3.Role of Computed Tomography in Blunt Chest Trauma.

PURPOSE: In patient with blunt trauma of chest, supine AP x-ray cannot differenciate the lung contusion, laceration, atelectasis, and hemothorax definitely. Therefore, computed tomographic evaluation is needed for accurate evaluation of the injuries. In our knowledge, there are few reports about CT findings of blunt chest trauma, in our country, therefore we tried to fiud the characteristic CT findings in patients with blunt trauma. MATERIALS AND METHODS: We analyzed the plain x-ray and CT image of 4 patient with blunt chest trauma. Location and morphology of lung parenchymal contusion and laceration, hemopneumothorax, chest wall injuries and location of chest tube. RESULTS: Lung parenchymal contusion was noted in 53 segments. of 16 patiants'infiltration(n=27 segment), and multiple nodular pattern was noted in 15 segment, pattern of consolidation along the lung periphery was seen in 11 segment. Laceration was noted in 18 lesion and most commonly located in paravertebral area(b=8). CONCLUSION: CT scan of chest in patient with blunt chest trauma, provides accurate informations of the pattern of injuries, and localization, therefore, should be performed as po9ssible.

5.A Histologic Classification of Diffuse Interstitial Lung Disease and Its Clinical Significance.

Diffuse interstitial lung disease(DILD) is a large group of heterogeneous diseases that diffusely involve the pulmonary connective tissues, principally subpleural, interlobular and alveolar wall portions. Terminology and classification of these diseases are not uniform, and the cause and the pathogenesis are unknown in many entities. It is generally accepted that the response to therapy is related to the relative degree of cellularity and fibrosis present, therefore a histologic evaluation of the relative extent and severity of these changes is required. We reviewed 52 cases of DILD from January 1990 to May 1995 diagnosed by open lung biopsy to reappraise classification and quantitative assessment of the histopathologic features. Differential histopathologic features between usual interstitial pneumonia(UIP) and nonspecific interstitial pneumonia(NIP) were examined with a correlation of HRCT findings and clinical findings. Among 52 cases of DILD, 18 cases(34.6%) were UIP, 6 cases(11.5%) were hypersensitivity pneumonia was NIP was 5 cases(9.6%), interstitial lung diseases associated with the connective tissue diseases were 5 cases(9.6%), inorganic dusts were 4 cases(7.7%), infections were 4 cases(7.7%), durgs were 2 cases(3.8%), acute interstitial pneumonia was 1 case(1.9%), sarcoidosis was 1 case(1.9%) in order of frequency plus 6 other cases(11.5%). UIP was the most frequent DILD in this study which seemed to be a criteria bias of patient selection on open lung biopsy. Quantitative assessment of histopathologic features was useful in the differential diagnosis of DILD and differentiation of UIP from NIP was possible based on histopathologic features and supported by HRCT. UIP disclosed a significantly high score of fibrotic changes, especially in the interstitial fibrosis, smooth muscle proliferation and honeycombing, otherwise NIP appeared relatively high score in inflammatory changes. Correlation between histopathologic scores and the clinical outcome after steroid therapy or no therapy in UIP was not evident.

6.The Use of Ender Nails in Fractures of the Tibial Shaft

Twenty one cases of adult tibial shaft fractures were fixed with flexible intramedullary nails at the Department of Orthopedic Surgery, Kang Nam General Hospital. The results are summerized as follows; l. Average healing time was 17 weeks. No case of nonunion was experienced. 2. As the operation was perfomed by closed method under fluoroscopic control, reduction of operation time is a major advantage. 3. Fixation was usually strong enough to allow immediate weightbearing and mobilization of the adjacent joints without external immobilization. 4. Three cases of delayed union and 2 cases of rotational malunion were experienced, but shortening and angulation were not so severe as to affect overall functional results. 5. In conclusion, Ender nailing is considered to be a valuable method in treating tibial shaft fractures. Even in cases with multiple injuries and in selected open cases, Ender nailing can be a saft procedure.

7.Intraductal Papillary Mucinous Neoplasm of the Bile Ducts.

Intraductal papillary mucinous neoplasms of the bile ducts (IPMNs-B) are uncommon lesions that are characterized by innumerable papillary fronds that contain fine vascular cores, enriched mucin production and bile duct dilatation. IPMNs-B are histologically similar to intraductal papillary mucinous neoplasms of the pancreas and they are occasionally associated with hepatolithiasis. IPMNs-B are considered to be relatively low-grade malignancy and they merit consideration for aggressive surgery. Thus, early and precise diagnosis is important to maximize patient survival. From July 2002 to March 2006, we identified four patients with IPMNs-B at our hospital. In three patients, intrahepatic or extrahepatic bile duct stones were associated with their condition. Computed tomography and magnetic resonance cholangiography were done in all four cases and this demonstrated marked dilatation of the biliary tree. Endoscopic retrograde cholangiography was done in two cases and a large amount of mucin that was draining from the patulous orifice of the duodenal papilla was seen on endoscopy. However, two cases were initially misdiagnosed as intrahepatic cholangiocarcinoma or choledochal cyst with intrahepatic bile duct stones. All the cases underwent ipsilateral hemihepatectomy with caudate lobectomy. Histologically, one case showed to be adenoma, one case was borderline and two cases were invasive adenocarcinoma.

9.Collision Tumor in the Gastric Stump after Radical Subtotal Gastrectomy.

Collision tumors is rare entities in which two distinct primary tumors develop in contiguity. They may arise as independent tumors in the same organ or in adjacent organs. The occurrence of a collision tumor in the stomach, consisting of adenocarcinoma and neuroendocrine tumor, is extremely rare. We report a case of a patient, who had undergone a radical subtotal gastrectomy for gastric cancer 3 year earlier, and an ulcerating tumor of the gastric stump had developed. A total resection of the remnant stomach was performed, and the pathological examination revealed a collision tumor consisting of an adenocarcinoma and a neuroendocrine tumor. The coexistence of both adenocarcinoma and neuroendocrine tumor of the gastric stump has never been reported. The authors present this case and a review of the relevant literature.

10.An Experimental Study on the Effects of Intraarticular Microparticles on Degenerative Change of Articular Cartilage of Rabbit Knee Joints.

Microparticle induced inflammatory reaction has been extensively studied as a potential cause of implant loosening. However, there has been little in vivo study on the effect of the particles on the preserved cartilage in partial joint replacements. The purpose of this study was to determine in vivo effects of microparticles on the articular cartilage. Ninety rabbit knee joints were challenged repeatedly with 1-3 micron commercial pure titanium (Ti) and 1-45 micron ultra high molecular weight polyethylene (PE). After 12 weeks, patella and distal femur were harvested for determination of degenerative change by light and scanning electron microscope. The results are as follows: 1, In the joints without exposure of the subchondral bone, neither Ti nor PE elicited significant change. However, simultaneous introduction of both particles resulted in significant degeneration of the articular cartilage. 2. With the subchondral bones exposed, Ti and PE both induced significant cartilage degeneration. In this condition the PE particles were more detrimental than the Ti particles in causing degeneration of the articular cartilage. Although there exists a species difference, these results imply that the longevity of partial joint replacements may be shortened not only by mechanical problems, but also by the microparticles causing secondary degenerative change.
