1.Detection of canine distemper virus (CDV) through one step RT-PCR combined with nested PCR.

A one step reverse transcription PCR (RT-PCR) combined nested PCR was set up to increase efficiency in the diagnosis of canine distemper virus (CDV) infection after developement of nested PCR. Two PCR primer sets were designed based on the sequence of nucleocapsid gene of CDV Onderstepoort strain. One-step RT-PCR with the outer primer pair was revealed to detect 10(2) PFU/ml. The sensitivity was increased hundredfold using the one-step RT-PCR combined with the nested PCR. Specificity of the PCR was also confirmed using other related canine virus and peripheral blood mononuclear cells (PBMC) and body secretes of healthy dogs. Of the 51 blood samples from dogs clinically suspected of CD, 45 samples were revealed as positive by one-step RT-PCR combined with nested PCR. However, only 15 samples were identified as positive with a single one step RT-PCR. Therefore approximately 60% increase in the efficiency of the diagnosis was observed by the combined method. These results suggested that one step RT-PCR combined with nested PCR could be a sensitive, specific, and practical method for diagnosis of CDV infection.

3.Chondroid Syringoma: A report of two case.

Chondroid syringoma is a rare primary skin tumor arising from eccrine sweat gland. In 1982, Nasse had found a primary skin tumor having similar morphology as in the mixed tumor of the salivary glands. In 1961, Hirsch and Helwig proposed more descriptive diagnostic term, "Choindroid syringma". The authors experienced two cases of chondroid syringma. The first case was a 47 year old male patient who had had a subcutaneous nodule in the right forehead for two months. It was a well circumscribed mass, that showed tan gray myxoid cut surface with firm consistency. Microscopically, it was a wel diagnosed as chondroid syringoma with tubular, branching lumina. The second case was a 51 year old female patient who had had a subcutaneous nodule in the left forehead for 3 months. It was well circumscribed, ad easily shelled out. It had a typical microscopic features of chondroid syringoma with small, tubular lumina.

6.A 14 Cases of Recurrent Shoulder Dislocation Treated by Modified Bristow Operation

A recurrent shoulder joint dislocation was performed on fourteen cases who were treated by modified Bristow operation from June, 1978 to April, 1981. and following results were obtained. 1. The age of initial dislocation varied from 13 years to 23 years of age with the average of 18. 6 years. 2. The age at operation varied from 20 years to 28 years of age with the average of 23 years. 3. The duration of recurrent dislocaticn varied from 2 years to 8 years with the average of 4.4 years. 4. The times of dislocation varied from 6 to 50 times with the average of 17.1 times. 5. The roentgenograms showed the Hill-Sachs' Iesion in 10cases (71.4%), subluxation in 4 cases (28.6%), and glenoid rim abnormality in 2 cases. 6. The operative findings were Bankart's lesion in II cases(78.6%), glenoid rim erosion in 2 case, and subscapularis laxity in 2 case. 7. The complication and recurrence were not observed in this period. 8. The average limitation of abduction and external rotation were 5. 7 and 12. 2 degree respectively.

7.A Study on the Changes of the Femoral Head following Treatment of Congenital Dislocation of the Hip

Although the incidence of avascular necrosis of the pmximal end of the femur has declined by early diagnosis and gentle dynamic reduction, it is still one of the major complication in treatment of congenital dislocation of the hip. From 65 cases, followed up at least one year after reduction at the Department of Orthopaedic Surgery, Busan National University Hospital from January, 1980 to June, 1984, the authom studied the factom causing avascular necrosis of the femoral head and its radiographic classification. The following results were obtained. 1. Based on Salter's criteria, avascular necrosis of the femoral head was considered in 16 of the 65 hips, a rate of 24.6%. 2. Based on the classification of Bucholz and Ogden, radiographic patterns of avascular necrosis were: type 1, 6(37.5%); type 2,3(18.8%); type 3, 5(31.3%); type 4, 2(12.5%). 3. Generally, the older the child when treatment was initiated, the more likely was avascular necrosis to develop, and the more severe was its damage. 4. The incidences of avascular necrosis in various treatments were: Pavlik harness, 10%; manipulative reduction, 17%; medial approach open reduction 40%; Salters innominate osteotomy only, 36%; femoral osteotomy with pelvic osteotomy, 20%. The high incidence in medial approach open reduction and Salters innominate osteotomy may be due to failure of previous manipulative reduction. 5. The preliminary traction of 14 days or more substantially reduced the incidence of avascular necrosis. 6. There was particularly high incidence of avascular necrosis in cast immobilization of Lorenz position. 7. The correlation between adductor tenotomy and avascular necrosis was not noticed. There was no avascular necrosis in the normal contralateral hip after cast immobilization.

8.Clinical Characteristics of Asthmatic Patients Who Visited Emergency Room.

BACKGROUND: Despite remarkable progress of understanding the pathophysiology and therapy of bronchial asthma, asthma morbidity and mortality are on the rise. Also hospitalization and attending rates of emergency department for asthma have been increasing gradually. We analyzed clinical characteristics and prognosis of patients who visited emergency room due to asthma attack in order to define clinical characteristics of these group of patients. METHOD: We reviewed 105 adult asthmatic patients who attended emergency department of Korea University Hospital between August 1995 and July 1996, retrospectively. RESULTS: 103 patients(56 female, 47 males, mean age : 48.6 years) attended-68 self referral, 18 practitioner referral and 17 OPD transfer- and 86 patients(83.5%) were admitted. Attending emergncy department was clearly more frequent in December(13.6%) and May(12.6%). Time lag between onset of asthmatic attack and arrival at the hospital was 14.2 15.5 hour and initial peak expiratory flow rate was 166.7 68.3L/min.(43.3% predicted) The commonest cause for visiting emergency room was aggravation of asthma due to upper respiratory tract infection in mild asthmatics. About half of them had history of previous ER visits. Their prognosis was not bad, but after discharge, about half of patients escaped from OPD follow-up. CONCLUSION: As a group they merit detailed attention and follow up arrangement. Clinician need to monitor and review the treatment plans, the medications, the patient's management technique, and the level of asthma control. For this group, plans for longer term treatment, including asthma education program and adjustment of overall treatment plan should be made.

9.Clinical Study fo Pulmonary Thromboembolism.

BACKGROUND: Pulmonary thromboembolism is relatively frequent and potentially fatal. However, it is commonly misdiagnosed. The incidence of pulmonary thromboembolism is not decreasing despite advances in diagnosis and effective prophylatic measures. Its potential for significant sequela necessitates a prompt diagnosis and treatment. Unfortunately, there are many difficulties and problems regarding accurate diagnosis. There is a low prevalence of deep vein thrombosis and pulmonary thromboembolism in Korea and only few reports on this subject are available. METHOD: The clinical features of 36 patients, who were diagnosed with pulmonary thromboembolism at the Korea University medical center, were reviewed. RESULTS: 1) There was no significant difference in prevalence between men an women, and the mean age was 50.9 years in men 59.2 years in women. 2) The frequent causes of pulmonary thromboembolism were malignancies (22.2%), surgery (22.2%), and heart disease(8.2%). Specific causes were not identified in 33.3%. 3) The most common symptom was dyspnea(72.2%), and the most common sign was tachypnea(61.1%). 4) The EKG findings were normal in 28.6%, and S1Q3T3 pulmonale pattern in 25.7%, ST or QRS changes in others. 5) The chest X-ray findings indicated pulmonary infiltation in 37.5%, cardiomegaly in 15.6%, pleural effusion in 12.5%, and normal in 27.8%. The perfusion lung scan showed a high probability in 66.7%, and intermediate or low probability in 33.3%. 6) The pulmonary arterial pressure(PAP) in the high probability groups was 57.9mmHg with a higher mortality rate(35%). CONCLUSION: Pulmonary thromboembolism is not uncommon in Korea and its clinical features do not differ greatly from those reported in the literature. When pulmonary thromboemblism of unknown causes are diagnosed, a search for an occult malignancy is recommended. Rapid diagnosis and treatment are achieved when thromboemblism is suspected.

10.A Study of Serum Lipid Levels in Normal Subjects and Various Diseases.

Total cholesterol and triglyceride were measured in sera of 40 cases of hypertension, 22 cases of myocardial infarction, 14 cases of cerebral thrombosis, 18 cases of subarachnoidal hemorrhage and 30 cases of normal control in Chungnam national University Hospital from October 1980 to September 1981, and the results are as follows; 1) The mean serum total cholesterol and triglyceride levels in normal control group are 153.57+/-40.60mg% and 150.82+/-51.76mg%. 2) The age incidence of these diseases were highest in 6th decade. 3) Serum cholesterol levels of myocardial infarction, hypertension, cerebral thrombosis and subarachinoidal hemorrhage group were higher than that of normal control group. 4) Serum triglyceride levels of acute myocardial infarction and hypertension were significantly higher than that of normal control group, but there is only slightly increases in cerebral thrombosis and subarachinoidal hemorrhage. 5) In general, serum lipid levels are higher in cardiovascular diseases(hypertension and myocardial infarction) than cerebrovascular diseases(cerebral thrombosis and subarachnoidal hemorrhage). 6) Serum Cholesterol and triglyceride levels decreased by administration of pancreatic mucopolysacharide for 1 or 2 months. In conclusion, it seems that high serum cholesterol and triglyceride level may play and important risk factor in development of hypertension and myocardial infarction.
