7.Effect of the Exchange of Saline Used in Surgical Procedures on Surgical Site Infection.

PURPOSE: The purpose of this study was to identify the effect of the exchange of saline used in surgical procedures on surgical site infections. METHOD: Patients with stomach cancer were assigned to the experimental group or to the control group by random sampling, respectively. The experimental group received an exchange of saline during the operation right after the excision of the stomach in a gastrectomy but the control group did not. Data were collected from the medical charts of 34 patients from Dec. 1, 2002 through May 31, 2003. RESULT: The surgical site infection rate of the experimental group was 5.9% while surgical site infection rate of the control group was 17.6%. In total, the surgical site infection rate was 11.8%. The experimental group maintained a normal level of WBC on post operative day 3; however, the control group, showed an increase of WBC on post operative day 3. CONCLUSION: The exchange of saline used in an operation immediately after the excision of the stomach in a gastrectomy decreases the contamination level of saline used in the operation, and can prevent surgical patients from a surgical site infection.

8.Pros and Cons of Various Research Designs in Clinical Psychiatry.

An appropriate research design for hypotheses and purposes leads to a good quality of research results. In this review article, we summarized the types of research methods and described the characteristics of clinical trials. Research designs are categorized into observational studies and experimental ones, depending on data collecting methods. In an observational study, there are cross-sectional, cohort and case-control studies. Parallel groups design and crossover trial studies are representative designs in a randomized controlled trial study, a kind of experimental study. Clinical researchers should understand the characteristics of clinical research designs including advantages and disadvantages and choose the suitable design according to their study purposes and the nature of collected data or subjects.

9.An experimental study on radionuclide imaging of bowel infarction using (99m)Tc-pyrophosphate

99m Tc-pyrophosphate was investigated for use as an indicator of intestinal infarction in intussusceptedbowel. Irreducible intussusceptions were created in 18 rabbits by surgery. 99mTc-pyrophosphate was then injectedintravenously 6-12, 18, 24 and 30-40 hours later for external scanning. In 15 of the rabbits, infaractiondeveloped with intussusception, and 13 of them demonstrated increased uptake of 99m Tc-pyrophosphate on externalinvivo scintiscans. The remained 2 of them showed no evidence of increased uptake. So false negative cases were2(sensitivity 86.7%). The 3 rabbits without infarction showed no increased uptake of radionuclide. Specimenscanning confirmed increased uptake of radionuclide in the infarcted segments. These observations suggest that 99mTc-pyrophosphate is a reliable indicator of the intestinal infarction that sometimes occures with intestinal intussusception.

10.Chondrosarcoma: MR Imaging Findings Correlated with Pathologic Classification and Grade.

PURPOSE: To evaluate the MR imaging findings of chondrosarcomas by correlation with pathologic classificationand grade. MATERIALS AND METHODS: We performed MR imaging-pathologic correlation of nineteen chondrosarcomas. Conventional chondrosarcomas accounted for 15 cases (grade I : 6, II : 6, III : 3) and the mesenchymal and dedifferentiated types each accounted for two. MR signal intensity (SI) of the tumor on T1- and T2-weighted images(T1WI and T2WI, respectively), was classified as homogeneous or heterogeneous low-, iso- or high SI, and enhancing pattern as marginal, marginal and septal, marginal and nodular, or diffuse enhancement. RESULTS: Eighteen cases of chondrosarcomas (95%) showed homogeneous or heterogeneous low- or iso SI on T1WI and high SI on T2WI. Low gradeconventional chondrosarcomas showed marginal and septal (n=8/10) or marginal (n=2/10) enhancement on Gd-enhanced MR images. Grade III conventional chondrosarcomas showed marginal or marginal and nodular enhancement. Dedifferentiated and mesenchymal chondrosarcomas showed marginal and nodular or diffuse enhancement. CONCLUSION: Chondrosarcomas showed iso- or low SI on T1WI and high SI on T2WI. Marginal and septal enhancement was demonstrated on Gd-enhanced MR images of grade I and II conventional chondrosarcomas. If a tumor showed amarginal and nodular or diffuse enhancing pattern, this suggested it was a of high grade chondrosarcoma.
