Ambulatory BP monitoring was performed in a patients with pheochromocytoma before and after removal of the tumor. Before surgery, it did not show any significant diurnal variation. But, after surgery the diurnal variation was restored.

6.Evaluation of Renal Injury in Patients with Renal Stone after ESWL.

The evaluation of renal damage in patients with renal stone after extracorporeal shock wave lithotripsy(ESWL) were analyzed by measuring chemical substances including cell-escaped enzymes in the blood and urine. We measured several chemical substances before ESWL, 1st, 3rd, and 4th day after ESWL. All patients had renal stones which were located in the renal pelvis and renal calices. Glutamic oxalate transaminase(GOT) and glutamic pyruvic transaminase(GPT) reached a peak on the 1st day after ESWL and decreased to pre-ESWL day values by the 4th day. Lactate dehydrogenase(LDH), creatine kinase(CK), and myoglobin also significantly increased on the 1st day after ESWL and decreased but did not return to pretreatment values by the 4th day. Urinary ratio of N-acetyl-beta-d-glucosaminidase(NAG) to creatinine(Cr) increased after ESWL and was still high on the 4th day. These results suggested that hemolysis and myolysis might be developed owing to shock wave exposure, and that the kidney was damagedto some extent. Therefore, we must be taken into account in renal damage during ESWL. Determination of urinary NAG and NAG/Cr ratio may be superior to GOT, GPT, LDH, CK and myoglobin in serum, after shock wave lithotripsy for renal stones.

9.The Need for the Introduction of Assessment Program of Good Animal Care and Use Facility.

The humane care and use of animals should be the prime concern and responsibility of all those individuals using animals for research, teaching or testing. The use of animals as experimental subjects in the 20th century has contributed to many important advances in scientific and medical knowledge. Although scientists have also developed non-animal models for research, these models often cannot completely mimic the complex human or animal body. Nevertheless, efforts to develop and use scientifically valid alternatives, adjuncts, and refinements to animal research should continue. Laboratory Animal Law that was prepared to improve the reliability and reproducibility of research data using laboratory animals also required an institutes to care and use animal in ways judged to be scientifically, technically, and humanely appropriate. The law introduced Assessment Program of Good Animal Care and Use Facility. The primary goal of the Assessment Program is to provide quality assurance for the care and use of experimental animals on a national basis. Although, an effective surveillance protects animals, promotes animal welfare and ensures the quality of science that uses animals, it is the responsibility of investigators to conduct animal experiment on the basis of the highest scientific, humane, and ethical principles. It is expected that the Program will accelerate the introduction of mind concerning humane and ethical animal experiment and will enhance animal well-being, the quality of biomedical research, and the advancement of biologic knowledge that is relevant to humans or animals.

10.Repair of Microform Cleft Lip with Minimal Incision.

Microform cleft lip is a mild form of incomplete cleft lip, also known as a minimal occult, abortive, forme fruste cleft lip. However, it has no definition and few methods have been reported for its correction. A microform cleft lip is characterized as the incomplete union of the superficial portion of the orbicularis oris muscle. It is more prominent during facial expression than in a resting state. We confined microform cleft lip in our study to the absence of philtral skin change and a contracted position on the top of cupid's bow. During the past 5 years, 17 patients of microform cleft lip were operated on. We corrected the defect of the upper vermilion border and nostril sill with minimal incision, and repaired the underlying lip musculature in superficial discontinuity. Reduction of the widened alar base was performed. Deformed alar cartilage was dissected via rim incision, and suspended in a medial and upper direction with pull-out sutures. The most important thing is precise repair of the superficial portion of the separated orbicularis oris muscle via minimal incision, and it is best to operate after 1-year of age for accurate repair. The results were satisfactory and the parents were also satisfied. The advantages of this procedure are as follows: 1. Less visible, minimal scar on upper lip 2. Simultaneous correction of vermillion notching, deformed cupid's bow and nasal deformity. 3. Eversion of philtral ridge due to tenting effect of horizontal mattress suture 4. Philtral elongation effect by reduction of alar base and Z-plasty of cupid's bow.
