1.The Incidence of Pityrosporum Orbiculare and Pityrosporum Ovale on Normal Skin.

In microscopical survey of clinically normal skin, P. orbiculare was found to be present on scalp, face, anterior chest, back, arm and leg in 68.2%, 68.3%, 90.2%, 86.4%, 28.6% and 16.7% respectively, and P. ovale was present in scalp, face, anterior chest, back, arm and leg in 84.1%, 56.1%, 51.2%, 38.6%, 19.0% and 7.1%. P. ovale was dominant species in scalp, on the other hand P.orbiculare was dominant species in anterior chest and back. The large number of spore of P. ovale was present on scalp, on which P. ovale was dominant species, and on anterior chest and back, the number of spore of P. orbiculare was more increased than the numbers of P. ovale. The hyphae, identical in size and shape with those of Malassezia furfur, were found on scalp in one subject. In cultural survey, the pcsitive culture rate in P. orbiculare was showed in over all from 87.5% to 80.8% and in P. ovale from 33.3% to 88.2%. Germ tube formation on culture was found in 3 subjects on anterior chest and in one subject on back.

2.Ichthyosis Hystrix: A Case Report Treated with Etretinate.

Ichthyosis hystrix is characterized by extensive bilateral systemic involvement of thick hyperkeratotic verrucous papules forming confluent plaques or linear arrangement and is currently classified into a systematized epidermal nevi, which was once conaidered as an extreme variant of congenital ichthyosiform erythroderma. Here is presented a case with typical clinical features and characteristic histopathologic findings of epidermolytic hyperkeratosis. Our patient exhibited no skeletal or CNS abnormalities reported to be associated with this condition. Marked improvement was obtained with oral administration of etretinate.

4.Scintimetric Evaluation of Femoral Neck Fractures by Tc-99m-MDP: Experimental Study in the Mature Rabbits

In femorsl neck fracture, viability of the femoral head depends on the revascularization of the femoral head. The purpose of this study is to observe the effect of early fixation and anatomical reduction by measuring the Tc-99m-MDP uptake ratio of the femoral head in experimental animsls. Femoral neck region of 20 mature rabbits were osteotomized completely and divided into 2 groups ; each group consisted of 10 rsbbits. Group 1 had two Kirschner wire fixation after osteotomy and anatomic reduction. Group 2 had no fixation after osteotomy. The uptake ratio of Tc-99m-MDP of the femoral head in each group were measured st the intervals of 24 hours, 48 hours, 1 week, 2 weeks, 3 weeks, and 4 weeks, respectively. The results obtained were as follows ; 1. In experimental group 1, the mean uptake ratio of the femoral head was 0.72±0.13 at 1 day after operation, increased to a level of 1.44±0.14 at 1 week, increased to a maximal value of 2. 90±0.36 at 3 weeks and decreased to a level of 1.10+0.12 at 4 weeks. 2. In experimental group 2, the mean uptake ratio of the femoral head was 0.70±0.10 at 1 day, 0.64±0.13 at 1 week, decreased to a maximal value of 0.33±0.05 at 3 weeks and 0.47±0.05 at 4 weeks, showing decreased level below 1.0. The results suggest that early anatomicsl reduction and rigid fixation of femoral neck fracture is helpful for revascularization of the femoral head and prevention of non union.

5.Avulsion of the Tibial Attachment of the Posterior Cruciate Ligament of the Knee Joint (Seven Cases Report)

The posterior cruciate ligament plays as basic stabilizer of the knee. Therefore the knee becomes unstable when tibial insertion of the posterior cruciate ligament is avulsed. The diagnosis may be delsyed or missed because of surgeon's 1sck of awareness or rare condition. The authors treated seven cases of avulsion fracture of posterior cruciate ligsment at the tibial attachment ares ranged from ten to thirty-two years. There were five male and two female. The results were as follows : 1. The cause of all seven cases injury was traffic accident. 2. The physical findings were seven positive posterior drawer test, seven cases of abrasion or laceration over the anterior proximal tibial surface and antero-medial surfsce of the knee joint. 3. The associated injury was comminuted fracture of the tibia and femur (1 case), rupture of collateral ligsment and comminuted fracture of the tibia (1 case) and zygomatic fracture (1 case). 4. The treatment methods were four cancellous fixation, one K-wire fixation after epiphyseal plate fusion but each one pull out wire and dexon suture before epiphyseal plate fusion. 5. The motion of knee joint was within normal limit and no instsbility was inspected at the three months after reduction.

6.A Clinical Study of Femoral Neck Fracture

We analysed forty-six cases of femoral neck fracture treated with internal fixation and followed for more than one year at Korea University Hospital from January 1979 to June 1986 and the following results were obtained. 1. Sex distribution was 23 cases(50%) male and 23 cases(50%) female, and average age was sixty years old (range from 21 to 91 years). 2. The most common cause of injury was slip down(76%). 3. The most common type of fracture was Garden's type III(46%) and Pauwels' type III (67%). 4. In cases of internal fixation, the most commonly used method was closed reduction and multiple pinning under the local aneshtesia(83%). 5. Bony union was established within 5.9 months in average. 6. Among forty six cases, 7 cases(15%) of avascular necrosis, 4 cases(9%) of malunion, and 2 cases(4%) of nonunion was occurred. 7. The clinical results were excellent in twenty cases(43%), good in sixteen cases(35%), fair in seven cases(15%), and poor in three cases(7%) according to Lunceford's assessment.

7.Operative Treatment Of Burst Fracture On The Thoracolmbar Junction

We analyzed 41 cases of burst fractures on the thoracolumbar junction which were operated with Kaneda and Cotrel-Dubousset implant at Chosun University Hospital between 1989 and 1993. The purpose of this study was to evaluate the radiologic sign, the amount of reduction, complications, and functional results. The results were as follows: 1. According to McGrorys Criteria to evaluate the posterior superior vertebral body angle of burst fractures, 33 out of 41 cases(80.5%) were positive and the average angle degree was 107.6°. 2. The average postoperative kyphotic correction was 15.4° in the Kaneda group and 13.8° in the C-D group. The average loss of correction at follow-up examination was 5° in the Kaneda group and 4.8° in the C-D group. 3. Indirect reduction was achieved in 10 cases(50%) and we obtained a good indirect reduction even though canal compromise was over 50%. 4. The pain at operative site was much more severe in the Kaneda group(6 cases) than in the C-D group(2 cases) and both groups experienced 2 cases each of implant failure. 5. According to Denis' pain and work scale, 28 cases(63.8%) had good and excellent, 8 cases had fair and 5 cases had poor results. In summary we recommend doing 1) a posterior instrumentation first for stability and indirect reduction, if it is not a severe neurologic symptom and 2) anterior decompression if it is a severe or progressive neurologic symptom.

8.A Clinical Study of Microporous Coated Hemispherical Acetabular Cup

In order to observe the differences in fixability and stability of the acetabular cup through the differences in cup size in relation to the size of acetabular reaming, we analyzed 75 cases of porous coated acetabular cup of Harris-Galante type which be followed up more that two years. All cases were divided into two group ; the first group (On line fit :45 cases) used with acetabular cup equal in size to the last reaming size and the second group (Press fit : 30 cases) used with acetabular cup 2mm larger than the last reaming size. For initial stability, all cases were fixed with acetabular screws. Both groups were analyzed for presence of gap, change of acetabular inclination, presence of radiolucent line, degree of displacement of vertical distance, stability and clinical results. The results were as follows; 1. The average Harris hip score was improved from 55.8 points preoperatively to 90.2 points postoperatively in the first group, from 56.9 points to 91.2 points in the second group, Clinical results are excellent in 39 cases, good in 4 cases, fair in 2 cases in the first group, and excellent in 29 cases and good in 1 case in the second group. 2. The gap between the acetabular cup and acetabulum was found initially at 17 zones in the first group but no gap at final follow up. In the second group, the gap was found at 23 zones and finally remained less that 1 mm gap was found at 3 cases in zone B1. 3. No radiologic instability of acetabular cup by Martell evaluation was noted in both 1st and 2nd group. Our data suggests that initial gap between acetabular cup and acetabulum in microporous coated acetabular cup were replaced with new bone and also, radiologic stability and good clinical results could be obtained in the second group used with acetabular cup 2 mm larger than the last reaming size.

9.Subcutaneous Angiolymphoid Hyperplasia of the Inguinal Region and Anteromedial Side of the Proximal Thigh: A case report

Subcutaneous angiolymphoid hyperplasia is an uncommon disease characterized by eosinophilia, proliferation of the capillary vessels and lymphoid tissue and infiltration of the inflammatory cell, especially eosinophils. The etiology of the disease are obscure, but probably trauma, lower grade infection, nervous factor & hormonal status etc. The predilection sites are face, ears, scalf & neck, but rarely reported in the extremities. We experienced a case of subcutaneous angiolymphoid hyperplasia of the inguinal region and anteromedial side of the proximal thigh in a 10 year-old man who complained pain and tenderness on the above region. The patient was treated by excision with satisfactory result.

10.A Malignant Lymphoma of the Ilium: One Case Report
