classic pattern of Apert's syndrome and treated surgically for syndsctyly.

80 cases of Cysticercosis were studied and analysed, which were diagnoses by biopsy at Seoul National University in the past three years from January-1978 to March-1980. The results were summerized as follows. 1. The relative incidence ratio of cysticercosis among 18,941 surgical specimens was 0.42%. 2. The age group between 11 and 50 years old occupied 87.5%. 3. Of the 80 cases, there were 38 males and 42 female patients, without significant sex predominence. 4. The common sites of cysticercosis were upper extremities (35.0%) and anterior surface of the trunk (37.5%) 5. Epidemiologically, there was marked predominence of rural area over urban area. 6. There were 8 cases (10%) of definite cerebral cysticercosis and 4 cases (5%) of suspicious cerebral cysticercosis. 7. In the cases of nodular masses of extremities, the parasitic infection, especiaily cysticercosis celluiosae, must be included in differential diagnosis distinguished from other soft tissue tumors.

The cubitus varus is one of the most common complications of supracondylar fracture of humerus. The authors treated surgically 45 cases of cubitus varus in the Seoul National University Hospital, from 1977 to 1981, and the results were as follows: 1. The mild ulnar n. palsy signs were found in 11 cases(24.5%). 2. The supracondylar, closed wedge osteotomy was done through lateral approach and fixed by cross pinning with 2 K-wires. 3. The postoperative immobilization must be more complete, and the average duration of immobilization was 7 weeks. The osteotomy site united in the all cases, and no limitation of range of motion developed. 4. The incidence of postop. ulnar nerve palsy was the same in the osteotomy only group as in the osteotomy and ulnar n. transposition group. The osteotomy and ulnar n transpositin group recovered naturally, but 2 cases of the osteotomy only group did not recover so ulnar n. transposition was performed later. 5. There was no recurrence of deformity after the average follow-up of 2.2 years, so it was recommended that the correction of deformity should be done in the early childhood.

Congenital scoliosis is defined as a lateral curvature of the spine caused by congenital anomalies of the vertebral development. In this study seventeen congenital scoliosis patients seen and treated by either Milwaukee brace or surgical intervention at Department of Orthopedic Surgery, Seoul National University Hospital from December 1972 to April 1978 were evaluated according to frequency of the curve pattern, character of the deformity, method of treatment and its correctability. The following results were obtained in this study. 1. Age distribution was from 2 to 39 years and mean age was 13.4 years. 2. The most common deformity pattern was hemivertebra in 8 cases (47.1%), unilateral unsegmented bar in 4 (23.6%) and mixed type in 3 (17.6%). 3. The most common curve level was thoracic in 6 cases (35.3%), followed by thoracolumbar in 4 (23.5%) and lumbar in 4 (23.5%). 4. The best corrected type by Milwaukee brace was trapezoid vertebra (29.4%) and then mixed type(26.1%), followed by block type (20%) and hemivertebra(2.3%). 5. The operative treatment was performed in the cases of unilateral bar and progressive type with Milwaukee brace. In terms of the correctability of the curvature, the surgical intervention was better than the conservative treatment, with the result of its average correction of 33.5% that is compared with 15.9% of Milwaukee brace. 6. Halofemoral traction was effectively applied in the cases of rigid and severe curve; the initial curve 76.3 degress with 21.3% of flexibility. 7. Loss of correction in operative treatment was 2.3 degrees (6.5%).
