2.Scintimetric Evaluation of Femoral Neck Fractures by Tc-99m-MDP: Experimental Study in the Mature Rabbits

In femorsl neck fracture, viability of the femoral head depends on the revascularization of the femoral head. The purpose of this study is to observe the effect of early fixation and anatomical reduction by measuring the Tc-99m-MDP uptake ratio of the femoral head in experimental animsls. Femoral neck region of 20 mature rabbits were osteotomized completely and divided into 2 groups ; each group consisted of 10 rsbbits. Group 1 had two Kirschner wire fixation after osteotomy and anatomic reduction. Group 2 had no fixation after osteotomy. The uptake ratio of Tc-99m-MDP of the femoral head in each group were measured st the intervals of 24 hours, 48 hours, 1 week, 2 weeks, 3 weeks, and 4 weeks, respectively. The results obtained were as follows ; 1. In experimental group 1, the mean uptake ratio of the femoral head was 0.72±0.13 at 1 day after operation, increased to a level of 1.44±0.14 at 1 week, increased to a maximal value of 2. 90±0.36 at 3 weeks and decreased to a level of 1.10+0.12 at 4 weeks. 2. In experimental group 2, the mean uptake ratio of the femoral head was 0.70±0.10 at 1 day, 0.64±0.13 at 1 week, decreased to a maximal value of 0.33±0.05 at 3 weeks and 0.47±0.05 at 4 weeks, showing decreased level below 1.0. The results suggest that early anatomicsl reduction and rigid fixation of femoral neck fracture is helpful for revascularization of the femoral head and prevention of non union.

3.Malignant Osteoblastoma: A Case Report

Malignant osteoblastoma is very rare tumor which shows histological findings of an aggressive pattern and benign clinico-radiological features, and also called as aggressive or pseudomalignant osteoblstoma. The authors experienced one csse of malignant osteoblastoma involving diaphysis of left humerus in 22-year old man. The patient is healthy and maintains his normal activity without recurrence over five years after complete excision of cystic mass, curettage and bone graft.

4.Carpal Injuries

In carpal injury, a good functional end result seems to correlate with final anatomic alignment of carpus after treatment. Some cases are often misdiagnosed and result in inadequate treatment. Forty six cases of carpal injuries were analysed which were treated for 9 years from 1975 to 1984 at the Department of Orthopedic Surgery, Seoul National University Hospital. The observations can be summarized as follows. 1. The male to female ratio was 3.5: 1 and 82% of total cases were in the third and fourth decades. 2. The most frequent cause of injury was fall from height with outstretched hand. 3. Trans-scaphoid perilunar dislocation and its variant were the most common diagnosis in this analysis. 4. All of the scapho-lunate dissociation showed volar flexion intercalated segmental instability with scapho-lunate gap more than 2 mm in our series. 5. In 11 cases of trans-scaphoid perilunar dislocation and its variant, carpal collapse was found in 10 cases. In 5 cases dorsiflexion intercalated segmental instability pattern and in one case volar flexion intercalated segmental instability pattern were found to be present. 6, There was an increase in scapho-lunate angle in all cases of dorsiflexion intercalated segmental instability, but a decrease in scapho-lunate angle is not a consistent finding in volar flexion intercalated segmental instability. 7. In 24 cases more than 6 months follow-up, excellent and good results were obtained in 20 cases. 8. As a result of above findings, authors conclude that scapho-lunate gap more than 2mm has diagnostic value in scapho-lunate dissociation but that dorsiflexion intercalated segmental instability or volar flexion intercalated segmental instability only means the changes in the relationship between carpal bones and does not have definite or specific diagnostic value for a specific carpal injury.

5.Denture Cleansers.

Proper hygienic care of removable dentures is an important means of maintaining a healthy oral mucosa on denture wearers. Denture cleansing is often poor due to improper mechanical and the inefficient chemical cleansing of dentures. Dentists and patients should realize that microbial plaque on dentures may be harmful to both the oral mucosa and the patient's general health. This literature review was aimed to evaluate materials and methods for cleansing dentures and to discuss different means of keeping dentures plaque-free. A routine denture cleansing regimen should be designed to remove and prevent reaccumulation of microbial plaque and also to remove mucin, food debris, calculus, and exogenous discoloration. The combined use of chemical and mechanical cleansing is highly recommended for patients to clean their denture effciently.

7.The Surgical Outcome of Bilateral Lateral Rectus Recession in Large Angle Exotropia.

To evaluate the results of bilateral recession in large angle exotropia we examined medical records of 73 patients who had exotropia larger than 35PD and underwent bilateral lateral rectus recession. The mean of follow-up period was 16months. Successful alignment, defined as a final alignment of orthophoria, esotropia less than 5PD or exotropia less than 10PD was achieved in 44 patients(60.3%). There were no differences in age, refractive error, preoperative angle of deviation, prevalence of amblyopia, the presence and concurrent correction of vertical or cyclodeviation, the development of diplopia and the duration of follow-up period between the successful alignment group and failure group. The esodeviation on postoperative one-day tended to ensure a more satisfactory result, but it was not statistically significant. There were no statistically significant differences in stereoacuity and postoperative angle of deviation among the different fusional ability groups. The stereoacuity and postoperative angle of deviation did not show a positive correlation. However, on the two separate examinations of the stereoacuity and fusional ability, the latter results showed improvement in both parameters. This result suggests that the correction of exodeviation may play a role in improving binocular visual function in patients with large angle exotropia.

9.Magnetic Resonance Angiographic Findings and Clinical Outcome of the Striatocapsular Infarction.

BACKGROUND: Striatocapsular infarction involves the territory of the large lateral striate branches of the middle cerebral artery sparing the overlying cortex. Two kinds of angiographic findings of the striatocapsular infarction have been reported previously: one with a complete occlusion of the M1 segment and the other without. However, the clinical features and outcomes in relation to the magnetic resonance angiographic (MRA) findings remain unknown. In the present study, we evaluated the clinical outcomes in patients with striatocapsular infarctions according to the MRA findings. METHODS: Twenty-two consecutive patients with striatocapsular infarctions who were admitted to the Chosun University Hospital between July 1998 and June 2000 were included in the study. Striatocapsular infarction was confirmed by magnetic resonance imaging (MRI) and MRA. The patients were divided into two groups based on MRA findings. Ten patients with occluded MCA were included in group A, and the remaining 12 patients with normal or stenotic MCA were categorized as group B. We compared their clinical characteristics and outcomes at 3-months post-stroke. RESULTS: Only 1 out of 12 patients in group B and 6 out of 10 patients in group A showed cortical symptoms and signs. Patients in group A tended to have more severe disability and a worse prognosis than those in group B. CONCLUSIONS: The MRA findings may help clinicians to better predict the clinical outcomes after striatocapsular infarctions. (J Korean Neurol Assoc 19(5):452~456, 2001)

10.Surgical Experiences of Three Cases of Giant Pituitary Adenoma.

Three cases of giant pituitray adenoma are reported. Two cases were operated by transsphenoidal approach and a case by transcranial approach. Transsphenoidal approach showed satisfactory results despite of marked suprasellar extension of tumors. We report rare giant pituitary adenomas with review of relevant literatures.
