1.G-Score and the Percentage of Pseudo-Pelgeroid Polymorphoneutrophils in Myelodysplastic Syndrome.

BACKGROUND: Myelodysplastic syndrome (MDS) is characterized by signs of maturation disturbances in the both of peripheral blood (PB) and bone marrow (BM). Every hematologist who cares for patients with MDS will occasionally encounter difficulties in establishing the diagnosis, particularly in refractory anemia. Dysplastic features of cells in peripheral blood and bone marrow were studied to evaluate the significance of the degree of neutrophil hypogranulation (G-score) and the percentage of pseudo-pelgeroid polymorphs (PPP) in PB smear. METHODS: Cytoplasmic hypogranulation and nuclear abnormalites of the pelgeroid type of neutrophils were studied retrospectively on air dried peripheral blood and bone marrow smears stained with Wright reagent, and assayed according to Hast's method. In every cases, 100 neutrophils were analyzed as follows: the degree of hypogranulation was graded in arbiturary unit with the range 0 to 3. Neutrophils containing a single or bilobated nucleus with dense compact chromatin were counted as the pelgeroid type. Common morphological features of MDS were observed on bone marrow smears. A numetric level between 0 and 2 was assyed for two dysplastic features of the three hemoatopoiesis lineages. The score for total degree of bone marrow dysplasia (DysT) ranged between 0 and 12. RESULTS: There was a good correlation between PB and BM findings, of both C-score (r=0.5503, p<0.01) and PPP (r=0.8701, p<0.001). Patients with a high degree of bone marrow dysplasia had significantly higher PPP (r=0.5853, p<0.005) than those with less pronounced myelodysplasia. Patients with refractory anemia with excess blast (RAEB)/refractory anemia with excess blast in transformation (RAEB-T)/chronic myelomonocytic leukemia (CMML) had higher PPP, but lower G-score than those with refractory anemia (RA)/refractory anemia with ringed sideroblasts (RARS). CONCLUSIONS: The above results show that quantitative estimation of peripheral blood polymorph dysplasia by G-score and PPP seems to reflect the total degree of bone marrow dysplasia in MDS and may serve as a helpful method to bone marrow evaluation when the diagnosis is difficult.

3.Effect of the inhibition of PLA2 and PAF on the neutrophilic respiratory burst and apoptosis.

BACKGROUND: Since the exact pathogenesis of sepsis-induced ARDS has not been elucidated, the mechanisms of enhanced neutrophilic respiratory burst were probed in endotoxin primed neutrophils associated with the roles of phospholipase A2 (PLA2), platelet activating factor (PAF) and apoptosis. METHODS: In isolated fresh human neutrophils, effects of the inhibition of PLA2 and PAF on the apoptosis were examined by the method of Annexin-FITC/dual PI flow cytometry. The roles of PLA2 and PAF on the neutrophilic respiratory burst were also examined by measuring oxidant generation in cytochrome-c reduction assay. Activities of the PLA2 and lysoPAF acetyltransferase (lysoPAF AT) of the neutrophils were determined to understand the effect of endotoxin on these enzymatic activities which may be related to the neutrophilic respiratory burst and apoptosis. In addition, the role roles of PLA2 and PAF in neutrophilic adhesion to bovine endothelial cells were examined in vitro by neutrophil adhesion assay. To investigate the effect of oxidants on pulmonary surfactant, cytochemical ultrastructural microscopy was performed. To inhibit PLA2 and PAF, non-specific PLA2 inhibitor mepacrine (100 nM) and WEB 2086 (100 nM) or ketotifen fumarate (10 kg/ml) were used respectively in all in vitro experimental sets. WEB 2086 is PAF receptor antagonist, and ketotifen fumarate is a lyso PAF AT inhibitor. RESULTS: The mapacrine treatment, provided after the endotoxin (ETX) treatment, resulted in increased apoptosis of neutrophils (p<0.001) while treatments of WEB 2086 and ketotifen did not. The inhibition of PLA2 and PAF decreased (p<0.001) production of oxidants from PMA-stimulated neutrophils. While endotoxin increased the PLA2 activity of neutrophils (p<0.01), mepacrine supressed (p<0.001) the activity, provided after treatment of ETX. The lyso PAF actyltransferase activity (lyso PAF AT) increased(p<0.01) after treatment of ETX. In contrast, mepacrine, WEB 2086 and ketotifen showed a tendency of decreasing the activity after treatment of ETX. The Treatment of ETX increased (p<0.001) neutrophil adhesion to endothelial cells, which was reversed by inhibition of PLA2 and PAF (p<0.001). The binding of oxidants to pulmonary surfactant was identified histologically. CONCLUSIONS: The enhanced neutrophilic respiratory burst by ETX plays a pivotal role in the pathogenesis of ARDS in term of oxidayive oxidative stress. Increased production of oxidants form neutrophils is mediated by the activations of PLA2 and lyso PAF AT.

4.Effect of Cytokines on Expression of Intercellular Adhesion Molecule - 1 in Nasal Epithelial Cells.

Intexcellular adhesion molecule-10CAM- 1) is an important moleade in aehating immune and inflammatory responses. It is found on the surface of hematopoietic and nonhematopoietic cells. It can act as an adhesive ligand for integrins such as LFA-1 (CD1&/CD11a) and MAC-1 (CD18/ CD11b). ICAM-1 is basally expressed in sigaificant amount on a limited number of cell types, including monocytes and endothelial ceRs. But it is inducible or upregulated by INF-r, IL-1p and TNF-a on many cell types. IL-4, a pleiotropic cytokine and mast cell differentiation factor, is upregulated in human allergic disease and stimulates expression of vascular adhesion molecule-1 (VCAM-1) in endothelial cells. IL-4 also promotes expression of surhce ICAM-1 in human mast cells and dermal fidroblasts. So in allergic rhinitis and asthma, IL-4 may be an important cytokine implicated in the pathogenesis of inflammation. We studied the effect of INF-r and IL-4 on expression of ICAM-1 in human nasal epithelial cells (HNEC). HNEC were prepared by primary culture method of monolayer culture of dissociated cells hom human inferior nasal turbinate mucosa. Nasal mucosa were obtained by partial turbinectomy of septal deviation patients. Primary cultured cells were charaterized as an epithelial cell type by indirect immuno-fluorescence assay using antilmdies against cytokeratin-pan, cytokeratin No. 8, vimentin and von Willebrand factor. Using fluorescence activated cell sorter (Coulter EI1TE), we analyzed the quantitative expression of ICAM-1 on HNEC. Treatment of HNEC with IFN-r (1ng/ml) for 24 hours caused about 8 fold increase of the surface ICAM-1 compared with constitutive expression by mean fluorescence intensity (MIF) but IL-4 had little effect. Theses foundings suggest that IFN-r is a potent ICAM-1 inducer in HNEC and further studies are necessary for the role of IL-4 on HNEC.

5.Effectiveness of an Interpersonal Relationship Program on Interpersonal Relationships, Self-esteem, and Depression in Nursing Students.

PURPOSE: The purpose of this study was to examine the effectiveness of an interpersonal relationship program on interpersonal relationships, self-esteem, and depression in nursing students. METHODS: This was a quasi-experiment with a nonequivalent control group pre-posttest design. Sixty-four nursing students participated in the study with 31 in the experimental group and 33 in the control group. They were from 3 different colleges of nursing located in Seoul. The interpersonal relationship program was held 10 times over 10 weeks, taking 90 minutes per session. The interpersonal relationship change scale developed by Schlein and Guemey, Rosenberg's self-esteem scale, and CED-S for depression were the instruments used in the study. The data collection period was from January 4 to March 8, 2011, and the collected data were analyzed with SPSS 14.0 using the X2-test, t-test, and paired t-test. RESULTS: The results showed a significant difference between the experimental group and the control group in terms of the degree of interpersonal relationships, self-esteem, and depression. CONCLUSION: The results indicate that interpersonal relationship programs have positive effects for improving interpersonal relationships and self-esteem, and decreasing depression in nursing students.

10.The Effect of a Health Promotion Program through Multi-level Health Promotion.

PURPOSE: This study was carried out to investigate the effects of improvement in self-rated health, self-efficacy, perceived benefit and health promotion behavior by running a health promotion program through the coalition of industries, universities and districts. METHODS: This study was designed as non-equivalent control group research. Data were collected from 62 participants in a health promotion program who were enrolled in a community center (experimental group: 29, control group: 33). The program was applied from October to November, 2008. The health promotion program was composed of value, competence, action, and policy based on a multi-level health promotion model. Collected data were analyzed through chi2 test, t-test, and Wilcoxon test. RESULTS: After participating in the health promotion program, the experiment group showed statistically significant increases in self-efficacy, perceived benefit and health promotion behavior but not in self-rated health. CONCLUSION: It was proven that the health promotion program enhanced the health promotion level in the community.
