3.A Clinical Study of the Unstable Pelvic Fractures involving Sacro-iliac Joint

The unstable pelvic fracture involving sacro-iliac joint produced by excessive force, is relatively rare but causes a multiplicity of complications and presents a major therapeutic problem. The clinical results were obtained as follows: 1. 9 patients out of 16 were in age group between 20 to 29 year, Male and female ratio was 5:3 2. 10 patients out of 16 were produced by traffic accident. 7 patients among 16 were included in vertical shear type and 5 in lateral compression type. 3. There were 44 associated injuries in 15 patients. Fractures in other region were the most common associated injury in 20 and injuries of the genito-uninary system in 8. 4. All of the cases were treated conservatively and their result were excellent or good except 1 patient in pelvic disruption type, who has had paraplegia due to fracture-dislocation of L4-5.

4.Treatment of Fracture of Shaft of Humerus by Ender Nailing

Authors presented the experience of Ender nail treatment for the humeral shaft fractures in 24 patients. The results of treatment in all were satisfactory, though there were four cases of delayed union. Bridging callus was formed at 8.2 weeks on an average. The average clinical union time was 14 weeks. No complications developed during nailing procedure. A case of radial nerve palsy after fracture recovered spontaneously. Longer nails which hit the end of medullary canal of distal fragment distracted the fracture gap, and resulted in delayed union in 4 cases. Therefore, it is recommended to use the proper size of nail to avoid the distraction effect of the inserted nail, and to use two nails at minimum for better fixation. However, when intramedullary Ender nailing is properly done, single nailing also can give consistently good anatomic and functional results.

The absence of palmaris longus musle is common, about 10% in general population, but absence of another muscle(S) in forearm and/or hand is very rare. We experienced a case of conenital absence of flexor digitorum profundus in both hands. It was confirmed by surgical exploration. This anomaly seems to be extremely rare case in the English literature.

7.A Case of Multiple Right Atrial Myxomas with Pulmonary Embolism.

Primary tumors of the heart are rare. Half of all cardiac tumors are myxoma of which 20% are located in the right atrium. Especially multiple myxomas in the cardiac chamber were rarely reported. Clinically, myxomas present with various manifestations due to obstruction to blood flow, embolization,and constitutional changes. The association of pulmonary emboli and right atrial myxoma has been well documented but pulmonary emboli are more likely to be asymptomatic, misdiagnosed, or diagnosed late than peripherally-sited emboli. We report a case of multiple right atrial myxomas complicating pulmonary emboli in a 73-year-old man who had features of pulmonary embolism and was found to have multiple right atrial myxomas.

8.Clinical Outcomes of Arthroscopic Treatment of Calcific Tendinitis of the Shoulder.

BACKGROUND: Our study aimed to make a comparative analysis of clinical outcomes of arthroscopic decompression for rotator cuff calcific tendinitis by location of calcific deposits and by its size. METHODS: We enrolled a total of 38 patients, comprising 39 affected shoulders, who underwent arthroscopic decompression for calcific tendinitis. As our clinical scores, we evaluated the UCLA, the ASES, and the VAS scores and analyzed them by calcific location, by calcific deposit size, by the presence or absence of calcific remnants, and by whether concomitant cuff repair was performed. RESULTS: The clinical scores of those whose calcific deposit had an area greater than 77.0 mm2 and of those whose calcific deposit had an area smaller than 77.0 mm2 did not significantly differ (p=0.21 in ASES; p=0.19 in UCLA; p=0.17 in VAS). Nor did the clinical scores significantly differ with respect to the location of calcification (p=0.23). Further, the clinical scores did not significantly differ between those who had calcific remnants and those who did not and between those who received additional cuff repair and those who did not. CONCLUSIONS: We found that the clinical outcomes after arthroscopic decompression of calcific tendinitis were not significantly associated with the cuff tendon in which the calcium deposits are found; the location of the calcium deposits in the supraspinatus tendon (if found in this tendon); the size of calcific deposits; the presence of calcific remnants; and concomitant cuff repairs.

9.Solitary Cutaneous Myxoma.

A solitary cutaneous myxoma, referred to as superficial angiomyxomas, is relatively uncommon benign myxoid soft tissue tumors. The lesion usually present on the head, neck, and trunk of adults as a slowly growing asymptomatic nodule. Herein, we describe a case of a pedunculated solitary cutaneous myxoma arising on the vertex of a 48-year-old man, but with no evidence of Carney syndrome. Histologically, the tumor was composed of vascular, mucinous matrix with a network of fine collagen and reticulin fibers, containing stellate and spindle-shaped fibroblasts. 10.Assessment of primary care level in Korea and comparison with the developed countries.

BACKGROUND: Due to the limited medical resources, effectiveness, efficiency, and equity has become the main issue of health care system reform in the developed countries, which has lead to the focusing on strengthening of primary care. The study was performed to assess the primary care level and health indicators in Korea utilizing objective criteria used in other developed countries, so as to evaluate its current status compared to the developed countries. METHODS: Starfield's primary care scoring criteria of the system and practice characteristics was used to evaluate the primary care level in Korea and 13 members of the OECD countries. According to the primary care score, the countries were grouped in to 3 groups(highest, middle, lowest) and were compared the health indicators, health care expenditure, and satisfaction with health care system. RESULTS: Korea ranked the lowest in both primary care level and health indicators. The amount of health care expenditure and satisfaction with health care system were also low. Health indicators were worst in the lowest primary score group. But unexpectedly, middle group had better health indicators than highest group in most items. CONCLUSION: Primary care level in Korea is very low compared to the developed countries. Middle primary care level countries having better health indicators may be due to the overemphasis of efficiency and equal distribution in highest level countries leading to low quality service. Korea should consider modelling the middle group countries in the course of establishing primary care system with equal balancing in quantity and quality.
