1.The Effect of Orthopaedic Operations upon Postoperative Arterial Oxygen Tension

To 48 adult patients general and spinal anethesia was administered for elective orthopedic (intra-thoracic and extremity) and abdominal(general and gynecological) operations. Radial artery was cannulated and postoperative change of PaO2 was observed for 7days. The results are as follows: 1) General anesthesia administered for operations on the legs results in a reduction of PaO₂ which is maximal immediately after discontinuation of anesthesia, gradually returns toward normal in a 3-hour period, and becomes normal on the first postoperative day. 2) General anesthesia administered for thoracomy and laparotomy is followed by the same early changes, which do not return toward normal in the first 3 hours. Reduciton of PaO₂ persists, and PaO₂ continue to deteriorate for several days, not completely returning to normal even 7days post-operatively. 3) When spinal anesthesia is administered for laparotomy, PaO₂ does not begin to fall untill several hours after the end of operation. The subsequent course follows as that in 2, above. 4) When spinal anesthesia is administered for operation on the legs, PaO₂ does not change significantly throughout the postoperative period. 5) Thus, it is concluded that late forms of postoperative hypoxemia is influenced primarily not by the method of anesthesia, but by site of operation.

2.Expression of p53 Protein in Gastric Adenoma and Carcinom.

In the present study, immunohistochemical detection of p53 oncoprotein was performed to determine whether the grade of differentiation and the histologic type of gastric adenocarcinoma, and the degree of atypia accompanied with adenoma can be related to p53 mutation. Paraffin sections of 22 gastric adenomas and 56 gastric adenocarcinomas were examined for the overexpression of p53 oncoprotein with the avidin-biotin peroxidase complex staining procedure. The obtained results were as follows; 1. All the 22 cases of adenomas and 16 cases of well differentated adenocarcinomas showed uniformly negative staining. 2.Seven of 18 cases of moderately differentiated adenocarcinomas(39%), and five of 30 cases of poorly differentiated adenocarcinomas(17%) exhibited p53 protein expression. 3. Three of 29 cases of diffuse type (10%) and 9 of 19 cases of intestinal type(47%) exhibited p53 protein expression. These results suggest that p53 mutation is important in carcinogenesis of the intestinal type of gastric adenocarcinoma, and there is no correlation between the differentiation of gastric adenocarcinoma and the degree of p53 oncoprotein overexpression.

3.Gallbladder carcinoma: Regional lymph nodes metastases on CT scan.

CT scans of 23 patients with gallbladder carcinoma were retropectively reviewed to determine the incidence of lymphatic spread and to access the pattern of regional lymph node metastases. The lymphatic spread were noted in 74%(17/23). The involved patterns of regional lymph node metastases were nodes of porta hepatis 65%, portocaval nodes 41%, superior pancreaticoduodenal nodes 47%, posterior pancreaticoduodenal nodes 59%, nodes around the celiac trunk 35%, nodes around the superior mesenteric artery 29% and paraaortic nodes 35%. The findings suggested that the pattern of regional lumph node metastases in gallbladder carcinoma occurred along the normal lymphatic pathway of the gallbladder.

6.Morphological Study of a New Orbital Implant: Hydroxyapatite-Coated Porous Alumina in Rabbit.

PURPOSE: We evaluated the morphologic change of the hydroxyapatite(HA)-coated porous alumina as a new orbital implant when it was used in rabbit. METHODS: After evisceration of 18 eyes of 18 New Zealand white rabbits, we implanted 12 mm-diameter-sized HA-coated porous alumina with different pore sizes; 300, 500, and 800 micrometer, and then the implants were extracted back to evaluate their morphologic change according to different time interval; 2, 4, and 12 weeks. RESULTS: Fibrovascularization was noted at periphery of the implant 2 weeks after implantation in all of the three groups, and to the center of the implant 4 and 12 weeks after implantation. This fibrova-scularization was most predominant in 500 micrometerpore-sized group compared to the rest two groups. CONCLUSIONS: HA-coated porous alumina showed similar appearance of fibrovascularization with relatively more affordable price and easy manufacture, compared to the other porous implants.

7.A review of mathematical models and strategies for Pandemic Influenza Control.

Despite many countries' current effort to make mathematical models for pandemic influenza (PI) and predict the impact of an outbreak, natural history of PI is remains incomplete, so that any assumption or model is unable to completely predict the impact. Our objective is to review and summarize previous studies on parameters and models of PI, and to make suggestions for the controls in order to decrease the impact of PI. We searched PubMed to retrieve literature about the PI model systematically . Reference lists and review papers on the topic were searched, as well. We found 35 articles that examined the PI model over the period of May 2003 to August 2008. We reviewed modelling methods that were focused on the PI, and their parameters i.e. latent period, and basic reproductive number (R0). Then, we summarized PI controls: antivirals, vaccines, and social distancing. Recent studies showed with mathematical models that Targeted Antiviral Prophylaxis (TAP) is the best strategy for containing PI at the source. In case of an outbreak, quick measures of social distancing, including therapeutic and prophylatic antiviral for cases and closing contacts - school closure, workplace closure, border quarantine, and home isolation - were found to be most effective. We reviewed strengths and weaknesses of models that are adaptable in Korea, and summarized their parameters. It is our hope that these strategies with various interventions give important information for future preparation for and response to PI in Korea.

8.Surgical Treatment of Intracranial Arachnoid Cyst by Excision.

Arachnoid cysts are benign developmental cysts that occur between aachnoid layers, which represent approximately 1 percent of all intracranial space-occupying lesions, their most common location being the middle cranial fossa. Current surgical treatment options for intracranial arachnoid cyst are either craniotomy and excision of cyst or shunting of cyst contents extracranially. We treated 10 cases of intracranial arachnoid cyst by excision during last two years. For the best surgical curability, we have, not only removed the cyst wall and adhesive gliotic tissue as possible, but also made the opening to basal cistern for communication. During average 8.4 months follow up, we observed, not specific morbidity and mortality, but good clinical improvement and brain expansion. As above result, if haven't specific problem, the direct surgical removal may be the primary choice for intracranial arachnoid cyst, especially in children with noncommunicating type in sylvian.

9.A Case of Chemical Pneumonitis Induced by Ingestion of Hydrocarbon.

A 33-year-old woman was presented with dyspnea and chest discomfort after indigesting approximately 500ml of oil paint brush washing fluid. Hypoxic symptoms and radiographic infiltrates rapidly progressed. The patient was intubated and received mechanical ventilation. Bronchoalveolar lavage and transbronchial lung biopsies were performed. The CT scan of the lung showed bilateral extensive pneumonitis with necrosis and the lung tissue pathologic findings showed diffuse alveolar damage with extensive necrosis and numerous lipidladen macrophages. After intensive medical care with mechanical ventilation, her symptoms and radiological findings improved.

10.Surgical Treatment of Spontaneous and Traumatic Delayed Cerebrospinal Fluid(CSF) Rhinorrhea.

The authors presented 9 cases of surgically treated CSF rhinorrhea, two of which were spontaneous and 7 were traumatic delayed. The duration of leakage was longer than 1 month in most cases and seven of the nine cases were complicated by meningitis. The leakage site was most accurately detected by metrizamide computed tomographic cisternography(MCTC) and the most frequent leakage site was the cribriform plate. Good surgical results were obtained by direct repair and/or shunt without recurrence during follow-up period from 7 months to 2 years.
