3.The recovery of brain damage caused by cardiac arrest during anesthesia.

The recovery process of brain damage caused by an acute severe cerebral hypoxia has been reported in various literatures. And the possibility of complete recovery of such case was said to be good in younger age group than in adult's group. We experienced a case of cardiac arrest of a 12 year old girl during halothane anesthesia induction and the cardiac and pulmonary resuscitation was succeeded promptly. But the hypotensive period persted for few minutes before the diagnosis was made. The condition of the patient after the resuscitation was good except that her unconsciousness state persisted unusually. The proposed appendectomy performed uneventfully and the anesthesia recovery of the patient was carefully observed in our I.C.U. The uncoscious state lasted for 2 days with agitated movements of legs, EEG showed abnormal, irregular patterns but showed no signs of damage of gobus pallidum and putamen. During the first month, the order of recovery of cerebral function was comatous state, swallowing, eating, and urination. The cerebellar dysunction was prominent. Then recovery of amnesia and defection followed. Speech and gate started to regain in the 26 and 36 post-operative day respectively. Writing function started recover quickly with the recovery of speech function, which occurred in 4 to 6 post-operative months. Recovery of intelligence was slowest and gradually reached to the intelligent level of 11 year old child in one year. It was found the primitive function recovered first and the highly cultivated function recovered last.

4.A Case of Malignant Glaucoma-like Phenomenon During Cataract Surgery.

PURPOSE: Malignant glaucoma is a rare complication of anterior segment surgery. The authors present a case of a malignant glaucoma-like phenomenon during cataract surgery in a primary open-angle glaucoma (POAG) patient. CASE SUMMARY: A 74-year-old man with POAG underwent a cataract surgery in his left eye. After phacoemulsificiation, sodium hyaluronate was used to form the capsular bag for intraocular lens implantation. Following this procedure, the capsular bag space and anterior chamber became shallow. Intraocular pressure (IOP) elevated, and the shallowing of the anterior chamber and capsular bag space continued to a dangerous level. The surgical procedure was stopped and postponed for two days. After two days, IOL was successfully implanted in the posterior bag. CONCLUSIONS: Although the preoperative ophthalmologic examination failed to reveal a zonular problem, there was a possibility for a small area of zonular defect. A malignant glaucoma-like phenomenon was suspected to have occurred due to leakage of sodium hyaluronate or balanced salt solution into the vitreous through a small area of zonular dialysis. If a similar condition, occurs delayed surgery for IOL implantation would be more effective than vitrectomy or laser treatment.

7.Jejunogastric intussusception prone to misdiagnosis as gastric cancer

The authors report a case of a 78-year-old female with a history of gastric surgery 35 years ago. She was initially misdiagnosed as gastric cancer bleeding and underwent an emergency laparotomy under the diagnosis of jejunogastric intussusception (JGI), 23 hours after the onset of symptoms. We also reviewed 116 JGI case reports and analyzed clinical features and outcomes. Compared to the past, diagnosis of JGI is easier with diagnostic examinations such as an endoscopy, computed tomography, and the upper gastrointestinal series. And a good prognosis can be expected with proper fluid resuscitation and surgical reduction, even if the symptoms persist more than 48 hours.
8.Hypotensive Efficacy and Safety of Ramipril on the Patients with Essential Hypertension.

Thirty three patients with mild to moderate essential hypertension were administered ramipril, a new ACe inhibitor, 2.5-5mg once daily for 6 weeks to evaluate the hypotensive efficacy and safety. The results were as follows : 1) The patients consisted of 18 males, 15 females, aged 54 on average and classified as mild in 19 and moderate in 14 patients. 2) At the end of 6 weeks trial, blood pressure dropped 13/6mmHg on average, rewarding 70% effectiveness and normalized below 150/90mmHg in 39%. 3) The most frequent side reaction was dry cough in 5 patients(15%) followed by asthenia, GI trouble, dizziness, edema, eye injection and one case of GPT elevation which normalized after completion of trial. 4) The rating of safety of safety was 79% and of overall usefulness was 73%. In conclusion, ramipril 5mg once daily regimen is well tolerated and effective in the treatment of mild to moderated essential hypertension.

9.Click Evoked Myogenic Potentials in Vestibulocollic Reflex.

Click sound can evoke myogenic potentials in active sternocleidomastoid(SCM) muscle. These are due to activity of vestibular afferent rather than cochlear, which produce very short latency inhibition of ipsilateral motor unit activity. We investigated the click-evoked vestibulocollic reflexes in neck muscles to find out the significance and relationship of latency and amplitude of myogenic potentials between two different methods of muscles activation. EMGs were recorded from surface electrode evoked by sound stimuli(95dB SPL, 5/sec repetitive rate, 512 times) over SCM muscles on each side. Myogenic potentials have two biphasic responses which were termed P1, N1, P2, N2 based on the polarity of their components. The subjects were 24 normal volunteers. EMG recordings were obtained from 12 subjects(Group 1) who keeped head 10 cm above ground in supine position to activate their neck flexors to the degree required through the averaging runs(bilateral SCM muscles activation method). EMGs from another 12 subjects(Group 2) were recorded by rotating head to the opposite side to activate SCM muscle throughout the procedure(unilateral SCM muscle activation method). The latencies and amplitudes of myogenic potentials in the SCM muscle after stimulation were analysed and compared between the two methods. The latencies(msec) of P1 and N1 were 11.2+/-2.4, 21.7+/-1.1 respectively in Group 1 and 13.3+/-2.4, 23.0+/-1.1 respectively in Group 2. The amplitude(uV) of P1-N1 were 38.78.1 in Group 1 and 33.28.1 in Group 2. There were no significant differences in latencies and amplitudes of P1 and N1 between the groups(p<0.05). Unilateral muscle activation is easier and comfortable than the bilateral muscle activation and there were no significant difference in latency and amplitude of P1. The myogenic potential of unilateral vestibular function loss disappeared in a patient who was labyrinthectomized but there was normal response of potential in a patient who is profound sensory neural hearing loss. Click evoked myogenic potentials is a simple method of examining vestibulocollic reflex. Click evoked myogenic potential is related to vestibular status.

10.The Incidence of Pediatric Trampoline Injury: Statistics from a Single Institution.

PURPOSE: Recently, there has been increasing number of trampoline injuries at our pediatric orthopedic clinic and emergency room. Therefore, this study is to analyze the prevalence of the trampoline injuries with emergency room surveillance. MATERIALS AND METHODS: Between September 2015 and February 2017, all patients under the age of 15 years, who visited emergency room of a teaching hospital with trauma and referred to the department of orthopedic surgery, was included; the cause of trauma was analyzed. The severity of injury was also evaluated and compared according to the cause of injury. RESULTS: There were a total of 1,807 patients under the age of 15 years who visited our emergency room during the study period. There were a total of 71 trampoline injuries (3.9%). There were 28 patients below the age of 5 years, and among them, 14 minor injury were recorded; 43 patients were over the age of 5 years and with 4 minor, 20 moderate, and 19 severe injuries. In the age between 5 and 15 years, the prevalence of mild injury was lower in trampoline injury compared with those of other sports injuries (9.3% vs. 34.0%) and the prevalence of severe injury was higher in trampoline injury (44.2% vs. 26.3%) (p=0.002). CONCLUSION: Our findings indicate that the incidence of trampoline injury was lower than other country, but it was more severe than injuries from other sports. With increasing incidence of trampoline injuries, parents should be more mindful that trampolines are not safe and stricter supervision may be necessary.
