1.The Biomechanical Study for the Initial Stability of Porous - coated Acetabular Components in Total Hip Arthroplasty.

The purpose of this study was to investigate the gaps of bone-acetabular component interface, the initial mechanical stability and the effects of adjuvant screw fixation in the press-fitted two different shaped porous-coated acetabular components. Semi-hemispherical Harris-Galante II cup and full hemispherical Trilogy cup were press-fitted with or without adjuvant screw fixation in 40 bovine femur. The gaps of bone-implant interface were measured in each radiography of all specimen and axial compression tests and rotational stability tests were performed using servohydraulic tension-compression machine. There was significantly smaller gap at polar 1/3 region in the specimen implanted with Trilogy cup. But the adjuvant screw fixation did not affect the amount of gap. There was no significant statistical difference in axial load and rotational stability test in each situations of fixation. This study demonstrates that the initial mechanical stability is not influenced by the shape of semi-hemispherical Harris-Galante II cup and full hemispherical Trilogy cup and also independent of adjuvant screw fixation. But smaller gap of Trilogy cup at the polar 1/3 region indicates the possibility that less amout of wear debris of polyethylene or metal will be deposited and therefore less osteolysis or cup loosening will occur, but this hypothesis should be confirmed by the long-term clinical follow-up.

2.The Study of Motion of Two Components in Bipolar Hemiarthroplasty

The bipolar hemiarthroplasty is one of the treatment methods of the avascular necrosis of the femoral head and the fracture of the femur neck. The mechanism of bipolar hemiarthroplasty is intended to distribute the forces at the site of the inner and outer articulation, and theoritically diminish acetabular wear and produce a painless arthroplasty. The authors analyzed motion of two components in 23 cases of the bipolar hemiarthroplasty, performed at the Department of Orthopaedic Surgery, Korea University Hospital, between January, 1985 and January, 1993 with average 47 months follow up(range 24 to 84 months). The results obtained were as follows: 1. In the motion of the two components, inner bearing motion was average 11.5 degrees(from 1 to 29 degrees) and outer bearing motion was average 21.4 degrees(from 3 to 45 degrees). 2. The inner bearing motion was highest at 12 months with average 16 degrees and outer bearing motion was lowest during the same period with average 15 degrees. During the follow up the inner bearing motion was decreased whereas outer bearing motion was increased. 3. The inner or outer bearing motion of bipolar component was no correlation with age, sex and clinical result.

4.A Case of Transient Acantholytic Dermatosis.

We report herein a case of transient acantholytic dermatosis. The patient was a 24-year-old pregnant woman in the last trimester, who had pruritic, discrete, erythematous or brownish papules on the lateral aspect of the neck. Histopathologic findings of the skin lesion showed changes resembling Darier's disease showing well circumseribed, small foci of acantholysis with acantholytic and dyskeratotic cells over the suprahasal clefts. The patient showed good response to isotretinoin(Roaccutane) therapy, 20mg/day, for 30 days.

5.A case of onychogryphosis associated with onychomycosis.

We present a case of onychogryphosis of both great toe nails associated with onychomycosis of all toe nails occurring in a 90-year-old female patient. She also had senile dementia. Affected nail plates were thickened, increased in length, and curved like the horn of a ram. The nail plates were removed surgically form the nail bed. Direct microscopic examination revealed many hyphae with potassium hydroxide. Trichophyton rnbrum was isolated from the culture in Sabouraud glucose agar media. On histopathologic examination, we found many hyphae in the nail plate. We suggest an onychomycosis as one of the causes of onychogryphosis.

6.The Prevalence and Some Minor clinical Features of Atopic Dermatitis.

BACKGROUND: The incidence of atopic of atopic dermatitis (AD) varies widely by different ages and it has been well-founded that the epidemiology of AD has been studied on admission of the first grade schoolchildren. AD is a common skin disease of childhood and tends to increase significantly during recent decades, but informations regarding the exact prevalence of AD, based on the large-scale general population study, are unavailable in korea. OBJECTIVE: The purpose of this study is to elucidate the exact prevalence of AD and to evaluate the diagnostic significance of some well-known minor clinical features associated with AD in the first grade school children in one of Kouean cities, Puchon, kyunggi-do. METHODS: Four thousands and eighteen first grde school children (boys ; 2072, girls ; 1946) participated inthis study. Two dermatologists determined the presence of skin lesions by through skin examination. Diagnosis of AD was made only when typical flexural and/or extensor and/or infraauricular fissure/eczema exist. The frequency as well as their diagnostic significance of ten minor features of AD (xerosis, ichthyosiform skin lesion, hyperlinear palm,)keratosis pilaris, non-specific hand/foot dermatitis, cheilitis, nipple eczema, perifollicular accentuation, pityriasis alba, Dennie-Morgan infraorbital fold) thought suitable items for field survey by authors, were also evaluated. RESULTS: The total prevalence of AD in the first grade school children of Puchon city was 3. 78%. The prevalence in boys was 3.37% and that in girls was 4.2 Associated minor features in individuals diagnosed as having AD were, in order of frequency, pityriasis alba (59.8%), perifollicular accentuation (57.8%), , keratosis pilaris (56.5%), hyperlinear palms (55.2%), xerosis (43.3%), Dennie-Morgan infraorbital fold (26.3%), etc. All but the ichthyosiform skin lesions and nipple eczema were significantly more frequent in individuals diagnosed as having AD (P<0.01). CONCLUSION: A relatively low prevalence shown in this study might be due to rather strict diagnostic criteria. Some helpful and diagnostically significant minor clinical features of AD, suitable for field survy, is suggested. Further studies are needed for accurate estimation of the prevalence of AD in korea including different regional and age population.

10.Total Hip Replacement of Pathologic Fracture in Fibrous Dysplasia involving Proximal Part of the Femur

The fibrous dysplasia, divided into 3 categories of the monostotic, polyostotic and extraskeletal forms, is a relatively rare disease which was first reported by Well in 1922 and shows a characteristic findings replacing the medullary cavity of noe or more bones by the pathologically proliferating fibrous connective tissues. In a consequence, the structural integrity of the diseased bone becomes weakened and suffers frequent episodes of pathologic fracture and deformities of the bone, which are most common around the hip. Various operative methods and instruments have been tried to manage the pathologic fracture around the hip recently. The authors report a satisfactory resnlt of total hip replacement for the pathologic fracture in a patient of the fibrous dysplasia involving the proximal half of the right femur.
