4.A Case of Acrodermatitis Enteropathica Associated with CMV Hepatitis.

Acrodermatitis enteropethica (AE) is a rare autosomal recessive disorder of zinc absorption leading to chronic diarrhea and characteristic skin lesion. The term is also applied to any acquired zinc deficiency state resulting in the same clinical pictures. We experienced one case of AE in 1 month old male infant who had bacterial enterocolitis. The skin around mouth, anus, eyes, ears, hands and legs became reddish, vesicular and eczematoid. Serum zinc level was decreased to 51.4 microgram/dL (N=70~150). Endoscopic finding revealed pale gastric mucosa and villous atrophy of small intestine. Biopsy finding of small intestine showed no villi due to mucosal atrophy. On 13 day of admission jaundice with DIC were noted and AST & ALT were elevated to 110 & 36.8 IU/L, respectively. Diarrhea was improved but jaundice and liver function were not recovered until discharge from hospital. After discharge when the patient was 4 months of age serum bilirubin and AST/ALT had not been normalized. CMV shell vial culture of urine and CMV Ig G antibody were positive. So intravenous ganciclovir injection of 7.5 mg/kg, two times a day for 2 weeks and then 10 mg/kg/day for 3 months was done from 4 to 6 months of age. No virus was found in the urine and AST & ALT were normalized at 2 months after stopping ganciclovir treatment.

5.Isobaric and Hyperbaric Tetracaine Spinal Anesthesia for Knee Arthroscopy in Supine Position.

To compare the advantage and disadvantage of isobaric and hyperbaric tetracaine spinal anesthesia, ninety, either sex, adult patients scheduled for knee arthroscopy were assigned randomly into two groups; one with the isobaric (n=70), and the other with the hyperbaric (n=20) tetracaine spinal anesthesia. The isobaric solution was prepared to 0.5% tetracaine in 50% cerebrospinal fluid of the patient's own and the hyperbaric solution to 0.5% tetracaine in 5% dextrose in water. Epinephrine was mixed to either solution in the ratio of 1: 200,000. The dosage of tetracaine was determined by the patient's height; 0.1 mg/cm height-difference, added or reduced to the reference dose of 12 mg/160cm. The site of lumbar puncture was L2-3 intervertebral space and 22 G Quincke spinal needle used. The speed of injection was fixed to the rate of 1 ml/10 sec. The onset, duration and maximal anesthetic level (MAL) in both groups were not significantly different and blood pressure and heart rate decreased with the elapsed time during anesthesia in similar pattern in both groups. And also, there was no significant difference in postspinal anesthetic complications such as headache, hypotension, etc. In conclusion, there was no significant difference in clinical aspects between hyperbaric and isobaric tetracaine spinal anesthesia for knee arthroscopy, with corresponding doses and strength of tetracaine, technics in room temperature.

6.In vitro study on the adherence and penetration of candida albicans into denture soft lining materials.

Purpose: Colonization of denture soft lining materials by Candida albicans can result in clinical problem, and deterioration of the materials. This study aimed to compare the retention and penetration of C. albicans into four denture soft lining materials commonly used. Materials and methods: Four denture soft lining materials(Coe-comfort., Coe-soft., GC soft liner., and Tissue conditioner.) discs were prepared to glass slide and dental stone. Adherence of yeast to surfaces was monitored after one hour incubation of standardized washed cell suspension with test disc surfaces. Adherent cells stained with acridine orange were counted fluorescence microscopy. Penetration of yeast into materials bonded with acrylic resin after 1, 2, 3, 4, 5, 6 and 7 days incubation was observed through sections stained using acridine orange and estimated to quantitative analysis using radioisotope. Results: There was statistical significance in cell numbers between smooth and rough surfaces(p<0.05). Higher numbers of cells were observed on rough surfaces. There was statistical significance in adherent cell numbers into smooth and rough surfaces individually(p<0.05). According to the increase of incubation periods, the cells penetrated into denture soft lining materials were shown to increase. The differences among all kinds of soft liner were statistically significant(p<0.05),and the largest number of cells penetrated into soft liners was observed in the Coe-soft. Conclusion: Initial adherence and penetration of yeast into denture soft lining materials has been influenced by surface roughness and chemical composition of them. The selection of appropriate materials and their fabrication may promote clinical performance.

7.Osteoporotic Vertebral Fracture with Myelopathy.

STUDY DESIGN: Retrospective analysis of surgical treatment in patients with osteoporotic vertebral fracture associated myelopathy. OBJECTIVES: To evaulate the clinical outcome of anterior decompression and fusion for osteoporotic vertebral fracture associated with myelopathy. SUMMARY OF LITERATURE REVIEW: Major treatment of osteoporotic vertebral fracture were conservative methods. In patients with myelopathy, surgical treatment is recommanded. MATERIALS AND METHODS: From January 1995 to December 1998, twelve patients who had osteoporotic vertebral fracture associated with myelopathy and treated by operation were evaluated retrospectively. With simple roentgenography and dual energy absorptiometry, osteoporosis was evaluated. And with MRI and nerve conduction velocity test, we could diagnosed myelopathy. In ten patients, anterior approach was used, and in two patients, posterior approach was used. RESULTS: In all patients after operation, the neurologic symptoms according to the Frankel grading scale were improved over one grade and follow-up X-ray showed bone union finding unrelated to the site, shape, and severity of fracture. No significant complications such as increasing of kyphotic angle and metal loosening were existed in all cases. CONCLUSIONS: For the osteoporotic vertebral fracture associated with myelopathy, we can treat properly with spinal decompression and interbody fusion with internal fixation.

8.Two cases of Creosote Induced Gastric Ulcer.

Secondary peptic ulceration and gastritis have been known to be associated with stress, exogenous agents, drugs or infection. Salicylate (aspirin) ingestion has been known to be associated with increased incidence of gastric ulcer and more frequently as the cause of hemorrhagic gastric erosions and gastritis. Some medications such as tetracyclines and iron preparations have been associated with ulceration of the gastrointestinal tract. Chemotherapeutic agents including cytoxan and methotrexate also have been implicated in the development of mucosal and gastrointestinal ulcers. We have experienced two cases of hemorrhagic gastric ulcers due to creosote ingestion in a 13 month and a 45 year old boys. Creosote is a main component of one of the popular digestives, Jungrohwan in Japan and Korea.

10.Influence of individual tooth tray on the teeth with undercut in impression taking.

PROBLEM: The several studies were reported to the effects of individual tooth tray with the parallel tooth, but not reported to the effect of individual tooth tray with severe undercut in impression making. PURPOSE: The purpose of this study is to examine the effect of individual tooth tray; 1) the accuracy of impression in in terms of distance. 2) reproducibility of pocket. MATERIAL AND METHODS: In this study, a metal model with 3 teeth including tooth prepared 30-degree undercut and gingival crevice of the depth 5mm, width 0.4mm was cast. Impressions of the metal model were taken in 5 times used 4 kinds of impression materials with individual tooth tray and another impression taking were made with conventional method without individual tooth, used same materials, procedure. The stone models were made. Distances between abutments were measured with a Micromeasurescoup-mm. RESULT: The following results were obtained; 1. In the comparison of inter-abutment distance between parallel tooth without individual tooth tray, 4elastomeric materials have not significant difference to the metal model(p>0.05). 2. In the comparisonof inter-abutment distance with undercut tooth without individual tooth tray, 4elastomeric materials have a significant difference to the metal model(p<0.05), especially polyether's dimensional change is large. 3. If individual tooth tray was used to take impression in abutment teeth with severe undercut, more stable results was achieved easily than conventional metho.(p>0.05). 4. It is not true that the pressure of impression material with individual tooth tray increased to gingival sulcus. CONCLUSION: By the results, I knew the fact that individual tooth tray was useful impression method on the teeth with severe undercut for the dimensional stability. Also, I could not confirm the fact that individual tooth tray would increase the penetrating pressure into cervice.
