BACKGROUND: The principle of Mohs micrographic surgery is to examine microscopically the entire peripheral margin of an excised tissue specimen to precisely localize the site of any residual tumor and enable its subsequent removal. In processing in the tissue for frozen horizontal sections, a technical difficulty arises in manipulating the deep and lateral margins into the same flat plane. OBJECTIVE: Our aim was to develop a new method allowing the optimal tissue preperation for Mohs micrographic smgery and to evaluate the usefulness of this methad. METHODS: The technique involves conversion of a non-planar surgical margin to a planar surface by applying and freezing the specimen against a flat glass surface. We compared this modified method used in our laboratory with standard methods for tissue preparation of Mohs micrographic surgery.

Acral persistent papular mucinosis has been thought to be a distinct for of cutaneous mucinosis not associated with systemic diseases. It was recently classified as one of five subtypes of localized lichen mixedematosus. A 64-year-old man presented with a 15-year history of flesh-colored to translucent papules and a few round hypopigmented patches on his wrists, back of the hands, and distal forearms. Biopsy from the papular lesions revealed focal mucin accumulation in the upper reticular dermis. The clinical and histopathological features were consistent with acral persistent papular mucinosis.

Angiokeratoma circumscriptum is present at birth or early childhood and is an uncommon dermatosis characterized by papules and small nodules that may coalesce to form plaques. Histopathologically, there are varying degrees of hyperkeratosis, papillomatosis, and irregular acanthosis. In the papillary dermis, greatly dilated capillaries are observed, The acanthotic epidermis encircles the vascular spaces(blood cysts) where, occasionally, organized thrombi may be found. The use of pulsed-dye lasers to treat cutaneous vascular lesions is based on the theory of selective photothermolysis. We report a case of an angiokeratoma circumscriptum in an 48-year-old woman for whom the flashlamp pulsed tunable dye laser proved to be a highly successful means of treatment.

Extramammary Paget's disease is a rare cutaneous malignancy and frequently associated with an underlying adnexal carcinoma and perhaps with underlying internal malignancy. In our case, genital Paget's disease was followed by the discovery of renal cell carcinoma. We report a rare case of extramammary paget´s disease of the penis and scrotum which associated with a renal cell carcinoma.

Alopecia areata (AA) is a recurrent, nonscarring type of hair loss. Although it is a medically benign condition, AA can cause great emotional distress to affected patients and their family. At this time, there are many treatments that have varying efficacies but there is no reliable cure. Topical anthralin treatment of AA has been found to be effective by some researchers. We experienced two cases of AA improved by topical anthralin therapy without serious side effects. Anthralin is much less expensive than most other topical therapies for AA and relatively well tolerated. In our experience, it is more effective in eliciting a cosmetic response than other topical treatments. Anthralin appears to be a reasonable therapeutic option for AA.

Cutaneous involvement has been reported in all types of malignant plasma cell disorders including multiple myeloma, solitary myeloma of bone, plasma cell leukemia, and extramedullary plasmacytoma. But metastatic plasmacytomas in the skin are rare in multiple myeloma and extramedullary plasmacytoma. If skin tumors appear during the course of multiple myeloma, these should be interpreted as a sign of poor prognosis. Treatment of patients with resistant multiple myeloma is challenging. Thalidomide has recently shown antitumor activity in patients with refractory myeloma. Until the introduction of thalidomide, no drugs other than cytotoxic agents and glucocorticoids had shown antitumor activity to cutaneous plasmacytoma. We recently performed a clinicopathologic study of a patient with cutaneous involvement of multiple myeloma and reported our result with thalidomide therapy in this patient.

Diffuse alopecia areata is the least common clinical type of alopecia, and this diffuse form lacks the characteristic hairless patches of alopecia and it begins as diffuse hair loss. Diffuse alopecia areata has been poorly characterized. In 2002, there was a suggestion to define this form of alopecia areata as acute, diffuse and total alopecia of the female scalp (ADTAFS). ADTAFS is characterized by a marked female predominance, tissue eosinophilia and a uniquely short clinical course. We report here on one case of ADTAFS.

BACKGROUND: Mohs micrographic surgery (MMS) is a specialized type of minimal marginal surgery that offers cure rates superior to other options in the treatment of skin cancer. But only a few reports have been published on the long term outcome of MMS in Korean literatures. OBJECTIVE: Our purpose was to report the clinical findings and recurrence rates of all patients with cutaneous malignancy treated with MMS. METHODS: We retrospectively evaluated 123 patients who had been diagnosed with cutaneous malignant tumors and had been treated with MMS and tumor reconstruction. The main outcome measures were patient demographics, duration of tumor, site, preoperative tumor size, method of reconstruction, and recurrence after MMS. RESULTS: The case series comprised 123 patients (51.2% female and 48.8% male) with a mean age of 65.8 years. Most tumors (78.0%) were on the face and 56.6% of all type of skin tumor were located on the left side. The most common skin tumor was basal cell carcinoma (67.5%). The local flap was the most common method to repair surgical defects (49.6%). Recurrence after MMS was diagnosed in 3 of the 123 patients (2.44%). CONCLUSION: MMS may be a safe and satisfactory method for the treatment of cutaneous malignant tumors and the low recurrence rate with MMS emphasizes the importance of margin controlled excision.
