5.Compositional difference of middle ear effusion according to viscosity of the effusion.

BACKGROUND AND OBJECTIVES: Serous otitis media (SOM) is usually responsive to medical treatment, whereas mucoid otitis media (MOM) is not. This study was undertaken to elucidate the compositional difference between serous and mucoid effusion, and to investigate whether MUC5AC acts as a major mucin in the middle ear mucosa with MOM. MATERIALS AND METHODS: Middle ear effusion (MEE) samples were collected from 29 patients with MOM and 20 patients with SOM. The level of mucin, lysozyme, secretory IgA and interleukin-8 were measured by dot blotting or enzyme-linked immunosorbent assay. Periodic acid-Schiff and immunohistochemical staining with monoclonal anti-MUC5AC antibody were performed on the serial sections of middle ear mucosa with MOM. RESULTS: Mucoid effusions contained higher levels of mucin, lysozyme, secretory IgA and interleukin-8 than serous effusion. Immunohistological study revealed that MUC5AC mucin was expressed in only a small portion of the goblet cells of middle ear mucosa with MOM. CONCLUSION: Our study suggests that both serous secretions and mucin might make the middle ear effusion more viscous, and that mucins other than MUC5AC might have a major role in the viscosity of MEE. Further study is necessary to identify the major mucins in the MEE of otitis media with effusion.

6.Reliability and Validity of Korean-Version of Information Needs in Cardiac Rehabilitation Scale

Purpose:
The Cardiac Rehabilitation scale (INCR scale) is a verified measure worldwide. The scale measures the extent to which cardiac rehabilitation meets the various needs of the participating patients and determines how education related to heart disease and rehabilitation satisfies their preferences. The current study applied the INCR scale to verify and develop it for use in Korea by conforming to Korean culture and language.
Methods:
The Korean version of the INCR scale was created after a six-step process in accordance with the international questionnaire translation law. The Korean version of the INCR scale was assessed in 45 cardiac rehabilitation patients (29 males, 16 females). Internal consistency was verified using Cronbach’s alpha. Test-re-test reliability and interrater reliability were verified using the intraclass correlation coefficient (ICC). Content validity was selected to verify the validity.
Results:
The content validity index of the Korean version of the INCR scale was high, with an average of 0.93. Cronbach’s Alpha for Internal consistency also revealed high reliability of 0.89 in all 10 categories. The interclass correlation coefficient of the test-re-test and interrater reliability scores were 0.95 and 0.81, respectively.
Conclusion
Our results indicate that the Korean version of the INCR scale would be instrumental in the clinical setting to determine the knowledge, educational needs, and preferences of patients participating in cardiovascular disease and cardiac rehabilitation.
7.Endoscopic Clip Ligation on Mucosal Defect after Endoscopic Mucosal Resection.

The main complications associated endoscopic mucosal resection are bleeding and perforation and the rate of complication is increasing in larger size of mucosal resection. We used a new method of endoscopic clipping technique for ligation on the large mucosal defect after endoscopic mucosal resection. A 53-year-old female patient visited our hospital because of epigastric pain. Endoscopic examination revealed a type IIa EGC(early gastric cancer) below angle. The endoscopic mucosal resection was performed and the size of resected specimen was 2.7x2.5cm. The mucosal defect was ligated with 6 hemoclips(Olympus, MD-850) after approximation of both resected margins by grasping forceps(Olympus, FG-4L). Follow up endoscopic examination revealed good quality of ulcer healing. The Endoscopic clipping technique might also be useful for treatment of minor perforation.

Capsule endoscopy (CE) is a valuable modality for directly examining the small bowel in a relatively noninvasive and safe manner. CE is being increasingly used for obscure gastrointestinal bleeding, Crohn's disease, drug induced ulcer and small bowel tumor. Although capsule retention is a relatively infrequent complication, small bowel obstruction and strictures have been considered contraindications to CE. But some authors have reported that capsule endoscopy can be safely used to help identify the etiology and site of small bowel obstruction, and the retention of the capsule indicates the presence of a lesion requiring surgery. We report here on a case of small bowel obstruction and capsule retention by a small bowel ulcer, and the small bowel ulcer was not found when performing capsule endoscopy.
