1.Effects of water running exercise on renal function.

A study was conducted to determine the effect of water running exercise (WRE) on renal function. The study involved 5 healthy males who performed maximal work by both WRE and bicycle ergometer exercise (BEE), and 6 males who performed head-out water immersion.
The results obtained were as follows;
1) The values of VO2max and maximum heart rate (HRmax) during WRE and BEE were approximately similar, and blood lactate concentration after exercise was significantly higher then the basal value in both types of exercise, although the value was significantly higher after WRE than after BEE.
2) Compared with the resting values on land, significant decreases in HR and plasma aldosterone (Ald) concentration were noted in head-out water immersion, but plasma antidiuretic hormone (ADH) concentration and urinary volume were not changed significantly from the resting values.
3) The secretion of both plasma ADH and Ald was significantly increased at the end of both types of exercise. However, a larger increase in Ald and ADH was observed after BEE than after WRE.
4) The rate of urine flow was obviously decreased soon after both types of exercise; this was positively correlated with changes in creatinine clearance and negatively correlated with changes in Ald concentration.
The results of the present study suggest that a better venous return is induced by the water pressure and cardiac output during WRE, possibly inducing the release of atrial natriuretic fsctor (ANF) . ANF may inpair the release of ADH and Ald to a greater extent in WRE than in BEE.

The category “Food with Health Claim” contains “Food with Nutrient Function Claim” and “Food for Specified Health Use (FOSHU)”. The definition of “Food with Nutrient Function Claim” is “food used to supplement nutritional factors such as vitamins and minerals”. Once certain standards have been met, public sale of the item is possible. In concrete terms these requirements specify that the daily intake of the aforementioned nutritional factors must be within a certain range, as well as the display of health claims and warning labels.
At the same time, because FOSHU possess components capable of affecting physiological function, and their application is essentially as specific health foods, each food to be sold as FOSHU requires authorization from the Japanese Ministry of Health. In this article we focus on the various tests required for approval.

Approximately 70 such FOSHU are recognized as useful in treating hyperlipidemia; in the present review we describe the safety and effectiveness of several of these foods.
6.A Rare Case of Abdominal and Thoracic Aortic Aneurysm Complicated with Buerger's Disease

We report a rare case of a 73-year-old man with abdominal and thoracic aortic aneurysms complicated with Buerger's disease. He complained of abdominal pain, nausea and an abdominal pulsatile mass. Computed tomography and angiography revealed an abdominal aortic aneurysm (58mm in diameter) and a thoracic aortic aneurysm (47mm in diameter). Y graft replacement was performed for abdominal aortic aneurysm. The size of the thoracic aortic aneurysm increased from 47mm to 60mm in 3 years, and hoarseness appeared. We then performed graft replacement of the thoracic aorta. In cases of Buerger's disease, we have to consider perfusion of the extremities when we need extracorporeal circulation, and we must shorten ischemic interval.
7.In utero hematopoietic stem cell therapy.

In utero hematopoietic stem cell transplantation (IUHSCTx) is a promising approach for the treatment of a potentially large number of fetuses affected by congenital hematologic disorders. With technical advances in prenatal diagnosis and fetal intervention, the majority of these diseases can now be diagnosed early in gestation, allowing consideration of prenatal treatment. It, therefore, stands to reason that there is increasing interest in performing in utero hematopoietic stem cell transplantation at many centers around the world. Although the approach remains experimentally promising, expansion of clinical application will depend on improved understanding of the biological barriers to engraftment in the fetus as well as on the development of effective clinical strategies based on the hematopoietic biology of individual disorders.

8.The process of bone response to running training in female growing rats.

Physical activity in the growing period has been shown to be effective for increasing bone mass because immature bones are more sensitive than mature adult bones to the stimulation with mechanical stress. However, bone growth is not uniform and changes markedly at puberty. Therefore, the response of bone to exercise may differ according to the growth process. The purpose of the present study was to investigate the process of the bone response to running training, and the relationship between the bone response and serum insulin-like growth factor-I (IGF-I) and IGF binding protein-3 (IGFBP-3) levels in female rats. Thirty-three female Wistar-Imamichi rats 4 weeks of age were divided randomly into control (CON) and running training (RUN) groups for 6 and 12 weeks. Training consisted of running on a flatbed treadmill at 30 m/min for 60 min/day, 5 days/week. The bone mineral content (BMC) and bone mineral density (BMD) in the whole and five parts of the tibia were measured by a dual-energy X-ray absorptiometer (DXA) . Simultaneously, we measured serum concentrations of IGF-I, IGFBP-3, osteocalcin and 17β-estradiol. The whole tibial BMD was significantly higher in the RUN groups than in the age-matched CON groups. When BMD was analyzed at five different studied parts within the same tibia, the increase of BMD. was noted in the proximal and distal cancellous bone in the 6-week RUN group, and in the diaphysial, cortical bone, in the 12-week RUN group. Serum concentrations of osteocalcin, a marker of bone formation, were not altered by training, whereas they decreased with aging. Serum IGF-I levels in the training groups were not changed, but IGFBP-3 levels were increased significantly only in 6-week RUN rats. As a complex between IGFBP-3 and IGF-I may be more improve than free IGF-I in the bone formation, the high levels of IGFBP-3 in the 6-week RUN group may induce an increase in the activity of IGF-I. There was a significant positive relationship between serum IGF-I concentration and BMD of the whole tibia in the 6-week study, and between the IGFBP-3 level and BMD in both the 6 and 12-week studies.
In conclusion, 1) the process of the skeletal response to running training is site-specific within the same bone, and 2) the increment of the IGFBP-3 level with training in the growth period may reflect the increment of tibial BMD through training.
9.Changes in bone mineral density during sexual maturation in male and female rats: correlation with serum IGF-1, IGFBP-3, osteocalcin and sex steroids.

Bone (tibia, femur, and lumbar spine) and blood samples were obtained from 100 (50 males and 50 females) Wistar-Imamichi rats in groups aged 3, 5, 7, 9, 12, 15 and 20 weeks old to investigate the changes in bone mass during puberty in relation to insulin-like growth factor 1 (IGF-1), IGF binding protein (IGFBP) -3, osteocalcin (OC) and sex steroids in normal rats.
Sharp increases in BMD (bone mineral density) in the tibia, femur and lumbar appeared earlier in female than in male rats, and the BMD in females tended to be higher than in males between 5 and 9 weeks old. After 9 weeks old, BMD in males was higher than that in females, as BMD in males continued to increase whereas that in females tended to remain in a steady state after this stage. This sex-related difference in changes in BMD pattern is probably related to the serum concentrations of IGF-1, IGFBP-3, testosterone, and 17β-estradiol with maturation. In males, marked increases in serum IGF-1 and IGFBP-3 concentrations appeared earlier than that in serum testosterone level. IGF-1 and testosterone peaked at 9 weeks of age, and thereafter remarked in a steady state, whereas IGFBP-3 reached a peak at 7 weeks of age, and then declined gradually. In females, the changes in patterns of serum 17β-estradiol, IGF-1, and IGFBP-3 levels were very similar. The levels increased gradually from 3-5 weeks old, peaked at 9 weeks, and then decreased slowly thereafter. In contrast, serum OC concentrations remain relatively high from 3 to 9 and from 3 to 7 weeks old in males and females, respectively, although OC in both sexes declined gradually with aging.
These observations suggest that BMD development occurs earlier in female than in male rats. This sex-related difference in changes in the BMD pattern may result from the earlier onset of puberty in females, and from the sex-specific differences in concentrations of IGF-1, IGFBP-3 and sex steroids with maturation.
10.Tricuspid Valve Repair for Traumatic Tricuspid Valve Regurgitation over 30 Years after Causative Trauma

A 28-year-old man was involved in a traffic accident that sandwiched his chest between a wall and a truck. Shortness of breath and other symptoms started to appear several years later. Echocardiography at that time showed severe tricuspid regurgitation due to a failed valve and ruptured chordae in the anterior leaflet. He was followed up with medication. Leg edema developed at the age of 62 years and worsening symptoms of heart failure over a period of 6 months indicated a need for surgery. Intraoperative findings revealed the ruptured chordae attached to the anterior leaflet and a scarred myocardium at the septomarginal trabeculation. The tricuspid valve was surgically repaired, the anterior leaflet chordae were surgically reconstructed, an annuloplasty ring was implanted to address the tricuspid regurgitation and atrial fibrillation was treated using the Maze procedure. Surgery 34 years after trauma has improved hemodynamic cardiac function and normalized the cardiac rhythm in this patient.
