BACKGROUND: The purpose of this study was to measure and compare the optimum depth of the internal jugular venous catheterization between the right and left side. METHODS: Forty-four patients were enrolled for this study and divided into two groups (22 patients each). The optimum depth of the catheterization was calculated using the sum of two component (A and B); the advanced length of the catheter from the level of the cricoid cartilage (A) and the distance from the catheter tip to the junction of the superier vena cava and right atrium (B). RESULT: The optimum depths of the internal jugular venous catheterization were 16.0 1.0 cm (right) and 18.4 1.5 cm (left) respectively. Left side was significantly longer than right side (p<0.05). In this study, we experienced some complications; arterial punctures (5 cases) and migration of the catheter to the opposite subclavian vein (1 case). Five complications were associated with left internal jugular venous cannulation and one was associated with the right side cannulation. CONCLUSION: We concluded that the optimum depth of the internal jugular venous catheterization was longer in the left side than in the right side.

In general, the long head of the biceps brachii originates from the superior glenoid labrum and the supraglenoid tubercle, crosses the rotator cuff interval, and extends into the bicipital groove. However, rare anatomic variations of the origins of the long head have been reported in the past. In this report, we review the clinical history, radiologic findings, and arthroscopic identifications of 3 anatomic variants of the biceps tendon long head. As the detection of long head of biceps tendon pathology during preoperative radiologic assessment can be difficult without prior knowledge, surgeons should be aware of such possible anatomic variations.

Adenomyosis is a common disease of middle-aged women and adenomyoma is a variety of adenomyosis that formed localized tumor. Cystic degeneration of an adenomyoma is a rare clinical manifestation. A 30-year-old parous woman suffered from severe dysmenorrhea and menorrhagia for about 5 months, was operated under the impression of endometriosis of the pelvis. Following the operation, cystic degeneration of an adenomyoma was found incidentally. The authors experienced a case of adenomyosis that formed cystic tumor of uterus and presented with a pertinent literatures.

OBJECTIVES: Schizophrenia manifests a variety of interindividual differences in therapeutic response to antipsychotics. This might be attributable to dopamine and serotonin receptors that a important target for various antipsychotics, and the D3 receptor(DRD3) alleles they carry. The purpose of our study was to investigate whether the plasma levels of homovanillic acid(HVA) and 5-hydroxyindoleacetic acid(HIAA), and the polymorphism of DRD3 can be held as a predictor of treatment response ni chronic schizophrenic patients. METHODS: Therapeutic response for 16 korean schizophrenia patient treated during 48 weeks were assessed by PANSS used as the clinical symptom rating scales. The levels of concentration of HVA and 5-HIAA were examined by HPLC at baseline and at 48 weeks. We classified the polymorphism of DRD3 receptor using amplifying by polymerase chain reaction(PCR). RESULTS: Neither concentrations of HVA and 5-HIAA nor genotype of dopamine 3 receptor were not significantly associated with the therapeutic response. But, the patients who has A1 alleles of DRD3 gene showed poor therapeutic responses. CONCLUSION: A1 allele of DRD3 gene is associated with poor prognosis of chronic schizophrenia.

PURPOSE: Immunotherapy with tumor cell vaccines has shown limited success in established tumors, for reasons including suppressed expression of MHC molecules or absent tumor associated antigens, and active production of immunosuppressive molecules of tumor cells. The development of a vaccine targeting tumor angiogenesis might overcome the limitation of tumor cell vaccines. MATERIALS AND METHODS: Human umbilical vein endothelial cells(HUVEC) were infected with 20MOI of Ad-CMV-mGMCSF, and GM-CSF expression was measured by ELISA assay. One million mouse bladder cancer cells (MBT-2) were subcutaneously inoculated in C3H mice. The growth of tumors was measured after weekly intraperitoneal injection of saline(group I), HUVEC cells(group II), and HUVEC cells infected with Ad-CMV- mGMCSF(group III). At 4 weeks, the tumors were immunostained with anti-CD31 and the microvessel density(MVD) was measured. To evaluate the mechanism of this vaccine, flow cytometry analysis for activated CD4 and CD8 was performed. RESULTS: The mean expression of GM-CSF was 215ng/106 HUVEC cells at 24 hours after infection. There was no significant difference in tumor growth between groups I and II. However, the tumor growth in group III was significantly suppressed compared to that in groups I and II(p<0.05), as was MVD(p<0.05). In flow cytometry analysis, activated CD4 and CD8 cells were increased in group III compared to those of groups I and II. CONCLUSIONS: GM-CSF producing endothelial cell vaccine suppressed the growth of established bladder tumors, which may be explained by specific T cell mediated immune reaction to the tumor angiogenesis.

In order to evaluate psychiatric symptoms in survivors of the Sampoong accident, we interviewed 624 survivors of the accident. The most common complaint of the subjects was sleep disturbance(54.2%). And then, headache(31.8%), irritability and anger(23.3%), intense distress over reminders(24.2%) followed. Common somatic complaints were headache, chest discomfort, extremity pain, backache, and gastrointestinal discomfort. This study revealed that most of the subjects had various psychiatric symptoms after the Sampoong accident.

Nutrition during childhood is essential for growth and maintenance of health. Good food habits developed during the childhood will contribute both to the healthy growth and the prevention of the degenerative disease of later life. Both parents and the providers in child care centers play an important role for children's good eating behavior. Therefore all child care programs should achieve recommended standards for meeting children's nutritional and educational needs in a safe, sanitary, and supportive environment to promote the healthy growth and development of children. The purposes of this study were to evaluate the foodservice management practices and assess the needs for a Central Production Unit by contacting the child care center' providers. This approach was achieved using a variety of qualitative and quantitative information including the general foodservice management practices and the needs for a Central Production Unit. An indepth face-to-face interview with structured-questionnaires was undertaken at 32 representative child-care centers in Seoul. Statistical data analysis was done using the SAS program for descriptive analysis and ANOVA. The number of national/public and private sectors were 11 respectively, followed by 10 licensed home day-care centers. Total average number of children in child-care centers was 54.3+/-48.5. The foodservice productivity index in child-care centers was 4.8 minutes per meal for public child care centers, 6.0 for private child-care centers, and 9.8 for home child care centers. Home child care centers were found to have the lowest productivity index which indicated inefficient foodservice practice. The important factors in group purchasing were menus(39.6%) or close distance(39.6%) > type of foodservice operation(32.8%) > total number of meals(19.9%) > food costs(16.2%) in order. Average score of the efficiency for central food production in child-care centers was 3.80+/-0.84 out of 5.
