1.Malignant Fibrous Histiocytoma of the Liver.

Primary sarcomas of the liver are rare. A case of primary malignant fibrous histiocytoma of the liver is reported. A 55-year-old male was admitted with epigastric pain. An abdominal computed tomographic scan disclosed a 10cm, low-density area in the left lobe of the liver. Histological examination of the resected tumor showed bundles of spindle cells arranged in a storiform pattern. In some areas, many bizarre giant cells were scattered. Immunohistochemically, tumor cells were positive for alpha 1-antitrypsin and alpha 1-antichymotrypsin, and weakly positive for vimentin. The tumor cells did not express cytokeratin, desmin or alpha fetoprotein.

3.A Statistical Review of Benign Tumors of the Skin for the Past 10 Years.

We have reviewed the 503 cases of benign skin tumors which were confirmed by histopathological study from January 1977 to July 1987 in the Dermatologic Department of Kyungpook National University Hospital. The result were summarized as follows . l. Among the total of 26,690 dermatologic out patients, 503 cases(1.9%) were diagnosed as having benign skin tumors. The number of benign skin tumors has shown a noticeable increase in the last several years. 2. The tumors were found mainly in ages ranging from the 2nd to 5th decade, most commonly in the 3rd decade(124 cases, 24.7%). 3. Aceording to Lever's elasaification, the tumors were divided into the following 7 groupa . tumors and cyets of the epidermis(35.2%), melanocytic tumors(16.5%), tumors of vaecular tissues(13.3%), tumora of epidermal appendages(11.1%), tumors of fibrous tissue(8.9%), tumors of neura.l tiasue(7.6%) and tumors of fatty, muscular and osseous tiseue(7.4%) in decreasing orders. 4. The 14 common tumors in order of frequency were as follows epidermal cyst (17.l% ),:melanocytic nevus(16.5%), neurofibroma(7.6%), congenital hemangioma(6.4%), nevus sebaceous(6.0%), lipoma(5.2%),seborrheic keratosis(4,8%), linear epidermal nevus(4.6%), steatocystoma mutiplex(2.8%), dermatofibroma(2.6%), hypertrophic scar and keloid.(2.6%), granuloma pyogenicum(2.4% ), keratoacanthoma(2.0%) and syringoma (2,0%). 5. The predilection sites of seborrheic keratosis, keratoacanthoma, syringoma and melanocytic nevus were the face, linear epidermal nevus, dermatofibroma and granuloma pyogenicum were located on the extremities. 6. The duration of tumors from the presumed time of onset to the first visit was within 1 year for epidermal cyst, keratoacanthoma and granuloma pyogenicum, over 10 years for nevus sebaceous, congenital emangioma and neurofibroma. 7. Diagnostic accuracy was higher in melanocytic nevus, congential hemangioma, nevus sebaceous and neurofibroma and lower in steatocystoma multiplex, dermatofibroma, keratoacanthoma, lipoma and epidermal cyst.

4.Crescentic Glomerulonephritis in a Patient with Rheumatoid Arthritis: A case report.

Crescentic glomerulonephritis in rheumatoid arthritis is described recently with increasing frequency. It can occur directly as a manifestation of rheumatoid arthritis or may be a reaction to drugs such as D-penicillamine and bucillamine. We report a case of crescentic glomerulonephritis in a 46-year-old woman with rheumatoid arthritis for 20 years who had been treated intermittently with herb medicine or nonsteroidal anti-inflammatory drugs (NSAIDS). Light microscopic examination showed severe focal segmental and global necrotizing glomerulonephritis with crescent formation in 50% of the glomeruli. Immunofluorescent study revealed scanty amount of mesangial granular deposits of IgA, IgM, C3 and fibrinogen in a diffuse pattern.

5.General Anesthesia for Patient with Broncho-gastric Fistula: A case report.

Broncho-gastric fistula caused by benign gastric ulcer perforation after esophagectomy is very rare. In general anesthesia of a patient with broncho-gastric fistula, in spite of hyperventilation, leakage of the anesthetic gases through fistula may make the patient hypercapneic, and positive pressure ventilation may increase the risk of the pulmanary aspiration by the regurgitation of gastric fluid by stomach distension. For that reason, in this patient, denitrogenation was performed during patient's voluntary respiration with 100% oxygen for 5 minutes, and induction was performed without positive pressure ventilation, and one lung ventilation was carried out. Hypoxemia was followed by one lung ventilation because his pulmonary function was moderate obstructive type and his lung was damaged by aspiration of gastric fluid via broncho-gastric fistula. A low level of continuous positive airway pressure (CPAP) has no significant hemodynamic effect and can maintain the patency of nonventilated lung, so hypoxemia induced by one lung ventilation may be reduced. Thus we carried out one lung ventilation with CPAP (10 cmH2O) in nonventilated lung and blocked broncho-gastric fistula with a bronchial blocker for prevention of both regurgitation of gastric fluid and leakage of anesthetic gases. One lung anesthesia was performed without any problem in this case.

7.A Case of Hemosiderosis due to Multiple Transfusion.

Hemosiderosis is a disorder characterized by deposits of hemosicerin in multiple organs without organ dysfunction or injury. The color of the skin becomes brown, espeially on sun exposured areas. Mucous mernbranes are also affected in up to 5-25% of cases. A 43-year-old woman visited our clinic due to generalized brown colotation of her skin, including conjunct we and oral mucous mernbrane which she had had for 10 months. She had been treated with multiple he nodialysis combined with periodic transfusion for renal failure 3 times a month during the last 5 yeadrs Her plasma ferrit in level was elevated markedly and TIBC decreased, but cortisol and ACTH levels were within normal limits, Histopathologic findings of the neck skin showed hyperpigmentation of basal layer and deposition of brown pigment wrthin and around the sweat glands. In Fontana-Masson stain, the lower epidermis showed a massive deposition of melanin. Prussian blue stain revealei hemosiderin within snd around, the sweat glands. Hemosiderin could also be noticed on specirnens of thliver biopsy. The patient improved progressively after the rstriction of trasnfusion and treatment of renal failure.

The deletion 9p syndrome is a well characterized syndrome with about one hundred cases having been reported. Most patients have dysmorphic facial features, cardiac anomalies, and mental retardation. We report on a female infant with micrognathia, corneal opacity, cleft palace, cardiac anomaly, left polycystic kidney, and deletion 9p. Chromosome analysis and fluorescence in situ hybridization (FISH) showed her to have a derived chromosome 9 inherited from a maternal t(9;16) (p22;p13.2) by adjacent I segregation There are few reports of this particular chromosome rearrangement. We review deletion Sp syndrome.

9.A Case Report of Slipped Capital Femoral Epiphysis Associated with Hypogonadism and Diabetes Insipidus

The slipped capital femoral epiphysis is the condition in which the femoral head slips downward and backward on the femoral neck at the epiphyseal plate. The underlying cause of this disease is unknown. In general, it is believed that endocrine factors may play a part as shown in experimental work. But few endocrine abnormalities have been proved. It is rare in Korea and eight cases have been reported and only one case was associated with diabetes insipidus in hypogonadal Turner mosaicism. Authors report a case of mild, acute on chronic slipped capital femoral epiphysis in a 19 year old male with hypogonadism and diabetes insipidus. It was treated by internal fixation with three Steinmann pins and the result was good.

10.The Effect of Antihypertensive Therapy on Left Ventricular Mass in Hypertensive Patients.

BACKGROUND: Hypertension is the major risk factor for cardiovascular disease. The increased left ventricular mass has been recognized as an independent predictor of morbidity and mortality in hypertensive patients. The assessment of the regression of left ventricular(LV) mass after antihypertensive therapy offers prognostic information. 2D echocardiography has proved a sensitive tool for the detection of the change of LV mass. METHOD: LV mass and LV mass index were measured by area-length method of 2D echocardiography in 26 hypertensive patients and 10 normal control to evaluate the effect of betablocker(group I, n=16) and angiotensin converting enzyme(ACE) inhibitor(group II, n=10) on the regression of LV mass. RESULT: There was a significant increase of LV mass and LV mass index in the hypertensive patients(199.0+/-37.7gm, 119.2+/-21.2gm/m2) compaired to the control(129.7+/-11.7gm, 87.4+/-8.8gm/m2)(p<0.01, p<0.01). After 13.1 week treatment, LV mass was significantly decreased in group I(200.9+/-35.3gm vs 164.7+/-25.4gm)(p<0.01) and group II(195.9+/-43.3gm vs 152.4+/-27.1gm)(p<0.01). The LV mass index was also significantly decreased in group I(120.3+/-20.7gm/m2 vs 98.8+/-15.5gm/m2)(p<0.01) and group II(117.5+/-22.9gm/m2 vs 91.5+/-13.6gm/m2)(p<0.01). CONCLUSION: This results showed that LV mass can be reduced in hypertensive patients who receive beta blocker and ACE inhibitor. The effect of antihypertensive therapy on LV mass should be considered in treatment of the hypertensive patients who had hypertrophied LV.
