1.A Clinical Observation on Spinal Epidural Abscess.

Twenty-two cases of non-tuberculous spinale pidural abscess treated at the Dept. of Neurosurgery, Kyungpuk University Hospital during the past 7 years from 1962 to 1969 were observed clinically. The results were summarized as follows: 1. The ratio between male and female was 2; 1. The peak age incidence of these cases was between 40 and 50 years old with 36% of patients and between 10 and 20 years old with 27% of patients falling into this range. 2. The most important source of infection was the furuncle or abscesses of the skin, which was responsible for 50% of total known cases. The most frequent site of infection was thoracolumber area, which was responsible for 50% of total cases. Among others, thoracic (32%) and lumbar (18%) areas were included. Bacterial culture was performed in 14 cases and staphylococcus was isolated in 11 cases. 3. As for clinical symptoms 12 cases revealed back pain, there showed fever, root pain and headache in some cases. Paraplegia or paraparesis observed in 19 cases, sphincter disturbance in 18 cases, sensory deficits in 17 cases, back tenderness and stiff neck were included in these groups. Pyogenic pus was aspirated in 3 cases through lumbar puncture from the epidural space. Manometric studies during the lumber puncture revealed a partial or complete blockage in 13 cases. The CSF analysis showed an increase of protein in 15 cases and an increase of cell count in 12 cases. 4. The operative findings were classified in three types. The type I, which was composed of pus only, was 4%, The type Ii of pus and granulation tissue was 50% and the type II of granulation tissue and adhesion was 36%. The postoperative prognosis has revealed to be excellent in 5 cases, good recovery in 8 cases and unimprovement of neurological deficits in 6 cases. Three cases succumbed to uncontrollable sepsis and pneumonia.

2.Treatment of the Acromioclavicular Dislocation with the Modified Weaver and Dunn Procedure

We treated 27 cases of the complete dislocation of the acromioclavicular joint with the modified Weaver and Dunn procedure described by Shoji et al, which is resection of the lateral end of the clavicle and bone block transfer of the coracoacromial ligament, from July 1987 to December 1989. Twenty-one patients were followed for an average of fourteen months and the brief summary of the observations are as follows: 1. The peak incidence was in 3rd and 4th decades and much more prevalent in male (76%). 2. The most common cause of the injury was traffic accident and followed by fall down. 3. In functional evaluation by the Weitzmann criteria, an excellent result was seen in fourteen cases; a good result, in five; a fair result, in two. 4. The comparision of the coracoclavicular interval ratio before surgery with that after surgery facilitated the evaluation of the effectiveness of the coracoclavicular ligament reconstruction. 5. There were no major complications that could affect to end result.

3.A Case of Epidural Abscess in Middle Cranial Fossa Secondary to Acute Sphenoid Sinusitis.

Acute or chronic sinusitis is common disease of ENT. Most cases usually respond to appropriate treatment; however, some cases occasionally progress to the development of serious complication and poor prognosis. The orbital complications are most common complication fallowed by intracranial complications. The incidence of sinogenic intracranial complications is about 3.7-11.0% and these complications are progressively decreasing however the complications occur due to incorrect use of antibiotics, antibiotics resistant pathogens, decreasing host immunity, improper diagnosis and delayed surgical treatment. Therefore the case of epidural abscess in anterior portion of middle cranial fossa secondary to acute sphenoid sinusitis, which was treated with transnasal endoscopic sphenoidotomy and intravenous antibiotics, is being reported.

4.A Case of Anhidrotic Ectodermal Dysplasia with Atrophic Rhinitis.

Anhidrotic ectodermal dysplasia is a rare genetic disorder characterized by absence or diminished numbers of structures derived from the ectoderm, and it is reported to be inherited as an x-linked recessive trait. It is recognized clinically by anhidrosis, hypotrichosis, anodontia or reduced numbers of teeth with deformed shape and characteristic facial features. In addition, otolaryngological manifestations include atrophic rhinitis, sensorineural hearing loss, and conductive hearing loss and satyr ear, among others. Early diagnosis of anhidrotic ectodermal dysplasia can prevent fatal hyperpyrexia and appropriate genetic counseling can be followed to make a reasonable future plans for the pediatric patient. A 2-month-old infant was referred with symptoms of intermittent nasal obstruction and crust formation in both nasal cavities. The nasal endoscope demonstrated atrophic changes of nasal mucosa and radiologic study showed an unerupted conical shaped tooth. The diagnosis of anhidrotic ectodermal dysplasia was confirmed with the finger impression test that revealed deficiency of sweat pores. We report a case of anhidrotic ectodermal dysplasia with a review of the literature.

5.A Case of Neurilemmoma of the Lateral Wall of the Hypopharynx Treated with Endoscopic Removal.

Neurilemmoma is a relatively rare benign tumor originating from the Schwann cells of all nerve fibers outside the central nervous system, with the sole exception being the optic and olfactory nerves. About 25-40% of neurilemmoma occurs in the head and neck region and the acoustic nerve is most frequently involved. But the tumor rarely affect the hypopharynx. In this paper, authors report a case of neurilemmoma of the lateral wall of the hypopharynx in a 61-year old female patient. The tumor was successfully removed via endoscopic approach. So, we report the case with literature.

6.Right Pulmonary Artery Agenesis: A case report.

Unilateral pulmonary artery agenesis is a rare congenital malformation usually associated with other cardiovascular anomaly such as Tetralogy of Fallot. Isolated pulmonary artery agenesis is very rare, and usually asymptomatic. It is usually highly suspected by routine chest X-ray, and associated symptoms are hemoptysis, blood tinged sputum, repeated pulmonary infection, and dyspnea on exertion. We have recently experienced the right pulmonary artery agenesis in 27 year-old male patient, complaining of minimal hemoptysis and sustained blood tinged sputum. He was successfully treated by right pneumonectomy, so we report this case with the review of associated literature.

Peutz-Jeghers syndrome is a familial syndrome consisting of mucocutaneous pigmentation and gastrointestinal polyposis and appears to be inherited as a single pleiotropic autosomal dominant gene with variable and incomplete penetrance. Cases of hamartomatous polyps of the Peutz-Jeghers type without Peutz-Jeghers syndrome have only rarely been reported. Moreover, only one case of a Peutz-Jeghers polyp at the appendix has been reported; it was resected by appendectomy. We report here on a case of a 45 year old man who had a hamartomatous polyp of the Peutz-Jeghers type arising from the appendix. The polyp was successfully removed by endoscopic polypectomy. To our knowledge, this is the first case of a hamartomatous polyp of the Peutz-Jeghers type that originated from the appendix and that was resected endoscopically.

9.The Effect of Different Suturing Techniques on Astigmatism after Penetrating Keratoplasty.

The purpose of this study was to compare the effects on astigmatism after penetrating keratoplasty with three different suture techniques. In this prospective study, 38 eyes of 38 patients underwent penetrating keratoplasty with three suturing techniques: interrupted, single running, and double running. Topographic astigmatism was measured at 2, 6, 12, and 18 months after keratoplasty. During 18 months after surgery, the interrupted suture group had higher astigmatism than the double running suture group. There was no significant difference in the amounts of astigmatism during the first year after keratoplasty between the single running and the double running suture group. However, at 18 months after surgery, when all sutures were out, the double running suture group (3.60+/-1.58 diopters) showed significantly less astigmatism than the single running group (5.65+/-1.61 diopters). In conclusion, post-keratoplasty astigmatism was the least in the double running suture group of the three suturing techniques at 18 months after penetrating keratoplasty.

10.Clinical Manifestation and Predisposing Factors of Infectious Keratitis Following Penetrating Keratoplasty in Korean Patients.

PURPOSE: To report the clinical manifestation, predisposing factors, microbiological profiles and treatment outcome of infectious keratitis following penetrating keratoplasty (PK). METHODS: Medical records of the post-PK patients later diagnosed with culture-positive keratitis, between January 2003 and June 2008 at our hospital were retrospectively reviewed. RESULTS: Among 228 eyes of 226 patients who previously had PK, 18 eyes (7.89%) of 16 patients developed microbial keratitis. Fifteen patients had a bacterial infection, of which a Streprococcus species was the most common causative microorganism (6 eyes, 33.3%). Three eyes had fungal infection; one case was co-infected with bacteria. Six eyes (33.3%) presented with a suture-related problem, and sixteen eyes (88.9%) had been using topical glaucoma medications. The suture-related problem and use of glaucoma medication were significantly associated with the development of infectious keratitis (p=0.040 and 0.013, respectively). Remission was achieved in all cases within the mean duration of 2.47 months after treatment initiation. However, visual improvement was not achieved in 11 eyes (68.7%) due to graft opacity. CONCLUSIONS: Early identification of predisposing factors and appropriate management at an early stage may prevent the occurrence of graft infection and improve graft survival.
