2.A Clinical Observation on Spinal Epidural Abscess.

Twenty-two cases of non-tuberculous spinale pidural abscess treated at the Dept. of Neurosurgery, Kyungpuk University Hospital during the past 7 years from 1962 to 1969 were observed clinically. The results were summarized as follows: 1. The ratio between male and female was 2; 1. The peak age incidence of these cases was between 40 and 50 years old with 36% of patients and between 10 and 20 years old with 27% of patients falling into this range. 2. The most important source of infection was the furuncle or abscesses of the skin, which was responsible for 50% of total known cases. The most frequent site of infection was thoracolumber area, which was responsible for 50% of total cases. Among others, thoracic (32%) and lumbar (18%) areas were included. Bacterial culture was performed in 14 cases and staphylococcus was isolated in 11 cases. 3. As for clinical symptoms 12 cases revealed back pain, there showed fever, root pain and headache in some cases. Paraplegia or paraparesis observed in 19 cases, sphincter disturbance in 18 cases, sensory deficits in 17 cases, back tenderness and stiff neck were included in these groups. Pyogenic pus was aspirated in 3 cases through lumbar puncture from the epidural space. Manometric studies during the lumber puncture revealed a partial or complete blockage in 13 cases. The CSF analysis showed an increase of protein in 15 cases and an increase of cell count in 12 cases. 4. The operative findings were classified in three types. The type I, which was composed of pus only, was 4%, The type Ii of pus and granulation tissue was 50% and the type II of granulation tissue and adhesion was 36%. The postoperative prognosis has revealed to be excellent in 5 cases, good recovery in 8 cases and unimprovement of neurological deficits in 6 cases. Three cases succumbed to uncontrollable sepsis and pneumonia.

3.Treatment of the Acromioclavicular Dislocation with the Modified Weaver and Dunn Procedure

We treated 27 cases of the complete dislocation of the acromioclavicular joint with the modified Weaver and Dunn procedure described by Shoji et al, which is resection of the lateral end of the clavicle and bone block transfer of the coracoacromial ligament, from July 1987 to December 1989. Twenty-one patients were followed for an average of fourteen months and the brief summary of the observations are as follows: 1. The peak incidence was in 3rd and 4th decades and much more prevalent in male (76%). 2. The most common cause of the injury was traffic accident and followed by fall down. 3. In functional evaluation by the Weitzmann criteria, an excellent result was seen in fourteen cases; a good result, in five; a fair result, in two. 4. The comparision of the coracoclavicular interval ratio before surgery with that after surgery facilitated the evaluation of the effectiveness of the coracoclavicular ligament reconstruction. 5. There were no major complications that could affect to end result.

4.Echocardiographic Analysis of Systolic Mitral Valve Motion in Healthy Young Males: With Particular Reference to Mitral Valve Prolapse.

M-mode echocardiograms were recorded in 268 apparently healthy young male adults and the mitral valve motion during systole was analyzed. In 149 cases( 55.6%) out of 268 the predominant echo of mitral valve closure(CD line) was recorded as smooth line moving slowly anteriorly, in 48 cases(17.9%) the CD line showed an abrupt transient posterior motion during early systole, in 24 cases(9.0%) there were multiple additional echoes posterior to the CD line, 17 cases(6.3%) the mitral valve closure was recorded as pansystolic anteriorly concave echo deviating less than 2 mm from a drawn CD line, and 10 cases(3.7%) as curvilinear and anteriorly convex echo deviating from a drawn CD line. In the remaining 20 cases(7.5%), the mitral valve closure was recorded as pansystolic anteriorly concave echo deviating more than 2 mm from a drawn CD line, which was categorized to have mitral valve prolapse in the current study. However, there were no significant differneces in various echocardiographic dimensions of the heart and the aorta between those with and without mitral valve prolapse.

6.Comparison of Direct-labeling Method of Antibody with 99mTc and 188Re.

PURPOSE: We investigated the direct labeling method of antibody with 99mTc and 188Re and examined the stability and function of these labeled compounds in in vitro and in vivo. MATERIALS AND METHODS: Disulfide bond of nonspecific human IgG was reduced to -SH group by 2-mercaptoethanol. Stannous ion was used to reduce 99mTc and 188Re. The stability of 99mTc-IgG and 188Re-IgG was estimated upto 24 hrs. Biodistribution was evaluated in abscess bearing rats at 4 and 24 hr post-injection of 99mTc or 188Re labeled IgG. RESULTS: The number of -SH group per reduced IgG molecule was 2.34. The labeling yield of 99mTc-IgG and 188Re-IgG were 90% and 95%, respectively. The stability of 99mTc-IgG at 1, 4, 6 and 24 hr was 91%, 83%, 78%, 7% and that of 188Re-IgG, high uptake was found on kidney, blood, stomach and abscess (9.42+/-0.68, 1.43+/-0.24, 0.86+/-0.18, 0.72+/-0.10 %ID/g, respectively). The uptakes at 24 hr were kidney, abscess, stomach, and blood in descending order. In case of 188Re-IgG, high uptake at 4 hr post injection appeared on kidney, blood, abscess and stomach (3.92+/-0.62, 1.32+/-0.08, 0.88+/-0.01, 0.26+/-0.06, respectively). The upatkes at 24 hr were kidney, abscess, blood abd stomach in descending order. The abscess to blood uptake ratio of 99mTc-IgG was 0.5 at 4 hr and 2.02 at 24 hr and that of 188Re-IgG was 0.67 and 1.29. CONCLUSION: 99mTc-IgG and 188Re-IgG and 188Re-IgG canbe labeled efficiently with direct labeling method. However, 99mTc-IgG and 188Re-IgG, labeled with direct method, was unstable. Further study in needed to enhance the stability of the antibody labeling.

7.The Effect of Deacetylation of Chitin and Chitosan on Wound Healing in Rats.

Chitin and chitosan, polysaccharides made up of polymeric N-acetly glucosamine and D-glucosamine, are widely found in nature, forming the skeletons of crustaceans and insects, as well as the components of bacteria cell walls. Chitin and chitosan have been known to have many useful biological properties such as biocompatibility, biodegradability, antimicrobial activity, and wound healing. There properties have been known to be different depending on the degree of deacetylation of chitin, but it has not yet been fully evaluated. The purpose of this study was to evaluate the wound healing effect by the degree of deacetylation. Full-thickness skin incision was made on the backs of Sprague-Dawley rats. Three concentrations of powder, comprising 90% chitin, 50% chitin and 10% chitin, were separately embedded in the wounds of 3 rat groups. The wound-breaking strength and the collagen-hydroxyproline content of the skin at the wound sites were measured and histological examination was performed at postoperative 3, 7, and 10 days. The 50% chitin group had the highest tensile strenght of all groups. But the 50% chitin and 90% chitin groups had the lowest collagen hydroxyproline levels among all groups. The wounds treated with 50% chitin powder were completely reepithelialized and granulation tissue in the wound was observed 7 days after initial wounding. The arrangement of collagen fibers in the skin was ordered and similar to normal skin The 50% chitin powder is considered to be the most efficient wound healing accelerator among different concentrations of chitin powder.

10.Acute Motor Weakness of Opposite Lower Extremity after Percutaneous Epidural Neuroplasty.

Recently, percutaneous epidural neuroplasty has become widely used to treat radicular pain caused by spinal stenosis or a herniated intervertebral disc. A 19-year-old female patient suffering from left radicular pain caused by an L4-L5 intervertebral disc herniation underwent percutaneous epidural neuroplasty of the left L5 nerve root using a Racz catheter. After the procedure, the patient complained of acute motor weakness in the right lower leg, on the opposite site to where the neuroplasty was conducted. Emergency surgery was performed, and swelling of the right L5 nerve root was discovered. The patient recovered her motor and sensory functions immediately after the surgery. Theoretically, the injection of a large volume of fluid in a patient with severe spinal stenosis during epidural neuroplasty can increase the pressure on the opposite side of the epidural space, which may cause injury of the opposite nerve by barotrauma from a closed compartment. Practitioners should be aware of this potential complication.
