1.Serum Insulin-like Growth Factor-1(IGF-1) and Insulin-like Growth Factor Binding Proteins in Hyperthyroidism and Hypothyroidism

Backgrounds: Thyroid hormones play a fundamental role in the initiation and maintenance of somatic growth in mammalian species, and the insulin-like growth factors(IGFs) occupy a position of central importance in the growth of all tissues. To evaluate the changes in serum insulin-like growth factor-I(IGF-I) and insulin-like growth factor binding proteins in hyperthyroid and hypothyroid patients, sera was obtained from 19 hyperthyroid patients, 9 hypothyroid patients, and 10 healthy volunteers. Methods: IGF-I concentration was determined by radioimmunoassay, and changes in IGFBPs were assesed by Western Ligand Blotting. To evaluate the binding pattern of IGF-I & IGFBPs, autoradiographs were obtained. Results & Conclusion: IGF-I levels were increased significantly in hyperthyroid patients(mean ±SE, 267.88±9.80 ng/ml, p<0.05) and decreased significantly in hypothyroid patients(154.81±1.43 ng/ml, p<0.01) compaired to healthy control group(209.45±.60 ng/ml). Autoradiograph of serum IGFBPs from patients with hyperthyroidism and hypothyroidism did not show any change in the intensity of IGFBP-3 bands(40-45 KD) and IGFBP-1 bands, but in hyperthyroid patients, it showed increased intensity of IGFBP-2 band compared to healthy control group and hypothyroid patients.

3.Short-Term Results of Dexamethasone Intravitreal Implant in Patients with Refractory Diabetic Macular Edema.

PURPOSE: To evaluate the effectiveness of a dexamethasone intravitreal implant (Ozurdex(R)) in the treatment of diabetic macular edema refractory to combined treatment of bevacizumab and triamcinolone. METHODS: We reviewed 9 eyes of 9 patients with diabetic macular edema treated with dexamethasone intravitreal implant. The patients were included in the study if presenting with refractory diabetic macular edema of more than 3 months despite combined treatment of intravitreal bevacizumab injection with posterior subtenon triamcinolone injection or intravitreal triamcinolone injection. We assessed the best-corrected visual acuity (BCVA) and central macular thickness (CMT) using optical coherence tomography at initial visit and 1, 3 and 4 months. RESULTS: The mean follow-up was 6.7 +/- 2.2 months. The baseline mean BCVA was 0.81 +/- 0.47 logarithm of the minimum angle of resolution (log MAR), which improved to 0.61 +/- 0.37 log MAR (p = 0.017), 0.57 +/- 0.38 log MAR (p = 0.011) and 0.62 +/- 0.36 log MAR (p = 0.027) at 1 month, 3 months and 4 months, respectively. The baseline mean CMT was 558.0 +/- 110.32 microm and decreased to 325 +/- 64.21 microm (p = 0.008) and 300.22 +/- 59.46 microm (p = 0.008) at 1 month and 3 months, respectively, then increased to 468.44 +/- 150.85 microm (p = 0.058) at 4 months after injection. CONCLUSIONS: Dexamethasone intravitreal implant showed short-term efficacy in the treatment of diabetic macular edema refractory to combined treatment of bevacizumab and triamcinolone and produced significant improvements in BCVA and CMT until 3 months after injection. The CMT then increased, but BCVA was sustained until the fourth month.

4.Influence of insulin injection on quality of life and depression in diabetic patients.

Insulin is the most effective drug for reducing blood glucose in patients with diabetes commonly. But it has negative aspect for diabetic patients to change from oral hypoglycemic drug to insulin injection. Insulin injection makes patients feel reducing self-esteem and QOL (quality of life). So they complain depressive moods. Multiple insulin injection has an effect on patient's emotion more negatively than one or two injection of insulin. It is suggested that physicians much more concern about self-esteem and emotional aspect of patients when they prescript insulin

5.Influence of weight gain to cardiovascular risk factors.

BACKGROUND: The obese patients increased recently in Korea due to excessive energy intake and decreased physical activity. We are well known that obesity is associted with an increased risk of cardiovascular disease, angina pectoris, hypertension, but not known to influence of weight gain to change in cardio-vascular risk factors. Once, the study of obesity are many but the study of influence of weight gain are small in Korea. This study investigated to influence of weight gain in cardio-vascular risk factors. METHODS: We gatherd case and control group among persons who has taken twice medical examination except disease person from October 1990 to October 1995 at SungAe General hospital. In our study, total 167 case, compare weight gain group(>3kg) with control group to change in cardiovascular risk factor. RESULTS: Sex, age, smoking history, obesity index were not statistically significant difference between two group in the first medical examination. systolic blood pressure, diastolic blood pressure, fasting blood sugar, HDL, total cholesterol/HDL were not statistically significant difference between two group for weight gain. Total cholesterol and triglyceride, LDL were stastistically significant increased. CONCLUSIONS: Weight gain was increased of total cholestrol and triglyceride, LDL, so weight control was important.

6.Psychotherapy and Psychosocial Therapy for the Geriatric Mood Disorders.

Among the mood disorders in the elderly aged older than 65, dysthymia, major depression, and mania constitute the prototypes and depressed mood is the most frequent symptom. The mode of psychotherapy and psychosocial therapy for geriatric depression, which are similar throughout adulhood, include brief dynamic psychotherapy, interpersonal psychotherapy, psychoanalytic psychotherapy and psychoanalysis, supportive psychotherapy, cognitive therapy, family therapy, and group therapy. Psychotherapy alone is of value primarily in dysthymia and major depression. Short-term cognitive therapy, brief dynamic psychotherapy, and interpersonal psychothearapy have been found to be effective, without medications, in high-functioning elders suffering from major depression with minimal or no cognitive impairment. Cognitive-behavioral psychotherapy may be the treatment of choice in the depressed medically ill. Whatever technique is utilized, it is important to be aware of the dynamic issues that are prevalent in geriatric depression patients, as well as the transference and countertransferenc problems that may be aroused.

8.Perceived Stressful Life Events, Coping Style, Social Support and Depressive Symptoms of the Functional Dyspepsia Patients.

OBJECTIVES: To investigate various factors that can mediate stress and response, such as coping style, social support, level of perceived stress, and psychopathology(especially anxiety and depression) of the functional dyspepsia(FD). METHODS: Thirty adults with functional dyspepsia were compared to thirty healthy adults who had visited the hepatobiliary clinic due to non-organic complaints(tiredness and fatigue) without FD, but the results of medical examination were normal. All subjects were evaluated for anxiety and depressive symptoms, perceived stress, coping style and social support. Symptom Checklist-90-Revision, Beck Depression Inventory, Spielberger State-Trait Anxiety Inventory, Ways of Coping Checklist, Interpersonal Support Evaluation List, and a self-reporting questionnaire were included to measure the quantity of perceived stressful life events. RESULTS: The FD patients had significantly more symptoms of depression, more perceived stressful life events, demonstrated less problem-focused coping, and less social support than the control subjects. Depressive symptoms of the FD patients were negatively correlated with interpersonal support and were positively correlated with trait anxiety, whereas, problem-focused coping were positively correlated with interpersonal support. The two groups did not differ significantly in terms of anxiety and emotion-focused coping. CONCLUSION: Our results suggest that FD patients need to be evaluated for psychiatric problems, and might benefit from psychiatric treatment with psychotherapeutic and psychopharmacological modalities.
