1.Effect of Duloxetine in Functional Gastrointestinal Disorder : In the Perspective of 'Brain-Gut Axis'.

The pathophysiology of functional gastrointestinal disorder(FGID) is not completely understood, but the importance of the 'Brain-Gut Axis(BGA)' model in FGID is being increasingly recognized. The BGA model is a bidirectional, hard-wired and homeostatic relationship between the central nervous system(CNS) and the enteric nervous system(ENS) via neural, neurohormonal and neuroimmunological pathways. In addition, the BGA model would provide a rationale for the use of psychotropics on FGID. The authors experienced two cases in which duloxetine, a serotonin-norepinephrine reuptake inhibitor, was effective in relieving FGID symptoms as well as psychiatric symptoms such as depression and hypochondriacal anxiety. Therefore we discuss the vignettes from the perspective of BGA theory. Duloxetine showed efficacy in these two patients by reducing visceral hypersensivity (bottom-up regulation) and by relieving depression and anxiety(top-down regulation).

2.Guidelines for the Screening of Uterine Cervical Cancer.

Cancer has become a major cause of death in Korea since 1989. Despite the major advances in the treatment of cancer, prevention and early detection is the most effective strategy to decrease the mortality from cancer. The indicence of invasive cervical cancer has declined since the 1950s, and indirect evidence suggests that this decline is the result of the institution of screening with cervical smear. However, cervical cancer continues to be a major health problem in the developing world today, where screening is not universal. Among Korean women, the incidence of cervical cancer is still high and was the 3rd most common cancer following stomach and breast cancer in 2000. If preinvasive conditions would be included, the disease would be the most frequent. The first organized screening programmes were initiated in 1949 at British Columbia, Canada. Since then, cervical cancer screening programmes have been implemented in almost all developed countries. In Korea, opportunistic screening began in the late 1950s, and organized screening began in 1998 by Health Insurance without a consensus meeting. The guidelines for the screening of cervical cancer were developed by National Cancer Center and the Koean Society of Obstetrics and Gynecology in May, 2001 ; and the national screening programmes were developed by the Supporting Evaluation Board of National Cancer Screening Program in December, 2001. This article introduces these programmes.

3.Molecular Species Identification of Forensically Important Flies in Korea.

To estimate the postmortem interval (PMI) by using entomological evidence, species identification of forensically important flies is mandatory. However, the traditional species identification method, which relies on the key morphological features of adult flies, is not always available to investigators and has limitations to the immature samples. Because of these limitations, species identification using DNA sequences has long been an issue in the field of forensic entomology. In this review, I have briefly described the basic principles of molecular species identification and phylogenetic analysis and their applications in forensic entomology. I also recommend an experimental and statistical method to identify unknown fly samples obtained from the field.

6.A Case of Generalized Pustular Psoriasis in Infancy.

A case of familiar benign chronic pemphigus is reported. A 41 years old housewife has recurrent history of developing grouped vesicular eruptions with erythematous erosion on the intertrigenous area since 29 years of her age. The lesions were used to aggrevated in summer and improved in autumn. Familiar history revealed same disease of her grand mother, father, 44 year old elder sister and 39 year old younger sister. All of whom had similar clinical signs and symptoms. Routine physical and Iaboratoryfindings were within normal limits. The histopathological findings, from vesicular eruption of the lateral aspect of neck, showed multilocular suprabasal clefts with acantholsis, villi formation and hyperkeratosis. The lesions were improved hy the local application of corticostcroidoid ointrnent only.

10.The Effect of the Local Anesthetic Cream in Alleviating Pain form Vaccination.

To evaluate the effect of a new topical anesthetic cream (EMLA : Eutectic mixture of local anesthetics) on reducing pain associated with DPT vaccination, we conducted a clinical observation on eighty infants who were brought to well baby clinic of Yeungnam University Hospital for DPT vaccination. 80 Infants, who were between 2 months and 8 months in age, were divided into two groups. EMLA treated group and control group. Male to femle sex ratio was 1.4 to 1. EMLA cream was applied 60 minutes before DPT vaccination, the effect of reducing pain was assessed by using McGrath's face scale, Oucher pain scale and modified behavioral pain scale (MBPS) and also evaluated by measuring the duration and time of crying (the time of the first crying after iniection, duration of the first crying, total duration of crying). The scores of those scales were lower in EMLA treated group than in control group significantly (P<0.01 in McGrath's face scale. MBPS and p<0.05 in Oucher pain scale). There was no difference in the time of the first crying after vaccination in both groups. The duration of the first crying was shorter in EMLA treated group than control group of crying was also shorter in EMLA treated group (EMLA treated group 9.0±12.5 sec, p<0.05). Transient skin erythema was noted in 5 infants after EMLA application, but no other adverse effects were observed. We conclude that the application of EMLA cream before vaccination seems to be an effective and safe way to reduce the pain from vaccination, but it takes usually 60 minutes to get the anesthetic effect of EMLA and it is expensive, so EMLA cream can not be recommended in routine vaccination in infants now.
