1.A Clinical and Statistical Evaluation of the Teenage Pregnancy.

OBJECT: In order to compare several Obstetric characteristics and outmmes of teenage pregnancy. METHODS: The 10 year clinical records of 11,583 women undergoing Cesarean section and delivery were reviewed in the department of Obstetrics and Gynecology, Incheon Christian Hospital from July 1, 1989 to June 30, 1999. There were seventy-eight pregnancies under age 20 and we compared them with two hundred pregnancies aged 20-29 who were randomly chosen during the same periods as the control group. RESULT: The following results were obtained. The proportion of birth by teenagers increased from 0.5% of all birth in 1989 to 2.61% in 1999. The most of cases were in high teen group, age 14- 19.The majority(50.5%) were in the age of 19. Seventy two cases(92.3%) had no previous pregnant history. Six cases had one previous pregnant history. The frequencies of prenatal care were significantly lower than that of the control group(p<0.05). The mean gestational period was 36+4weeks, and showed significant difference between the study group and the control group(39+3 weeks). The incidence of preterm labor was significantly different between the two proups(p<0.05). But the incidence of postterm delivery were not significantly different between the two groups. The mean weight gain of pregnant teenagers revealed 10.64kg, significantly less profound than the control group. The matemal weight gain of control group revealed 12.95kg, respectively. The mean birth weight(2.54kg) of infant was significantly less profound than the control group. The incidence of birth of small for gestational age(birth weight 1,500-2,499gm ; 14.5%) was significantly increased among study group than the control group(1%). The rate of abdominal delivery(21.8%) were not significantly decreased than the control group(47%). The incidence of cephalopelvic disproportion and malpresentation for cesarean section were somewhat higher than the control group, and were statistically significant(p<0.05). During the antepartun period, anemia, pyelonephritis, and preterm labor occurred more frequently in the teenage pregnancy than the control group, but not statistically significant. Among postpartum complications, incidence of anemia(51.3%) increased significantly. The perinatal mortality, intrauterine growth retardation, congenital anomaly occurred more frequently than the control group. CONCLUSION: These results show that pregnancy of teenage will be comeout high risk and complications. And then we will be more concern and study about teenage pregnancy.

3.Correlations among Perceptions and Practice of Taegyo and Maternal-Fetal Attachment in Pregnant Women

PURPOSE: This study was conducted to examine the relationships among the perceptions and practice of taegyo (a traditional set of practices and beliefs related to healthy fetal development) and maternal-fetal attachment in pregnant women.METHODS: The participants were 136 pregnant women who visited a public health center or maternity hospital for prenatal care. The collected data were analyzed using descriptive statistics, the t-test, analysis of varience, and Pearson correlation coefficients with SPSS version 22.0.RESULTS: The mean age of the pregnant women was 32.24±3.99 years. The mean scores for perceptions of taegyo, practice of taegyo, and maternal-fetal attachment were 3.96±0.53, 3.74±0.64, and 3.94±0.49, respectively. Perceptions of taegyo were significantly correlated with the practice of taegyo (r=.72, p<.001), and maternal-fetal attachment (r=.55, p<.001). A significant correlation was also found between the practice of taegyo and maternal-fetal attachment (r=.65, p<.001).CONCLUSION: Perceptions of taegyo affected the practice of taegyo, and had a positive effect on maternal-fetal attachment. These findings suggest that primary care nurses at hospitals and public health centers should provide nursing intervention programs to improve the perceptions of taegyo, the practice of taegyo, and maternal-fetal attachment.

4.A case of congenital hypertrophic pyloric stenosis in two siblings.

Congenital hypertrophic pyloric stenosis is the most common intra-abdominal disease required surgery during the first few months of life. The expression of pyloric stenosis is dependent upon the genetic influence of ancestors affected with the disease, as well as unknown environmental influences in the postnatal period. Pyloric stenosis has been reported in multiple sibs in a family, which suggests the genetic influence on the expression of this disease. Until now, the genetic influence is thought a sex-modified polygenic or multifactorial background which facilitates the expression of a common dominant gene. We experienced a case of congenital hypertrophic pyloric stenosis in a two siblings. The siblings suffered projectile vomiting for 2~4 days at 16 days old of age and 15 days old of age. After we confirmed the diagnosis by upper gastrointestinal series and abdominal sonogram, the Fredet-Ramstedt pyloromyotomy was done successfully. This case suggests the genetic influence on the expression of this disease.

5.A Case of Left Main Coronary Artery Disease Treated with Medication.

We report a case of 34-year-old woman who had left main coronary artery disease and treated with medication only. She was presented with severe chest pain. Isolated left main coronary disease was demonstrated on selective left coronary angiography. There was no regional wall motion abnormality on echocardiogram with normal global LV systolic function. We considered the lesion as a varient of Takayasu's arteritis. So with anti-anginal medication, treatment started with steroid therapy. After then there was no chest pain. We performed follow-up coronary angiography after 1 month and 3 month in which revealed improved left main lesion. Since then there was no subjective symptom and Thallium-201 SPECT revealed no significant ischemic finding. We think that this case's underlying disease is may be Takayasu's arteritis which has isolated left main stenosis without systemic involvement and improved with medical treatement only.

7.Primary squamous cell carcinoma of thyroid gland with local recurrence: ultrasonographic and computed tomographic findings.

Primary squamous cell carcinoma of the thyroid gland (PSCCT) is a rare malignancy that presents with advanced disease and poor prognosis. It is difficult to diagnose PSCCT in its early stage because of its rarity and lack of typical imaging findings. We experienced an elderly woman with PSCCT confirmed by surgery. Although preoperative fine-needle aspiration revealed no malignancy, surgical resection was performed because the ultrasonogram showed diffuse microcalcifications, which suggested malignancy, and clinically, the mass grew rapidly to compress the trachea. Local tumor recurrence was noted at 3 months after surgery. Surgical resection or repeat biopsy should be considered if a cytologically benign thyroid mass shows imaging or clinical features of malignancy.

8.Influence of parenting role sharing, parenting stress, and happiness on warm parenting behavior in mothers of children aged 6 years: an analysis using data from the seventh panel study on Korean children

Purpose:
The aim of this study was to examine the relationships among parenting role sharing, parenting stress, happiness, and parenting behavior of mothers with 6-year-old children.
Methods:
This study used data from the seventh Panel Study of Korean Children, which began collecting longitudinal data on a sample of newborn households nationwide in 2008 and will continue yearly until 2027. The participants were 1,560 mothers of children aged 6 years. We conducted statistical analyses using descriptive statistics, the t-test, analysis of variance, Pearson correlation coefficients, and stepwise multiple regression.
Results:
The mean age of the mothers was 36.8±3.7 years. The mothers' perceived parenting role sharing (r=.07, p=.007), parenting stress (r=-.54, p<.001), and happiness (r=.38, p<.001) were significantly correlated with warm parenting behavior. Stepwise multiple regression analysis revealed that parenting stress (β=-.47, p<.001), happiness (β=.15, p<.001), and parenting role sharing (β=.11, p<.001) were significant predictors for warm parenting behavior by mothers.
Conclusion
It is essential to reduce mothers' stress and increase their positive emotions (happiness). Fathers should actively share parenting roles in raising children to enhance mothers' warm parenting behavior.
9.Mutations of the Pre-S Region in HBV-Associated Liver Cirrhosis and Hepatocellular Carcinoma.

BACKGROUND/AIMS: According to the recent research, mutations in the HBV pre-S region may have an impact on the progression of hepatitis B virus(HBV)-related liver disease. The aim of this study was to clarify the frequency and location of naturally occurring mutations in the pre-S region of HBV, and their possible effects on the clinical course of HBV-associated chronic liver diseases. METHODS: HBV DNA was extracted from the sera of 15 patients (8 with liver cirrhosis and 7 with hepatocellular carcinoma). The pre-S sequence was amplified via polymerase chain reaction, subcloning and sequenced. RESULTS: All patients had point mutations in the pre-S region. Nine of 10 mutation sites (90%) in the pre-S1 region, and 4 of 5 mutation sites (80%) in the pre-S2 region were identical in both liver cirrhosis and hepatocellular carcinoma. Deletions were detected in seven patients (4 with liver cirrhosis and 3 with hepatocellular carcinoma). Among the 4 patients with liver cirrhosis, three had deletion in 5'-end of the pre-S2 region and one spanning the 3'-end of the pre-S1 to 5'-end of the pre-S2 region. All 3 patients with hepatocellular carcinoma had deletions in 5'-end of the pre-S1 region, and two patients had simultaneous deletion spanning the 3'-end of the pre-S1 to the 5'-end of the pre-S2. CONCLUSION: The pre-S mutants were frequently detected in HBV-associated liver cirrhosis or hepatocellular carcinoma and the point mutations or deletions in the pre-S gene were clustered in specific regions.

Acase of trigeminal neuralgia caused by a contralateral acoustic neurinoma is presented. After removal of the tumor, the neuralgic pain has completely disappeared. The pathophysiology of this entity is briefly reviewed. The neuralgic pain may be caused by the compression of the contralateral trigeminal nerve by the mass effect in this case.
