3.Liver Cirrhosis: Etiological diagnosis and morphological characteristics of 369 biopsy-proven cases.

To pursue a desirable format for the pathological diagnosis of liver cirrhosis, the authors attempted to classify 369 biopsy-proven cirrhosis on the basis of etiology and made effort to find out the morphological characteristics of each category. About 735 of total cases were HBsAg seropositive postnecrotic cirrhosis. Alcholic cirrhosis ws the second most frequent type, although accounted only 6.8%. In about 15%, the etiology was not known. Excluding the congenital biliary atresia, chronic biliary obstruction appeared to be a rare cause of cirrhosis among these biopsied cases. Of the HBsAg positive postnecrotic cirrhosis, the eAg seropositive cases tended to be micronodular and to show a higher necroinflammatory activity, in contrast to eAg seronegative cases and those complicated by hepatocellular carcinoma (HCC), suggesting that the loss of eAg is followed by a decrease of the destructive activity, active regeneration of hepatocytes and finally the development of HCC. alcoholic cirrhosis was micronodular in 64% and revealed histologic evidences of alcoholic liver disease in most cases. The results indicate that etiological diagnosis can be made in most cases of cirrhosis by the morphological characteristics and the precise clinical informations, including those on the NANB virus and the inborn error of metabolism, and that the pathological diagnosis should be more comprehensive, implicating the etiology, the nodular size and the necroinflammatory activity.

4.Nationwide Systems for Nosocomial Infections Surveillance in the Countries Other than Korea.

Nosocomial infections surveillance is the basic activity of infection control programs, and many countries conduct surveillance using various methods. We convinced that each country should conduct an adequate and efficient surveillance system depending on their situations. Here we reviewed the recent nosocomial surveillance systems of other countries. As many countries have succeeded in reducing hospital infections, we expect that Korean Nosocomial Infections Surveillance System would contribute to nosocomial infection prevention and health promotion in Korea through an active surveillance at the national level.

6.Clinical Experience of Plasma Exchange.

BACKGROUND: Plasmapheresis or plasma exchange is the procedure to remove various pathogenic materials from the circulation of patients and retransfuse the formed elements into the doners with type specific fresh frozen plasma or albumin for the replacement of the withdrawn plasma. METHODS: In order to evaluate clinical experience of plasma exchange, we examined a total of 133 plasma exchange procedure on 25 patients of January 1993 - May 1994 in Kyungpook national university hospital. RESULTS: The results are as follows: 1. There were 8 kinds of diseases and the most frequent diseases was Guillian-Barre syndrome(l 2 cases) and the decreasing order was CIDP(6 cases), multiple myeloma(2 cases), acute fulminant hepatitis(1 case), mushroom poisoning(1 case), lupus nephritis(l case), TTP(I case) and myasthenia gravis(l case). 2. The patient's age ranged from 13 to 65 years. 3. There were 14 males and 11 females and the sex ratio was 1.3:1. 4. We had got clinical improvement in 16 of 25 cases, especially most dramatic effect in Guillian-Barre syndrome. CONCLUSION: To achieve more successful outcome of plasma exchange, we should select patients with restrict application, prevent rebound phenomenon with intense practice, choose what kind of replacement fluid to consider patient's clinical condition and check serological parameter with clinical evaluation to monitor effectiveness of treatment.

7.In silico Analysis of Downstream Target Genes of Transcription Factors.

OBJECTIVE: In the previous study, we compiled the differentially expressed genes during early folliculogenesis.1 Objective of the present study was to identify downstream target genes of transcription factors (TFs) using bioinformatics for selecting the target TFs among the gene lists for further functional analysis. MATERIALS AND METHODS: By using bioinformatics tools, constituent domains were identified from database searches using Gene Ontology, MGI, and Entrez Gene. Downstream target proteins/genes of each TF were identified from database searches using TF database (TRANSFAC(R) 6.0) and eukaryotic promoter database (EPD). RESULTS: DNA binding and trans-activation domains of all TFs listed previously were identified, and the list of downstream target proteins/genes was obtained from searche of TF database and promoter database. Based on the known function of identified downstream genes and the domains, 3 (HNF4, PPARg, and TBX2) out of 26 TFs were selected for further functional analysis. The genes of wee1-like protein kinase and p21WAF1 (cdk inhibitor) were identified as potential downstream target genes of HNF4 and TBX2, respectively. PPARg, through protein-protein interaction with other protein partners, acts as a transcription regulator of genes of EGFR, p21WAF1, cycD1, p53, and VEGF. Among the selected 3 TFs, further study is in progress for HNF4 and TBX2, since wee1-like protein kinase and cdk inhibitor may involved in regulating maturation promoting factor (MPF) activity during early folliculogenesis. CONCLUSIONS: Approach used in the present study, in silico analysis of downstream target genes, was useful for analyzing list of TFs obtained from high-throughput cDNA microarray study. To verify its binding and functions of the selected TFs in early folliculogenesis, EMSA and further relevant characterizations are under investigation.

8.Histopathologic Findings & Expression of bcl-2 of the Endometrium Analysis of 1,000 consecutive biopsies of uterine bleeding .

We evaluated 1,000 consecutive endometrial curettage samples obtained over a 30 month period. The clinico-pathologic correlation was analysed according to Hendrickson's five criteria based on the practical view. The causes of uterine bleeding in decreasing order of occurrence were as follows: 1) hormonal imbalance lesions (49.2%) encompassing glandular and stromal breakdown suggesting anovulatory bleeding, proliferative phase endometrium, and disordered proliferative endometrium, 2) pregnancy associated lesions (24.2%), 3) organic lesions (13.5%), 4) endometrial hyperplasia (6.9%), and 5) inadequate specimen (6.2%). According to age, pregnancy related lesions were most frequent in the third decade. In the fourth, fifth, and sixth decades, hormonal imbalance lesions were the most common cause. In approximately 30% of the samples, there were two or three morphologic patterns such as anovulatory bleeding with an endometrial polyp, postabortal bleeding with inflammation, and glandular-stromal dissociation with a polyp, which suggested there was a variable histologic morphology in the same disease spectrum. Using immunohistochemical techniques we studied the hormonal dependency of bcl-2 oncoprotein in anovulatory bleeding, endometrial hyperplasia, and proliferative endometrium. 70% of anovulatory bleeding specimens showed weak positivity in the epithelial cytoplasm, and all cases of endometrial hyperplasia and carcinoma showed a strong positivity. These results suggest that there is a estrogenic hormonal dependency of apoptosis in the endometrium.
