Background: Sarcoidosis is a rare systemic granulomatous disorder with unknown cause. It is rare in the world and Vietnam. Objective: To study clinical, subclinical manifestations and therapy of sarcoidosis in Department of Respiratory of Bach Mai Hospital. Subjects and method: A study included 14 patients in Department of Respiratory of Bach Mai hospital, from 1989 to August 2006. This was a retrospective, descriptive study. Results and conclusions: In 14 patients, 14 patients (78.6%) was females, 3 (21.4%) was males. The disease occurred common in the ages of 20-49 years (85.7%). The clinical symptoms were very multiform but pulmonary symptoms were the most manifestations (78.6%). Abnormalities on chest radiographs were 92.8%, with stage 0 (7.1%), stage 1 (57.2%), stage 2 (28.6%), stage 3 (7.1%). Histopathologically, injuries of the sarcoidosis were typical in 9 patients (64.3%). The corticoid therapy satisfied the most success in 92.8% of patients. The side effects of the treatment occurred in 4 of 12 patients.

Infiltration of sarcoid granuloma in old cutaneous scars is one of the uncommon cutaneous manifestations of sarcoidosis. Here, we report the case of a 47-year-old female who presented with swelling and irritation in 5 old scars. She had acquired these scars 9 years ago in a traffic accident. An incisional scar biopsy revealed noncaseating granulomas consistent with sarcoidosis. High-resolution CT (HRCT) revealed right paratracheal, both hilar, paraaortic, and subcarinal lymphadenopathy without any nodular densities in both lung fields. Successful regression of cutaneous inflammation was achieved using a short course of oral steroids.

Patients with significant cardiac sarcoidosis are at increased risk of sudden death from ventricular dysrhythmias or conduction disturbances. We report a patient in whom there was radiographic and histologic evidence of systemic sarcoidosis; though histologic confirmation of involvement of heart by sarcoidosis is lacking, the clinical manifestations, radionuclide image findings, rhythm disturbances, and the response to steroid therapy are strong evidence in favor of myocardial involvement by the granulomatous process.

Standard endocrine therapy and chemotherapy can induce long-term remission in breast cancer patients; however, breast cancer can recur at any site. Pulmonary nodules with lymphadenopathy in advanced cancer patients are likely to be assumed as metastases. A 44-year-old woman with a history of breast cancer was presented to our institution with abnormal findings on 18-fluorodeoxyglucose positron emission tomography imaging, which suggested lung metastasis. She had previously been diagnosed with breast cancer (T1N2M0, Stage IIIa, intraductal carcinoma, triple negative cancer). Histological analysis of the mediastinal lymph node biopsy demonstrated sarcoidosis, showing a chronic, non-caseating, granulomatous inflammation. Our case highlights the need for non-malignant diagnoses in those with prior malignancies, and the need for histological evaluations in the event of first recurrence following potentially curative therapy.

Sarcoidosis is a systemic noncaseating granulomatous disorder of unknown origin that can involve any organ, including the skin. It has occasionally occurred on trauma sites, such as on the sites of herpes zoster infections and venepuncture, operations, cosmetic tattoos, and previous laser surgeries. About 25% of systemic-sarcoidosis patients have skin lesions, but cutaneous sarcoidosis can occur without a systemic disease, as in the patient whose case is described herein: a 40-year-old female who presented an erythematous plaque on the left nasolabial fold area, to which acupuncture was applied at an oriental clinic about 3 months earlier, as treatment for left facial nerve palsy.
